Early Warning Signs of Pancreatic Cancer Diabetes, especially if it comes on suddenly. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. The good news is that an enlarged gallbladder can be seen on imaging tests, and it may even be possible for a doctor to feel it during a physical exam. Pale, floating, smelly stools. If a pancreatic tumor prevents digestive enzymes from reaching the intestine, the result is an inability to digest fatty foods. So you end up with loose, smelly "floaters" as a result of the excess fat. Doctors say this symptom, in particular, can be an early clue and is too often overlooked. Dark, tarry stools. Bleeding in the upper intestines causes this symptom. Sudden, unexplained weight loss. Weight loss is not always, as many people mistakenly believe, a sign of advanced cancer that's spread to the liver. It can also happen because a lack of pancreatic enzymes is causing fat to pass through the body undigested. New videos everyday! Doctor with Survivor Discusses Her Battle Against Advanced Pancreatic Cancer and Amazing Long-Term Remission It’s well known that pancreatic cancer has the highest mortality rate of all cancers, making a diagnosis a virtual death sentence. In fact, it’s estimated that 94% of pancreatic cancer patients die within five years of diagnosis, and 74% of patients die within the first year. Given its deadly nature and the fact that it’s anticipated to move from the #4 leading cause of cancer-related deaths in the US to #2 by 2020, finding additional treatment options for advanced pancreatic cancer is critical. Now a study led by the University of Louisville’s James Graham Brown Cancer Center in collaboration with other leading US centers, called Soft Tissue Ablation Registry or STAR, is showing great promise for people with locally advanced pancreatic cancer. The data provides additional evidence that irreversible electroporation (IRE) ablation has the potential to augment the current treatment model and change the standard of care for patients with locally advanced pancreatic cancer. The NanoKnife® IRE system is a tool that destroys cancerous cells by subjecting them to a series of short electrical pulses using high-voltage direct current that does not injure surrounding cells, blood vessels or other vital structures. Diagnosed at forty-three years old, with two young boys in kindergarten and second grade, Elise Tedeschi, is available to discuss her personal battle with pancreatic cancer. Given just 9 months to live, she is now more than two and a half years in remission and thriving. Joining her is leading oncology surgeon, Dr. Robert Martin. Together they bring both a professional and personal, real-world view to what this new surgical technology could potentially do for those faced with a diagnosis of locally advanced pancreatic cancer. They also want to encourage patients be advocates about their healthcare and explore all options available to them. ABOUT: Robert Martin, M.D., Ph.D.: Dr. Martin has devoted most of his career to cancer treatment and care. He is the Director of the Division of Surgical Oncology, faculty member, and an Associate Professor in the Department of Surgery at the University of Louisville, James Graham Brown Cancer Center in Kentucky. He received his M.D. from the University of Louisville School of Medicine, his surgical oncology training and hepato-pancreatico-biliary training from Memorial Sloan-Kettering Cancer Center, and his Ph.D. from the Department of Pharmacology and Toxiciology at the University of Louisville School of Medicine. About STAR: The Soft Tissue Ablation Registry (STAR) is a retrospective analysis of Irreversible Electroporation (IRE) performed on 200 locally advanced pancreatic cancer patients at six medical centers in the US. This interview is provided by the University of Louisville The study was conducted at the University of Louisville, Cleveland Clinic, Cancer Treatment Centers of America in Atlanta, the Henry Ford Health Systems in Detroit, Piedmont Hospital in Atlanta and Swedish Medical Center in Denver. Pancreatic Cancer Whipple Surgery Pancreaticoduodenectomy Heidelberg Hospital Recovery From Pancreas Removal Pancreatitis Cancer Survival Liz, 48 was diagnosed in the UK with unresectable locally advanced pancreatic cancer on the 30/03/12. The UK surgeons considered it too advanced for surgery so we turned to Germany's leading cancer centre in Heidelberg. Liz underwent a pancreaticoduodenectomy in Heidelberg carried out by Professor Buchler on the 30/04/12. We have nothing but praise for the surgical and medical care she received, meaning we left Heidelberg free of all viewable cancer to convalesce in Liemen and then to fly back to the UK for Chemotherapy. We remain deeply thankful for the emotional, financial and practical help we received from family and friends. This is her story so far in pictures.

