This is a compilation of the various involuntary muscle movements in my neck, face, eyes, arms, legs, hands, feet I've been having for several months. These movements get worse when relaxed, for example at bedtime, but also appear periodically throughout the day. They also vary depending how I am sitting or laying down. MRI results were negative, neurologists and movement disorder specialist seems to think this is stress/anxiety related and was worsened by surgery I had in November 2012, where I had a temporary pin surgically inserted to repair a broken finger. Tried Clonidine and Gabapentin with no success. Took Ambien for 4 months to be able to fall and stay asleep but made me groggy and unable to think clearly. Currently only taking 15mg of Remeron at bedtime to help with sleep and anxiety. Thought it was worth posting in case anyone has experienced something similar or has any suggestions on possible diagnoses or treatments.

Mild seizure Update on my sweet boy: 9.16.11-2.5.13 Loved and never forgotten. Thank you to all the wonderful people out there who viewed our videos and gave us their support. I hope this synopsis will be helpful to those out there dealing with similar issues. These videos were posted to help my boy and his doctors, locally and across the country, diagnose his disease. Our hope was the videos in connection with all the testing data we accumulated would help him and other dogs. I first recognized his symptoms in May of 2012. He did not have SM or hydrocephalus. His MRIs were surprisingly unremarkable given his condition and his breed. His sire is German/English with incredibe lines and extensive health testing; likewise, his American dam, again from great lines and quite healthy. His breeder too pains to breed out of the line and was wonderful, fully transparent and could not have been more supportive. As you see from the early videos he began around 8 months with something we called episodes. They were, most likely, focal seizures. Within weeks he experienced grand mal seizures that quickly progressed into cluster seizures. All drugs known to suppress seizure activity were tried and many in combination. Sometimes they would work for a week to 10 days but the seizures always returned. We also tried experimental drugs and holistic options. We were trying to wait for an experimental procedure but, he got too sick. In the end, he lost all voluntary muscle control and he didn’t seem to have any cognitive function—I couldn’t watch him suffer any more. After he passed, an autopsy was performed to try to get to the root of his symptoms. Nothing abnormal was present but the repeated and prolonged seizures were found to have completely destroyed his frontal lobe accounting for his debilitated condition in the last days of his life. We miss him terribly but he is at peace. If you have a dog with seizures, see a neurologist and take videos. The combination is your best hope.

Metatarsalgia is classically described as pain felt at the front of the foot on the plantar surface between the third and fourth metatarsals with: Occasional sharpness Tingling Numbness Exacerbation by walking barefoot, especially on a hard floor Exacerbation with flexing the foot Feels like there is a pebble in your shoe when walking This pain can be attributed to compression from tight shoes, subluxation of the metatarsal heads, foot deformities, excessive weight bearing, or other compressive forces placed on the foot in a repetitive motion such as running or jumping or standing for long periods of time. When palpating the foot, you may also inspect for calluses or corns around the second metatarsal head and or a high or dropped arch of the foot. What else can present like Metatarsalgia? Metatarsalgia is often differentially diagnosed with Morton's Neuroma and Sesamoiditis, because of similar symptoms. With Morton's Neuroma the pain will be shifted over just a little to the second and third metatarsals, and sometimes a small mass can be palpated where the nerve has been entrapped. Sesamoiditis will present with pain on the plantar surface near the bottom of the great toe due to direct trauma of the flexor hallucis brevis muscle. Treatment of Metatarsalgia: Treatment of metatarsalgia can vary depending on how chronic the condition is. In rare cases surgery to realign the metatarsal bones may be an option. If you're considering foot surgery, discuss the benefits and risks with your doctor. Conservative treatment is preferred, especially for people who are actively involved with high impact sports and exercising, resting the feet temporarily will alleviate the pain and stress put on the foot. Ice can help with the pain as well for about 20 minutes. Some sources recommend an insole to provide cushion to the foot, I would not suggest an orthotic or an insole without first consulting with an orthopedist because the foot is