Long-term care (LTC) is in need of a major reform as living facilities are filling up fast, and the cost of care is rising making it hard for individuals to afford the care they need. This is especially true since Medicare and Medicaid are the main drivers in determining what long-term care services an individual can receive. Extended care applies not only to older adults (those over 65 years), but also other individuals that need assistance with any activities of daily living. Due to the baby boomer population starting to reach the age of needing assistance with care, the United States will need to figure out how to accompany and care for this influx in LTC population. The government is already considering ways to do this such as encouraging families to plan and save for retirement earlier, and looking into making long-term care insurance more affordable. Additionally, there is an increase in the amount of community based care versus institutionalized care; helping alleviate the problem of overcrowding in facilities and allowing LTC patients to be an active part of the community. As time goes on, it is becoming more important that changes are made to LTC policy and technological advances are implemented into the long-term care system.

In Dividing Military Pensions and Benefits Part 2, we covered formulas for splitting retirement pay for services members recommended by the Uniform Services Former Spouses’ Protection Act (or USFSPA). With this blog posting we are covering some of the benefits and privileges you may continue enjoy even after divorcing a military service member. Continuing Military Benefits. Commissary and Post Exchange (“PX”) privileges. An un-remarried former spouse of a military service member is entitled to commissary and PX privileges to the same extent and on the same basis as the surviving spouse of a retired member of the uniformed services. This means that if you had commissary and PX privileges at the time of divorce, you can continue to receive these privileges as long as you remain un-married. The USFSPA uses the “20/20/20” test to determine eligibility for commissary and PX privileges. This is to say that the service member spouse has 20 years of credible service, the marriage lasted 20 years, and there is a 20 year overlap between the marriage and credible service. Medical Benefits. There are three categories of health care coverage for former: 1) full military health care coverage, 2) transitional healthcare for up to a year after the divorce, and 3) the DOD Continued Heath Care Benefit Program. To qualify for the full military health care program: the former spouse MUST remain un-remarried; the former spouse must satisfy the “20/20/20” rule; and the former spouse cannot be enrolled in an employer-sponsored health plan. To qualify for the transitional health care plan (1 year of coverage) the requirements are similar to the requirements for the full military health care program except that the former spouse satisfies the “20/20/15” rule instead of the “20/20/20” rule. To qualify for a second year of coverage the former spouse must have enrolled in the DOD Continued Heath Care Benefit Program. To qualify for the DOD Continued Health Care Benefit Program (or “CHAMPUS”) you must be losing entitlement to a military health care program such as divorce from a military service member, non-career military personnel (and family members) who are about to lose coverage. It is important to note that there is a time limitation and cost associated with this coverage. For more information about CHAMPUS coverage, please consult your military health care administrator. Designated Survivor Benefit Plan (“SBP”) Beneficiaries. Generally military retirement pay terminates when the military service member dies. The SBP acts a life insurance policy of sorts that makes up for the loss of military retirement pay to eligible survivors such as spouses (or former spouses) and children, in the form of a monthly payment. Generally, a spouse’s coverage under the SBP expires upon divorce. However, there are states that allow for divorce decrees to direct service members to elect their former spouses as SBP beneficiaries. This requirement and other drivers in the public health system, such as national voluntary accreditation, have increased focus on these investments and on the partnerships between hospitals and their local public health system. Build ripped muscle like Zac with this program… http://athleanx.com/x/built-like-zac-year-round Zac Efron’s workout and diet plan are making waves this Summer because of his incredible physique transformation preparing for Baywatch. Along side the Rock, Zac holds his own, having carved out a chiseled body by focusing on 7 key elements of his training and nutrition. Recently, Men’s Fitness magazine published a story about the gains Efron made to get ready for his Summer comedy role. In this video, I am breaking down the most important aspects of his diet plan and workouts to help you to start getting better results from your workouts heading into Summer. I’m also going to take a look back at a former “most wanted” celebrity body (that of Taylor Lautner) to see what went wrong and prevented his gains from being permanent. First and foremost, it is worth noting that neither Zac Efron or Taylor Lautner are heavyweights. Both stand just 5 foot 10 inches tall and weigh no more than 170 pounds. To make his transformation for Baywatch, Efron gained just 10 pounds of lean muscle. The thing is, when you minimize your body fat levels and maximize the amount of muscle your body can naturally carry…you look much bigger than you are. Both of these guys have mastered this. That said, you can’t expect to look better without being willing to put in some hard work along the way. Zac did just this by