Choosing the Right Surgical Weight Loss Procedure for You: Gastric Bypass, Sleeve Gastrectomy OR Gastric Lap Band Patients considering weight loss surgery often ask "which procedure is right for me?" At the New Jersey Bariatric Center all three surgical weight loss procedures -- gastric bypass, sleeve gastrectomy, or gastric band -- result in successful weight loss. The best way to make the decision regarding which procedure is best for you is to consider several factors: loss of pounds, reversibility, diet requirements and risk, and compare the pros and cons of them against your personal goals. Weight loss: Initial weight loss with the gastric bypass is approximately 3lbs to 5lbs per week for the first three to six months. Over 18 months, the gastric bypass procedure results in approximately 70-80% excess weight loss. However, gastric bypass is both a restrictive (restricts the intake of food) and mal-absorptive (less absorption of calories you take in) procedure resulting in weight loss from both decreased food intake and decrease calorie absorption. Weight loss after sleeve gastrectomy is in between that of the gastric bypass and gastric band. Patients can expect to lose an average of 2lbs to 3lbs per week for the first three to six months. Over 18 months, the sleeve gastrectomy results in approximately 60-70% excess weight loss. With the gastric band procedure weight loss is slower, about 1lbs to 2lbs a week. Over 18 months it will approximately result in a 50-60% excess weight loss. Since the Gastric Band is only restrictive, and not mal-absorptive, it requires much more personal will power to avoid high calories and snacking. In addition, Gastric Band requires significant and consistent follow up for the first year to adjust the band. Keep in mind that a slower weight loss does not necessarily mean less 'hanging skin,' nor does faster weight loss result in more 'hanging skin' problems. These issues are a factor of the amount of weight loss and your age, not the amount of time it takes to lose the weight. Reversible: Unlike the gastric bypass and sleeve gastrectomy, the gastric band procedure is reversible. At any time, you can choose to have the band removed. Keep in mind, patients who have the band removed often gain the weight back over a short time period. Diet: Regardless of the procedure, patients are required to eat healthy and exercise to maintain the weight loss. However, when comparing the procedures, the gastric bypass requires less dietary compliance as it combines both the restrictive and mal-absorptive components. With the bypass you simply can't intake the same amount of food, high caloric food or eat to quickly without getting physically ill (dumping syndrome). However, due to the mal-absorption component of the bypass, patients must take protein and vitamin supplements for life. Dumping syndrome can occur after gastric bypass surgery resulting in dizziness and heart palpitations. It occurs 15 to 30 minutes after a high carbohydrate meal, especially one that's rich in sugars such as sucrose (table sugar) or fructose (fruit sugar). Some people also experience low blood sugar (hypoglycemia), related to excessive levels of insulin delivered to the bloodstream as part of the syndrome. Insulin influences your tissues to take up the sugar present in your bloodstream. Risk: To date, at the New Jersey Bariatric Center we have no mortality and below average complications for both procedures. However, when comparing the three procedures and looking at the national statistics the gastric band has a significantly lower risk of death than the gastric bypass procedure since it is less invasive. Making the decision: Ultimately, the decision to have the procedure is entirely up to you. After reviewing all the information and attending one of the free educational seminars at New Jersey Bariatric Center, you must decide if the benefits outweigh the side effects and potential complications. This surgery is only a tool. Your ultimate success depends on strict adherence to the recommended dietary, exercise and lifestyle changes. After the successful results of weight-loss surgery, most patients enjoy an improvement in obesity-related health conditions such as diabetes, heart disease, mobility, self-image and self-esteem. The goal is to live better, healthier and longer. The key is not to be pressured to pick a procedure by anyone, including your medical doctor, your family or a friend. Do the research yourself and learn the pros and cons of both the procedures. In the end, the decision is yours, it's your life and you will need to be comfortable with what you choose.

The ReShape gastric balloon procedure is a new FDA approved, non-surgical approach to weight loss that is proven to help people achieve and maintain significant weight loss. New Jersey Bariatric Center patients who undergo a gastric bypass operation typically lose between 65 percent and 70 percent of their excess body weight. For someone who is 150 pounds overweight, that means a weight loss of about 100 pounds after gastric bypass. How Gastric Bypass Works Gastric BypassGastric Bypass works by changing the patient’s anatomy, creating a smaller stomach pouch and rerouting the path that food normally takes so that it “bypasses” a large section of the stomach and a portion of the small intestine. New Jersey Bariatric Center surgeons perform the gastric bypass procedure laparoscopically through approximately four or five incisions in the abdomen. The surgeons staple off a section of the stomach, leaving the rest of the stomach in place and creating a pouch that can hold about a quarter-cup of food. The small intestine is also divided and connected to the new smaller pouch. Food bypasses the stapled-off section of the stomach and the upper portion of the small intestine. The result is an early sense of fullness and satiety that reduces the desire to eat. Not only do patients who’ve undergone gastric bypass surgery lose weight because they simply cannot eat as much, but they also absorb fewer calories. This dual approach – restrictive (restricts the amount of food) and malabsorptive (reduces the amount of calories absorbed) – gives gastric bypass twice the weight-loss punch as other procedures.

When the REALIZE Band™ is implanted, it molds the stomach into two connected chambers: a small upper stomach and the lower stomach. The new upper stomach can only hold about 4 ounces [1/2 cup] of food. For this reason, you will feel full sooner than usual. The lower stomach does not need to fill for you to feel full. The Gastric Bypass procedure combines the restrictive aspect of the Lap Band procedure with a malabsorptive component. The remainder of the stomach is not removed, but is completely stapled shut and divided from the stomach pouch. The connection or outlet from this newly formed pouch empties directly into the lower portion of the intestine (jejunum), thus bypassing calorie absorption. This is done by dividing the small intestine just beyond the first part of the intestine (duodenum) and constructing a connection with the newly formed stomach pouch. The other end is connected into the side of the Roux limb of the intestine creating the "Y" shape. The length of either segment of the intestine can be increased to produce lower or higher levels of malabsorption. ABOUT LAPAROSCOPIC GASTRIC BYPASS Gastric Bypass works by restricting the amount of food a patient can comfortable eat and also reducing the absorption of calories. During a gastric bypass, the surgeons staple off a section of the stomach, leaving the rest of the stomach in place and creating a pouch that can hold about a quarter-cup of food. The small intestine is also divided and connected to the new smaller pouch. Food bypasses the stapled-off section of the stomach and the upper portion of the small intestine. TYPICAL WEIGHT LOSS 60% -- 70% of excess weight at 18 months after surgery. KEY CONSIDERATIONS Bypass patients absorb fewer calories but also absorb fewer vital nutrients, so they will need to take protein and vitamin supplements for life. Considered a "good" side effect, dumping syndrome in bypass patients can occur as a result of the rapid emptying of stomach contents into the small intestine. It is triggered when too much sugar or large amounts of food are consumed. The results are unpleasant and can include nausea, weakness, sweating, faintness and, on occasion, diarrhea after eating. For most patients, this syndrome acts as training mechanism. Once you know the behaviors that trigger dumping, you avoid them and eat in a way that is will maximize weight loss. The surgery is irreversible. RECOVERY & FOLLOWUP Surgery takes approximately two to three hours to perform. On average, our patients leave the hospital two days after gastric bypass surgery. Return to work varies from one to three weeks depending on the type of work. No heavy lifting (more than 10 lbs.) is allowed for four to six weeks after surgery.