Patients are assiged a stage by their doctor that describes how much the cancer has spread beyond the liver. The more the the cancer has spread, the higher the stage and the lower the survival rate. Survival rates for these stages are traditionally given as the percentage of patients who live at least 5 years after their diagnosis. Here are the 5-year survival rates for liver cancer, according to the National Cancer Institute, based on studies done between 2005-2011. 31 Percent For people whose cancer is "localized," that is, has not spread outside the liver, the 5-year survival rate is about one in three. Localized means the cancer is still confined to the liver, and includes stages I, II, and some stage III cancers. This encompasses a wide range of cancers, some of which are easier to treat than others. 11 Percent The 5-year survival rate for "regional" liver cancer is about one in ten. Regional means the cancer has grown into nearby organs or has spread to nearby lymph nodes. This typically happens in stages IIIC and IVA. 3 Percent If liver cancer has spread to other parts of the body, the 5-year survival rate is about 1 in 33. Distant means that the cancer has spread to distant organs or tissues and is the same as stage IVB. The overall 5-year survival is better than 50 percent in patients with "resectable" tumors -- that is, tumors that can be completely removed. These patients are likely to do well if their tumors are removed when they're not suffering from cirrhosis or other serious health problems. At the present time, those who have early-stage liver cancers and get a liver transplant have a 5-year survival rate is in the range of 60% to 70%. Of course, the 5-year survival rate for liver cancer tends to be better if the cancer is found and treated at an earlier stage. The purpose of this video has been to provide quick, basic information on liver cancer survival rates. Dr. Truty emphasizes that in 2013; approximately 45,000 patients were diagnosed with pancreatic cancer. Today, with Mayo Clinic's research and modern approach to multimodal surgical management of pancreatic cancer, patients who previously may have had very little hope, now have a greater chance of long-term survival. One week after operation to remove tumour.. Among common cancers, pancreatic cancer has one of the poorest prognoses. Because pancreatic cancer often grows and spreads long before it causes any symptoms, only about 6% of patients are still alive five years after diagnosis. For some pancreatic patients, however, a complex surgery known as the Whipple procedure may extend life and could be a potential cure. Those who undergo a successful Whipple procedure may have a five-year survival rate of up to 25%. The classic Whipple procedure is named after Allen Whipple, MD, a Columbia University surgeon who was the first American to perform the operation in 1935. Also known as pancreaticoduodenectomy, the Whipple procedure involves removal of the "head" (wide part) of the pancreas next to the first part of the small intestine (duodenum). It also involves removal of the duodenum, a portion of the common bile duct, gallbladder, and sometimes part of the stomach. Afterward, surgeons reconnect the remaining intestine, bile duct, and pancreas Who Is a Candidate for the Whipple Procedure? Only about 20% of pancreatic cancer patients are eligible for the Whipple procedure and other surgeries. These are usually patients whose tumors are confined to the head of the pancreas and haven't spread into any nearby major blood vessels, the liver, lungs, or abdominal cavity. Intensive testing is usually necessary to identify possible candidates for the Whipple procedure. Some patients may be eligible for a minimally invasive (laparoscopic) Whipple procedure, which is performed through several small incisions instead of a single large incision. Compared to the classic procedure, the laparoscopic procedure may result in less blood loss, a shorter hospital stay, a quicker recovery, and fewer complications. The Whipple procedure isn't an option for the 40% of newly diagnosed patients whose tumors have spread (metastasized) beyond the pancreas. Only rarely is it an option for the 40% of patients with locally advanced disease that has spread to adjacent areas such as the superior mesenteric vein and artery, or for those whose tumors have spread to the body or tail of the pancreas. Who Should Perform the Whipple Procedure? The Whipple procedure can take several hours to perform and requires great surgical skill and experience. The area around the pancreas is complex and surgeons often encounter patients who have a variation in the arrangement of blood vessels and ducts. CONTINUED After the Whipple procedure was introduced, many surgeons were reluctant to perform it because it had a high death rate. As recently as the 1970s, up to 25% of patients either died during the surgery or