adopting more heavy weight training into his workout routine. He stated that he never had lifted very heavy or explosive before preparing for this film. A lot of people actually find themselves in a similar position either because they are uncomfortable lifting heavy or just don’t have the base strength to be able to do it. By increasing his strength levels on the basic compound lifts, Efron was able to start adding to his muscle density and thickness. Next he followed a push, pull, legs training split. This workout sequence allows you to train your body more athletically since you train movements instead of individual muscles. The back and biceps are responsible for all lifts in which you pull the weight towards you or pull your body towards a fixed bar. It’s impossible to isolate their actions so it helps to train them together to allow you more recovery time between workouts. Daily ab training is another key to having the resting tone needed to showcase your abs year round (provided your diet and nutrition are in check). Zac Efron’s diet is also covered here as well. By including a cheat day once in awhile, he is able to maintain his sanity and consistency required to keep his body fat levels low year round. This underlines the fact that, when managing are circadian rhythm, both elements are important! • One of the more surprising effects of time-restricted feeding in mice eating a so-called healthy diet: increases in muscle mass and even endurance in some cases. The driver who hit and killed a Chattanooga runner is being charged with vehicular homicide and leaving the scene of an accident. 59-year-old Valerie Bray was driving on Moccasin Bend in September when she drove into 28-year-old Cameron Bean. Bean was one of the top runners in the country. ____ The Hamilton County school board held a meeting about the Ooltewah rape case that violated Tennessee’s Sunshine Law. The board held an unannounced meeting Wednesday about 30 minutes before the scheduled public meeting. The law requires the board to give public notice beforehand. In the unannounced meeting, Superintendent Rick Smith made his first extensive public remarks about the assault. He said action is being taken to make sure a similar incident does not happen again. School board chairman Jonathan Welch says he did not mean to violate the law. _____ The Tennessee Board of Regents chancellor plans to resign in protest of the governor’s plan to break up the system. Governor Haslam’s proposal would create independent boards for six state universities currently governed by the Board of Regents. But John Morgan says the plan is wrong and unworkable. The governor’s proposal is set to go before lawmakers next week. _____ It turns out the Hamilton County School Board actually is allowed to discuss the Ooltewah rape case. The board’s chairman told those gathered at a public meeting Wednesday that the board could not talk about the case at the request of law enforcement. But a Sevier County official says a gag order has not been filed that would prevent the school board from saying anything. The school board’s attorney says he still advises the board members not to discuss the case. ____ Elton John is returning to Chattanooga. The pop icon is coming to McKenzie Arena March 12th. Tickets start at $27 dollars. They go on sale next Friday at 10 a.m. at tickettracks.com. ____ A million-dollar Powerball ticket has been sold in Chattanooga. Samantha Teter has been named interim director. ____ A 27-year-old Calhoun, Georgia, man is missing. Sean Patrick McDuffy was last seen December 20th. He planned a trip to western North Carolina. Now detectives believe foul play may be involved. ____ Nearly a thousand acres will be preserved in Jackson County, Alabama. The Nature Conservancy struck a deal with John Gully to save the land. The Paint Rock River watershed reportedly has several species found nowhere else. The land will not be open to the public. ____ A convicted murderer in Grundy County will get a new trial. Adam Clyde Braseel was 21 when he was accused of killing Malcolm Burrows in 2007. Now a judge says Braseel did not have an effective attorney. He could get a bond set today. _____ Rico Monuments has moved. The company has operated in the center of St. Elmo for 30 years but its lease has not been renewed. Owner Manny Rico says the business is now on Rossville Boulevard. _______ Roberts Mill Road on Signal Mountain has reopened. The MyVA Advisory Committee (MVAC) took a tour of the prosthetics lab at the Jamaica Plain VA facility in Boston. Certified prosthetist Eric Lydon showed the group different types of prosthetics he has been working on, and the process they go through when using enhanced technology to meet a Veteran's needs. Title: Care Transitions to and from the Hospital for Individuals with Alzheimer’s Disease and Related Dementias Description: This webinar presents some of the core competencies on how to best prepare and ease the difficulties surrounding care transitions, particularly to and from a hospital environment for adults with dementia. A transition of care is defined as moving from one practitioner or setting to another as condition and care needs change (Coleman and Boult, 2003). It is usually accompanied by a change in care plan. This transition can take place within settings (e.g. within the home care team), between settings (e.g. between a hospital and home) and across health states (e.g. curative and palliative care). During transitions of care, communication — between the individual with dementia and his or her family, within the home care team, and among all providers involved in caring for the person — is especially important to ensure medication safety, understanding of the care plan, clarity of roles and responsibilities, and care coordination.
