Spinal cord stimulation and physical therapy have helped a man paralyzed since 2013 regain his ability to stand and walk with assistance. The results, achieved in a research collaboration between Mayo Clinic and UCLA, are reported in Nature Medicine. With an implanted stimulator turned on, the man, Jered Chinnock, was able to step with a front-wheeled walker while trainers provided occasional assistance. He made 113 rehabilitation visits to Mayo Clinic over a year, and achieved milestones during individual sessions: Total distance: 111 yards (102 meters) — about the length of a football field Total number of steps: 331 Total minutes walking with assistance:16 minutes Step speed: 13 yards per minute (0.20 meters per second) “What this is teaching us is that those networks of neurons below a spinal cord injury still can function after paralysis,” says Kendall Lee, M.D., Ph.D., co-principal investigator, neurosurgeon and director of Mayo Clinic's Neural Engineering Laboratories. In the study, Chinnock’s spinal cord was stimulated by an implanted electrode, enabling neurons to receive the signal that he wanted to stand or step. “Now I think the real challenge starts, and that’s understanding how this happened, why it happened, and which patients will respond, says Kristin Zhao, Ph.D., co-principal investigator and director of Mayo Clinic’s Assistive and Restorative Technology Laboratory. Currently, as a safety precaution, Chinnock takes steps only under the supervision of the research team.

Shane Shapiro, M.D., orthopedic physician at Mayo Clinic in Florida, discusses a regenerative medicine clinical research trial to treat knee arthritis, which is the bone marrow stem cell treatment (BMAC) for knee osteoarthritis. Mayo Clinic and the Mayo Center for Regenerative Medicine is studying biologically based non-surgical treatments for osteoarthritis. One such treatment is the harvesting of the patient's own stem cells from their bone marrow. "In our procedure we draw cellular rich bone marrow from both sides of the pelvis. We then filter the resulting product and concentrate the stem cells and their corresponding growth factors. Using an ultrasound to image the knee joint, we are then able to precisely inject the cells into the arthritic knee. We are currently demonstrating that this procedure is safe and can relieve pain. Researchers at Mayo Clinic’s campus in Florida have conducted the world’s first prospective, blinded and placebo-controlled clinical study to test the benefit of using bone marrow stem cells to reduce arthritic pain and disability in knees. It is the first time that the belief that stem cells can provide substantial and possible regenerative relief in an ailing joint has been put to the test in such a rigorous fashion. The researchers say such testing is needed because there are at least 600 stem cell clinics in the U.S. offering one form of stem cell therapy or another to an estimated 100,000-plus patients, who pay thousands of dollars, out of pocket, for the treatment, which has not undergone demanding clinical study. The findings in The American Journal of Sports Medicine represent another first —patients not only had a dramatic improvement in the knee that received stem cells, but also in their other knee, which also had painful arthritis but received only a saline control injection. Each of the 25 patients enrolled in the study had two bad knees, but did not know which knee received the stem cells. Given that the stem cell-treated knee was no better than the control-treated knee — both were dramatically better than before the study began — the researchers say the stem cells’ effectiveness remains somewhat uninterpretable. They are only able to conclude the procedure is safe to undergo as an option for knee pain, but they cannot yet recommend it for routine arthritis care. “Our findings can be interrupted in ways that we now need to test — one of which is that bone marrow stem cell injection in one ailing knee can relieve pain in both affected knees in a systemic or whole-body fashion,” says the study’s lead author, Shane Shapiro, M.D., a Mayo Clinic orthopedic physician.