shortly thereafter. Since then, improvements in diagnosis, staging, surgical techniques, anesthesia, and postoperative care have reduced the short-term death rate to less than 4% in patients whose operation is performed at cancer centers by experienced surgeons. At some major centers, the reported death rate is less than 1%. But the rate may still be above 15% in patients who are treated at small hospitals or by less experienced surgeons. Because the Whipple procedure continues to be one of the most demanding and risky operations for surgeons and patients, the American Cancer Society says it's best to have the procedure done at a hospital that performs at least 15 to 20 pancreas surgeries per year. The organization also recommends choosing a surgeon who does many such operations. What Are Complications of the Whipple Procedure? Immediately after the Whipple procedure, serious complications can affect many patients. One of the most common of these include the development of false channels (fistulas) and leakage from the site of the bowel reconnection. Other possible surgical complications include: Infections Bleeding Trouble with the stomach emptying itself after meals After surgery, patients are usually hospitalized for a week before returning home. Because recovery can be slow and painful, they usually need to take prescription or over-the-counter pain medications. At first, patients can eat only small amounts of easily digestible food. They may need to take pancreatic enzymes -- either short-term or long-term -- to assist with digestion. Diarrhea is a common problem during the two or three months it usually takes for the rearranged digestive tract to fully recover. Prognosis After the Whipple Procedure Overall, the five-year survival rate after a Whipple procedure is about 20 to 25%. Even if the procedure successfully removes the visible tumor, it's possible that some cancer cells have already spread elsewhere in the body, where they can form new tumors and eventually cause death.

Most liver cancer is secondary or metastatic, meaning it started somewhere else in the body. Primary liver cancer, which starts in the liver, accounts for about 2% of cancers in the U.S. Most people don't experience signs or symptoms in the early stages of primary liver cancer. “Although it’s vitally important and you can’t live without your liver, you can remove up to 80% of a normal liver safely with low mortality rates and the liver will actually grow back to normal size,” says Dr. Mark Bloomston, a surgical gastrointestinal oncologist on Lee Memorial Health System’s medical staff. Dr. Bloomston says this regenerative ability opens the opportunity for advanced treatment techniques: like newly developed staged procedures involving chemotherapy. “We can put radioactive beads into the liver, we can do procedures to trick the liver into thinking it already had surgery. So the good side will start to grow and advance,” says Dr. Bloomston. In the coming decades doctors hope the number of liver cancer cases in this country will drop. Hepatitis C is a driving force behind this disease which is also impacted by alcoholic cirrhosis and morbid obesity. These are lifestyle issues which can be addressed through awareness. Until then, advanced techniques offer the best chance for a cure. “And so patients commonly live for many years after liver surgery,” says Dr. Bloomston. In general, survival rates are higher for people who can have surgery to remove their cancer, regardless of the stage. Pancreatic cancer has long proved the least survivable of the most common forms of cancer, in part because it tends to spread before symptoms appear. Surgery has offered the longest remissions, but for many people with advanced cancer, an operation wasn’t an option. Now, thanks to improvements in chemotherapy, radiation and surgical techniques, even this most recalcitrant of cancers is starting to budge, says Mark Truty, M.D., a Mayo Clinic gastrointestinal surgeon. Dr. Truty and his colleagues are teaming with Mayo vascular surgeons to perform complex tumor removal operations on pancreatic cancer patients who in the past would have been considered inoperable — and they are seeing survival times rise. A select number of high-risk pancreatic cancer patients initially deemed inoperable are, in fact, eligible for surgery and have a chance for a cure, thanks to a treatment protocol pioneered at MD Anderson. The protocol combines a more accurate reading of CT scans, the use of chemo-radiation upfront and an advanced surgical resection of the pancreas and removal and reconstruction of appropriate blood vessels. Jason Fleming, M.D., associate professor in the Department of Surgical Oncology and the study's lead author, says that collaboration between surgeons, radiologists, medical oncologists is also paramount. Adenocarcinoma of the pancreas is the most common and lethal type of the