already under compression and does not need any extra compression. The most effective form of treatment would be to address the scar tissue that has built up over time from overuse and adjust any subluxations of the metatarsals. Once you are able to walk without pain and the scar tissue has healed, the next phase of treatment would be to keep stretching the muscles of the plantar fascia in the foot and to strengthen them to prevent future injury. Warm ups for Metatarsalgia: Warm up of the muscles is important and can be done with simple arch stretching and continued on with the strengthening program. Exercises to strengthen the muscles associated with metatarsalgia will include laying a resistance band flat on the ground and scrunching it up with the foot flat on the ground, and simple inversion and eversion with a resistance bands. Approximately 6-8 weeks of treatment will be needed to decrease the pain and strengthen the muscles. How the Performance Place addresses Metatarsalgia: At Performance Place Sports Care, we approach injuries in a conservative manner that will give the patient the most weighted benefits. Our providers are board certified chiropractors and certified in full body Active Release Technique. We examine the patient and determine the most efficient and quickest treatment plan for them. Our goal is not only to get the athlete back out on the field doing what they love to do, but to also enhance their athletic performance. We address the scar tissue that is built up from micro tears in the tissue due to overuse or previous injuries and develop a program to strengthen and train the muscles to be used functionally. Most all of our patients find results within a few treatments with decreased pain, and increased range of motion, strength, and balance. I wonder why it has not been more widely publicized, considering how effective it seems to be. (Hmmmmmm) I am using "Girlie" footage as the theme just as I did with my other Vitamin C video relating to my use of ascorbic acid to nearly completely eliminate herniated disc pain. If you don't like it, I'm sorry...but the previous bikini video is my all-time-highest views video, and has helped a lot of people thanks to Morgan in that bikini. The most important things you must learn on your own, and use preferably in conjunction with a respected doctor, are the types of C to use (I personally would never ever use calcium ascorbate for example) and then how to come down off the high doses very very slowly, over the course of weeks. Why? Because "rebound scurvy" is very real, does affect some people who come off C too quickly, and is a painful misery like none other. There are other concerns with using high doses of vitamin C that relate to iron issues in the blood, as well as oxalates and kidney stones, among other complications. In other words, this is a serious modality not to be played with lightly, and to be done, as I will repeat, only under the guidance and supervision of your doctor. I feel strongly this kind of use of vitamin C should be temporary. I use isolated nutrients as supplements only in a few instances. All other nutrients I get from my food. I presented this video because of all the prescription drugs being marketed all over TV lately relating to shingles. I take naproxen, norco, gabapentin, and Valium (1/2 of 2mg) every 8 hours I can take the Valium every 6 hours if need be...but seem to be able to go 8 hours depending on what is going on around me. I have bursitis in my right shoulder right now I do stretches as described in the video I remember the other stretch where I sit in a swivel chair and put my right hand flat on a table (I'm my case stove) and turn until it hurts then come back a little till it don't hurt...anyways...I go back Friday the 14th to see if I'm doing better and he will be changing my stretches and exercises probably...we will see middle of my two week home treatment my son decided to pull on that arm out of frustration and try and make me punch myself...weird right? But painful and kinda set me back to where I was when I started this pt treatments...so who knows what will happen Friday... Anyway questions or comments? Patients and methods: MS patients with walking impairment deemed candidates for treatment with PR-fampridine tablets were included in this case series. Clinical assessments included the Expanded Disability Status Scale (EDSS), Timed 25-Foot Walk (T25FW), 12-item Multiple Sclerosis Walking Scale (MSWS-12), EuroQoL-5D, and the Fatigue Severity Scale (FSS). The T25FW was videotaped at each visit. Assessments were performed at baseline and after 4 weeks of treatment with PR-fampridine tablets 10 mg twice daily. Clinical benefit of treatment was defined as any improvement in T25FW or MSWS-12 score at 4 weeks. Patients who demonstrated clinical benefit continued treatment and were assessed at 3 and 6 