DKA and HHS (HHNS) Nursing | Diabetic Ketoacidosis Hyperosmolar Hyperglycemia Nonketotic Syndrome

DKA and HHS (HHNS) nursing NCLEX lecture review of the treatment, patient signs/symptoms, and management. Diabetic ketoacidosis and hyperosmolar hyperglycemia nonketotic syndrome are two complications that can present in diabetes mellitus. DKA is more common in type 1 diabetics, whereas, HHNS is more common in type 2 diabetics. Patients with diabetic ketoacidosis will present with ketosis and acidosis and signs/symptoms will include hyperglycemia (greater than 300 mg/dL), Kussmaul breathing, fruity (acetone breath), ketones in the urine, and metabolic acidosis. Patients with hyperglycemic hyperosmolar syndrome will NOT have ketosis or acidosis but EXTREME hyperglycemia (greater than 600 mg/dL). In addition, hyperosmolarity will present which will cause major osmotic diuresis and the patient will experience with severe dehydration. Decerebrate and decorticate posturing rigidity NCLEX review with mnemonic and pictures on how to tell the difference between the two conditions. Decorticate vs decerebrate posturing are two ABNORMAL types of rigidity associated with a brain injury. Let’s look at the difference between the two: Decorticate posturing: this is a type of flexed posturing and can indicate damage to the cerebral hemispheres. There will be adduction and flexion of arms and hands will be closed shut (flexed). The legs will be rotated internally and feet plantar flexed. Mnemonic for decorticate posturing: Remember the letters COR in the word decorticate for the word “core”. The patient will bring their ARMS to the core of the body (middle). Decerebrate posturing: this is a type of extended posturing and can indicate damage to the brain stem. This is the worst type of posturing between the two. There will be adduction and extension of the arms and pronation of the hands and fingers will be flexed along with extended legs and plantar flexion of the feet. Decerebrate posturing: Look at all the E’s in this word. There are a lot of them, so remember the word EXTENDED. The arms are going to be extended rather than flexed. Hyperglycemic hyperosmolar nonketotic syndrome (HHNS or HHS) nursing diabetes & pathophysiology NCLEX review on endocrine disorders of the body. HHNS vs DKA are two complications of diabetes mellitus. HHNS presents with extreme hyperglycemia (blood glucose greater than 600 mg/dL) and dehyrdation. Ketones and acidosis are not present in this condition as with diabetic ketoacidosis. The hyperglycemia hyperosmolar state can lead to a coma and even death if not treated promptly. This is an NCLEX hotspot practice question example about a nursing assessment for the drug Digoxin (Lanoxin). For this particular NCLEX practice question, you will have to identify what area on the diagram you will assess prior to administering Digoxin. Before a nurse administers Lanoxin, it is important to assess the apical pulse for 1 full minute. The apical pulse must be 60 beats per minute or greater. Therefore, to answer this question correctly you must identify on the diagram where the apical pulse is located on an adult patient. The apical pulse is located at the apex of the heart and is found at the 5th5th intercostal space midclavicular line. This location is also the point of maximum impulse. This video is part of a weekly NCLEX review series where I will be going over NCLEX-style questions with you. I will be helping you analyze and breakdown each question, and walk you through how to select the correct option. Incentive spirometry (spirometer) demonstration with step-by-step instructions on how to use this device. As a patient or nurse, it is important you know the proper procedure for using an incentive spirometer. An incentive spirometer is used to help improve lung function in patients and prevent atelectasis, especially prior to surgery. Many times a patient is educated how to use an incentive spirometer prior to surgery that involves the chest, thorax, or abdomen. These patients are at high risk for developing a condition called atelectasis. Atelectasis is a condition where the lung or part of it is collapsed and the alveoli sacs are unable to inflate and perform gas exchange. By using an incentive spirometer prior to and after surgery, it decreases the chances of a patient developing this condition. DKA diabetic ketoacidosis nursing management pathophysiology & treatment. DKA is a complication of diabetes mellitus and mainly affects type 1 diabetics. DKA management includes controlling hyperglycemia, ketosis, and acdidosis. Signs & Symptoms include polyuria, polydipsia, hyperglycemia greater than 300 mg/dL, Kussmaul breathing, acetone breath, and ketones in the urine. Typically DKA treatment includes: intravenous fluids, insulin therapy (IV regular insulin), and electrolyte replacement. Crohn's and ulcerative colitis are two types of inflammatory bowel diseases that cause inflammation and ulcers formation in the GI tract. Crohn's disease affects the whole bowel lining to the serosa. Rh incompatibility in pregnancy nursing NCLEX maternity review: management, pathophysiology, Rhogam shot, and hemolytic disease of the newborn. What is Rh incompatibility? It occurs when a father who is Rh-positive and a mother who is Rh-negative have a baby that is Rh-positive. The baby's Rh-positive blood will leak into mom’s circulation (most likely during delivery), and this will cause the mother's to build antibodies. The baby of the first pregnancy is not