Los Gatos, CA - Plastic surgeon Dr. Kamakshi Zeidler says Angelina Jolie's public disclosure of her preventive double mastectomy is helping to raise public awareness of breast cancer and new breast reconstruction techniques. Dr. Zeidler is participating in a study of a new breast tissue expander used to prepare patients for breast reconstruction. The experimental tissue expander, which is temporarily implanted allows the patient to control the rate of tissue expansion in preparation for permanent implants. Patients participating in the study say this gradual rate of expansion is more comfortable and more convenient than the conventional method of breast tissue expansion which requires repeated visits to the doctor's office for incremental injections of saline. Fascia and skin may get tight (scarring) after mastectomy surgery and radiotherapy. Denise speaks about the fascia techniques that work and the fascia techniques that may not work to loosen the tight tissue that can develop after mastectomy. Fascia treatment is described in detail, to help women understand the nature of gentle treatments to improve the body's tight (but should be soft) tissue. Often the term used for tight tissue on the chest is .... scar tissue or radiotherapy fibrosis. Again, often, the woman is told to learn to live with this.

A mastectomy results in a very big scar on the chest and may interfere with the balance of the muscles in the shoulder. What are the types of breast cancer? Breast cancer can begin in different areas of the breast -- the ducts, the lobules, or in some cases, the tissue in between. In this section, you can learn about the different types of breast cancer, including non-invasive, invasive. The first term is the non-invasive cancer. In fact, it means in the same place, this refer to the cancer not yet developed the ability to leave the block of the breast. It's not able to invade into the surrounding tissue. A discovery could soon produce real benefits for women suffering from a lethal strain of breast cancer. "Utilizing this genetic information about how the cancer really comes assembled will help us pick and choose what are the optimal treatments for the future," says Dr. Scott Dunbar, oncologist with Lee Memorial Health System's medical staff. As researchers explore cancer at its cellular level, they looked at the makeup of a form called triple negative breast cancer. "Typically on the breast cancer cell we expect to see some expression of the female hormones estrogen and progesterone on the surface of the cell that suggest a better nature to the cell. When neither estrogen and progesterone are present at all on the surface of the cell and the last protein that regulates growth is also missing, these are felt to be very immature or very aggressive types of cancer cells," says Dr. Dunbar. Triple-negative tumors account for 10-15% of all breast cancers and are more common in younger women and African Americans. Today, they're treated like most other breast cancer patients. But that could change. "It appears that these cells look and appear to have the same or many of the same genetic abnormalities that are associated with ovarian cancer, which may allow of management and treatment of these particular woman with chemotherapy regimens which have been developed to treat ovarian cancer," says Dr. Dunbar. This finding came out of a massive genome study that mapped every mutation of breast cancer. If this triple-negative breast cancer is genetically the same as ovarian cancer, it may mean these breast cancer patients will benefit from less toxic treatment. "So this may actually afford more treatment options for woman that historically have had a very difficult time or a poor prognosis related to that particular type of cancer," says Dr. Dunbar. A new study raises concerns of a possible association between cell phone radiation exposure and breast cancer in young women. The research team, led by Dr. Lisa Bailey, a former president of the American Cancer Society's California Division and one of California's top breast surgeons, studied four young women -- aged from 21 to 39 years old -- with multifocal invasive breast cancer. The researchers observed that all the patients developed tumors in areas of their breasts next to where they carried their cell phones, often for up to 10 hours per day, for several years. None of the patients had a family history of breast cancer. They all tested negative for BRCA1 and BRCA2 -- breast cancer genes linked to about one-half of breast cancer cases -- and they had no other known breast cancer risks. Imaging of the young girls' breasts revealed a clustering of multiple tumor foci in the part of the breast directly under where their cell phones touched their body.

Dr. Steeg argues for a different set of trials. One for primary mets prevention in people who have micromets but no metastatic disease. The other would be for those with limited metastatic disease The endpoint wouldn't be shrinkage of mets you already have ("Go get radiation for that"), but timed to the development of NEW mets. "Now doesn't that just make sense?" asks Dr. Steeg to thunderous applause. NCI's Pat Steeg makes a case for redesigning clinical trials to do "phase II randomized metastasis-prevention trials." Currently, phase I and phase II clinical trials are done in patients with advanced, refractory metastatic cancer, patients who have had MANY therapies. In phase II trials, researchers typically are trying to determine if a drug shrinks metastases. "But a drug that PREVENTS metastasis may not SHRINK a large, refractory tumor," says Steeg. "It has a different mechanism of action that is NOT picked up by the clinical trial system."