months. Results: Among all patients (N = 67; mean MS duration, 16.5 years; mean EDSS score, 4.8; mean T25FW, 13.9 seconds), 65, 52, and 48 completed the 4-week, 3-month, and 6-month visits, respectively. After 4 weeks, 50.7% and 32.8% of patients walked $10% and $20% faster, respectively; and in 65.7% of patients, MSWS-12 scores improved. Three patients experienced adverse events (nausea, n = 2, insomnia, n = 1) that resulted in discontinuation of treatment. After 6 months, 38.8% and 16.4% of patients walked $10% and $20% faster versus baseline, respectively; and in 59.7% of patients, MSWS-12 scores improved. Among patients who demonstrated clinical benefit of treatment at 6 months, FSS scores improved on average by 1 point and MSWS-12 scores by 10 points. Three case studies showing different outcomes of PR-fampridine treatment are detailed with a visual depiction of the changes observed. Conclusion: In this case series, a proportion of patients demonstrated a clinical benefit of PR-fampridine treatment on walking. Determining which patients derive benefit from PR-fampridine is an important aspect of treatment. A range of clinical and patient-reported factors should be considered when assessing the clinical benefit of PR-fampridine treatment in MS patients. What are they and how can I recognize them in a chart? It's one thing to have diabetes but if you didn't know, diabetics tend to have other organ problems because it affects the entire body. And there's a few manifestations that you need to be aware of. The first one you're going to see is renal manifestations which means anything that's happening to your kidneys, your urinary tract. So your kidneys, your bladders, your bladder, not bladders. People with CKD or Chronic Kidney Disease, they can get that because they are diabetic. These are just... this microalbuminuria, that just means they're spilling this protein type thing. Proteinuria means they're spilling protein. And nephropathy, not to be confused with neuropathy. This is nephropathy so that's the kidneys. The labs, you'll see with people that have renal manifestations are going to be BUN, creatine and CMP. The creatine, you see almost always they're checking in diabetics. They're constantly checking their creatine level. Another thing you'll see is they'll be on dialysis. And you know, they wouldn't have to be on dialysis if they didn't have renal function problems because of their diabetes. And that is V45.11. You'll have to code that pretty often with... if your person's having dialysis. 250.5, ophthalmic manifestations. When you have diabetics that start having problems with their eyes, they get cataracts and glaucoma. Retinopathy, blindness. It's very common to have problems with your eyes if you're diabetic. And I remember when I was working for medical records, I went to work one day and I kept looking at the clock across the room and it was blurry. It wasn't the day before and then it was and it was just... I just knew there was something wrong with my sugar. And I went to the doctor and I was telling all the nurses. And one of the doctors finally told me, he said... because if you're in medical records, they're at your fingertips. You can ask them, you know? And they said you know, if it was affecting your eyes, if you're having problems with your sugar and it was affecting your eyes, it's already too late. And he said, "You know, you should just consider that it's time to get glasses." And that was actually the problem. But you'll see that they'll have eye drops. But you know, I was just telling them, it's not that I'm getting older. I just... I can't see anymore. Ophthalmic conditions or consultations that... if a diabetic is having problems with their eyes, say they're... things are starting to look foggy because they're getting cataracts, they're going to send them straight to an ophthalmologist to have that checked because the PCP isn't going to follow them for ophthalmic conditions. So you'll see these consultations. Neurological manifestations, 250.6. This is where you know, this is your nerves. So people who have problems, they're diabetic, they have... their feet ache, their hands ache. Their feet are always cold. They get tingling sensations and stuff. That is because of the neuropathy and that's... you know, in my opinion, that's one of the first things to go. Now I could be wrong but as I'm reading charts and stuff of geriatric patients, it seems like... if they're diabetic, the first things to start going is the neurological manifestations. But what some people don't realize is this gastroparesis, that you know, that is actually... if you break the word down, gastro is your stomach or your intestines and it paralyzes parts of your digestive system. And it's all because it's neurological. It's nerves. I just learned that at a webinar the other day. I had never thought about that.