typically harmed. However, the baby during the second pregnancy, who is Rh-positive, will be affected because those antibodies created during the first pregnancy are in mom's circulation. These antibodies will attack the baby's red blood cells. This will lead to hemolytic disease of the newborn and cause: anemia, jaundice, heart failure, swelling/edema, splenomegaly along with hepatomegaly. The key is to keep mom from being sensitized, which is done through giving her a medication called Rhogam. This medication is given at 28 weeks and again at 72 hours after delivery if the baby is Rh-positive. Apical pulse assessment and location demonstration for nurses: Where is the location of the apical pulse? It is found on the left side of the chest in the 5th intercostal space at the midclavicular line. The apical pulse is also the location of PMI (point of maximal impulse) and is at the apex of the heart. As the nurse, you will be assessing the apical pulse during a cardiac assessment and before administering certain types of cardiac medications, such as Digoxin (cardiac glycoside). A normal apical pulse is 60-100 bpm in an adult. Prioring to auscultating the apical pulse, it is important to palpate the apical pulse. The apical pulse may not be palpable in all patients, especially if the patient has a thick chest wall (obesity etc.). Compartment syndrome nursing NCLEX review practice question on the nursing interventions and neurovascular assessment for this condition. Compartment syndrome occurs when too much pressure is exerted within the muscle compartments found within the fascia. This can occur when there is hemorrhaging (bleeding) or swelling present after an injury, like with a bone fracture (or with external factors like a cast being too tight or traction). All this can increase the pressure within the compartments. As the pressure builds, this will cut off the blood supply and nerve function to this muscle. If not corrected within 6 hours, the damage is permanent. Remember from anatomy and physiology that in the leg and arm there are individual compartments grouped together (but separated from one another) that contain bone, muscle, nerves, and vessels. Each compartment usually has its own muscle, nerve, and vessel supply. Fascia is what keeps all these structures in place and separated. The important thing to remember about fascia when talking about compartment syndrome is that is does NOT expand when pressure increases within a compartment (so there will be no relief within the compartment from the fascia). Instead the pressure stays within the compartment and causes blood vessel and nerve function to become compromised (diminished). So, ischemia is going to occur to the muscle and distal extremity to the fracture. Nursing interventions include: keeping the extremity AT HEART level (NOT below....remember you want to maintain arterial pressure and elevating it above heart level will cause more ischemia), loosen and remove restrictive items, notify the physician, perform neurovascular checks (6 P's), prepare the patient for possible bivalvement of the cast, reduction of weight in the traction, or in severe cases fasciotomy. NCLEX practice question on how to answer a NCLEX priority question that deals with requiring immediate nursing action. This video is part of a weekly NCLEX review series where I will be going over NCLEX style questions with you. I will be helping you analyze each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. This specific question is a type of question that wants to know based on your morning assessment findings which finding is abnormal and requires immediate nursing action. NCLEX Question: You have just received nursing report from the previous shift and are performing your morning patient assessments. You have a total of 4 patients that are either post-opt or pre-opt for surgery. Which assessment finding requires further nursing action? A. Orange-colored urine in a patient who is taking Pyridium and is post-opt day 3 from a TURP. B. No stool excretion in a patient who is post-opt day 2 from a colostomy. C. Shoulder pain in a patient is who is post-day 1 from a laparoscopic cholecystectomy. D. Pain rating that has decreased from a 10 to 0 in a patient who is awaiting an appendectomy. Chronic bronchitis vs emphysema nursing lecture on the pathophysiology, treatment, and symptoms. In this video, I review the similarities between emphysema vs chronic bronchitis and highlight the differences. The biggest differences between chronic bronchitis and emphysema are the pathophysiology and disease specific symptoms. In chronic bronchitis, there is excessive production of mucous and inflammation of the bronchioles. However, in emphysema there is the loss of elasticity of the alveolar sacs. Both conditions lead to respiratory acidosis. However, there is a V/Q mismatch in chronic bronchitis and a matched V/Q defect in emphysema and because of this the body compensates differently. In chronic bronchitis (also called blue bloaters) the patient will have cyanosis and bloating (from hyperinflation and late effects of cor pulmonale). In emphysema (called pink puffers), the patient will have hyperventilation and a barrel chest (no cyanosis due to the hyperventilation). Both conditions are irreversible, caused by inhaling an irritants (mainly smoking), and treated with bronchodilators, corticosteroids, theophylline, or Phosphodiesterase-4 inhibitors. How to Prime IV Tubing Line | How to Spike a IV Bag for Nursing