disease, with a five-year survival rate of just 5%. According to the American Cancer Society, an estimated 43,920 new cases of pancreatic cancer will be diagnosed in the United States in 2012, with approximately 37,390 deaths expected. Surgical removal of the pancreas, known as the Whipple procedure, is a patient's best chance for survival, yet, currently, just 20% of patients are eligible for the high risk procedure. Published in the Journal of American College of Surgeons, the MD Anderson study enrolled 88 high-risk pancreatic cancer patients from 1990-2010. All were initially told at outside institutions that they were surgical candidates; however, upon opening, their tumors were deemed more extensive and, thus, inoperable. They then were referred to MD Anderson for care. Of these 88 patients, 66 were able to complete the MD Anderson protocol regimen, culminating with the removal of their tumor. In surgical patients, the median survival was 29.6 months, compared to 10.6 months and 5.1 months in those with locally advanced disease at their time of referral or those who developed metastatic disease before resection, respectively. "This exciting news was that of the 88 patients, 60 percent could have their tumor successfully removed, even though they had had surgery before and it was deemed unresectable, or not removable," said Fleming. "Even more exciting to us is that the survival of those patients who can have surgery here is the same as our group of patients that we do surgery first." Dr. Reber reviewed the current principles underlying the management of pancreatic cancer. In 2014 over 46,000 new cases are expected to occur in the USA, and almost 40,000 people will die. The incidence of the disease continues to increase by about 1% a year, and by the year 2020, deaths from pancreatic cancer will be second only to those from lung cancer in this country. 85% of new cases are advanced at the time of diagnosis (designated Stage III or IV disease) and these patients are rarely operated upon. Even those with Stage I or II disease (termed "early") who are candidates for resection, have more advanced disease than is evident. Thus, although some of these patients are cured by surgical resection and chemotherapy, the median survival of most of them is less than 2 years because the cancer recurs. The specific features of each of the common operations for pancreatic cancer were explained. The standard Whipple operation or the Pylorus preserving Whipple are done for tumors in the head of the pancreas; a distal pancreatectomy that includes the spleen is done for body and tail of pancreas cancers. The operations today are done quite safely with so-called operative mortality rates at UCLA less than 1%; the 5-year survival rates are over 30%. Some time was spent describing the current management of patients with Stage III disease where the tumor is locally advanced and involves some of the major blood vessels. These patients are treated with chemotherapy and some of them respond so well to that treatment that the tumor regresses to the point where surgery and even cure becomes possible. Dr. Reber explained that further major advances in how the surgery is done are unlikely, but that improvements in survival are likely as surgery is combined with newer drugs or advances that permit earlier diagnosis.

Eight months after undergoing a successful surgery for colon cancer, Delaware resident Thelma Klase, was diagnosed with a large cancerous tumor in her liver, thought to be metastatic colon cancer. After an initial attempt at surgery failed and months of chemotherapy targeting her presumed colon cancer, Thelma and her family sought a second opinion from Matthew Weiss, a liver surgeon and surgical director of the Johns Hopkins Liver and Pancreatic Cancer Multidisciplinary Clinics. Dr. Weiss reassured his new patient that another approach offered good odds for success. He used a portal vein embolization procedure to grow the portion of her liver that was to remain. Then he removed the tumor, which turned out not to be metastatic colon cancer, but a completely different liver cancer. The surgery was successful, allowing Thelma a new lease on life. In roughly one-third of pancreatic cancer patients, tumors have grown around the pancreas to encompass critical blood vessels. Conventional wisdom has long held that surgery to remove the tumors is rarely an option, and life expectancies are usually measured in months. Mayo Clinic, teaming oncologists, gastrointestinal and vascular surgeons and others, is finding that many of these patients actually are candidates for surgery. Mayo has been fine-tuning a protocol to treat them, and in two studies, found survival now stretching into years. The findings were presented at the Pancreas Club and Society for Surgery of the Alimentary Tract annual meetings in San Diego. For more information, see the Mayo Clinic News Network.