How to prime IV tubing line (primary infusion) and how to spike a IV bag for nursing. The purpose of spiking an IV bag and priming IV tubing is to administer IV therapy to a patient by allowing the fluid from the bag to transfuse into the patient's blood stream. As the nurse, you will be doing this several times per day. It is standard nursing care because during a patient's hospitalization they will at some point receive IV fluids, medications, blood products, or fat emulsions. What does spiking the IV bag mean? It means you will be penetrating the IV bag with the spike of the IV tubing so the contents in the bag can flow into the IV tubing and then into the patient's blood. What does priming the IV tubing mean? It means you will be allowing the solution in the bag to flow through the tubing to remove any air in the line. It is very important to remove any air or bubbles from the tubing before infusion so an air embolism can be avoided. When you perform this nursing skill, it is important to remember to label the IV tubing when the IV therapy was initiated, because it expires. The CDC recommends changing IV tubing that is used continuously and not used for blood, blood products, or fat emulsion (TPN, Lipids) every 96 hours. It is very important to follow your hospital's protocol which are based on the CDC's recommendations. Some hospital require you change IV tubing every 72 hours. In addition, if the IV tubing is used for the administration of blood, blood products, or fat emulsions it should be changed every 24 hours. Again, follow your hospital's protocols. Lung sounds for assessment and auscultation of bronchial, bronchovesicular, and vesicular breath sounds for nursing and NCLEX review. This video includes the audio (breath sounds) and description for each type of normal breath sound. Each breath sound is made up of a different pitch and duration along with being auscultated in different areas of the chest. Bronchial lung sounds are heard in the tracheal area and are loud, high pitched sounds with a slightly longer expiration than inspiration. Bronchovesicular breath sounds are medium pitched sounds and are heard in the bronchi located anteriorly (1st and 2nd intercostal space next to the sternum) and posteriorly (between the shoulder blades) on the chest. The inspiration and expiration are equal. Vesicular breath sounds are heard throughout the peripheral lung fields and are soft, low pitch sounds. The inspiration is slightly longer than the expiration. Don't forget to watch the video on abnormal breath sounds! Pericarditis | Nursing Management, Treatment of Pericardial Effusion, Friction Rub,  & Pericardium

Pericarditis nursing NCLEX review on the nursing management, causes, signs and symptoms (pericardial friction rub), complications (pericardial effusion and cardiac tamponade), and treatments (pericardiocentesis or pericardiectomy). Pericarditis is the inflammation of the pericardium layer of the heart. The pericardium is a double layered sac that contains the fibrous pericardium and the serous pericardium. The serous pericardium is made up of two layers called the parietal pericardium and the visceral pericardium, which is also know as the epicardium. In between the parietal and visceral layer is the pericardial space that contains serous fluid called pericardial fluid. Causes of pericarditis include illness, trauma, autoimmune or idiopathic. There are two types of pericarditis, acute and chronic. Acute lasts for less than 6 weeks. While chronic pericarditis lasts more than 6 months. Chronic pericarditis can lead to constrictive pericarditis which is thickening of the pericardium that constricts heart filling. This leads to signs and symptoms of heart failure. Treatment for this is a pericardiocentesis. Signs and symptoms of pericarditis include: pericardial friction rub, fever, chest pain, pain while lying in the supine position, and ST segment elevation on an EKG. Treatments for pericarditis include aspirin or NSAIDS or Colchicine or corticosteroids. NCLEX pharmacology practice question on Insulin for the patient with diabetes mellitus. This NCLEX-style question will test your knowledge about Insulin Lispro (Humalog) and wants to know when the patient is at most risk for hypoglycemia based on the time you administered the medication. On the NCLEX exam, it is inevitable you will receive pharmacology type questions. Abnormal lung sounds that include crackles (formerly called rales), stridor, wheezes (formerly called rhonchi), pleural friction rub, and stridor. There are two types of abnormal breath sounds such as discontinuous and continuous. Discontinuous sounds include: coarse crackles, fine crackles, and pleural friction rub. Continuous sounds include: low-pitched, monophonic wheeze & high-pitched, polyphonic wheeze, and stridor. Fine crackles sound like high-pitch, crackling noises mainly on inspiration and can't be cleared when coughing. Coarse crackles are low-pitch, wet bubbling sounds that can occur mainly during inspiration but can extend into expiration. Pleural friction rub is a harsh, grating sound heard during inspiration and expiration. Low-pitched, monophonic wheezes are heard mainly in expiration and have a low-pitched whistling tune or whine with ONE sound quality. High-pitch, polyphonic wheezes are heard mainly in expiration and have a high-pitched musical instrument tune with MORE THAN ONE sound quality. Stridor is heard during inspiration and is a high-pitched whistling or gasping sound with a harsh sound quality.
