Sulfonamides are used to treat allergies and cough, as well as antifungal and antimalarial functions. The moiety is also present in other medications that are not antimicrobials, including thiazide diuretics (including hydrochlorothiazide, metolazone, and indapamide, among others), loop diuretics (including furosemide, bumetanide, and torsemide), acetazolamide, sulfonylureas (including glipizide, glyburide, among others), and some COX-2 inhibitors (e.g., celecoxib). Sulfasalazine, in addition to its use as an antibiotic, is also used in the treatment of inflammatory bowel disease. Sulfonamide drugs were the first antibiotics to be used systemically, and paved the way for the antibiotic revolution in medicine. The first sulfonamide, trade-named Prontosil, was a prodrug. Experiments with Prontosil began in 1932 in the laboratories of Bayer AG, at that time a component of the huge German chemical trust IG Farben. The Bayer team believed that coal-tar dyes which are able to bind preferentially to bacteria and parasites might be used to attack harmful organisms in the body. After years of fruitless trial-and-error work on hundreds of dyes, a team led by physician/researcher Gerhard Domagk (working under the general direction of Farben executive Heinrich Hörlein) finally found one that worked: a red dye synthesized by Bayer chemist Josef Klarer that had remarkable effects on stopping some bacterial infections in mice. The first official communication about the breakthrough discovery was not published until 1935, more than two years after the drug was patented by Klarer and his research partner Fritz Mietzsch. Prontosil, as Bayer named the new drug, was the first medicine ever discovered that could effectively treat a range of bacterial infections inside the body. It had a strong protective action against infections caused by streptococci, including blood infections, childbed fever, and erysipelas, and a lesser effect on infections caused by other cocci. However, it had no effect at all in the test tube, exerting its antibacterial action only in live animals. Later, it was discovered by Bovet,[7] Federico Nitti and J. and Th. Jacques Tréfouël, a French research team led by Ernest Fourneau at the Pasteur Institute, that the drug was metabolized into two pieces inside the body, releasing from the inactive dye portion a smaller, colorless, active compound called sulfanilamide.[8] The discovery helped establish the concept of "bioactivation" and dashed the German corporation's dreams of enormous profit; the active molecule sulfanilamide (or sulfa) had first been synthesized in 1906 and was widely used in the dye-making industry; its patent had since expired and the drug was available to anyone.[9] The result was a sulfa craze.[10] For several years in the late 1930s, hundreds of manufacturers produced tens of thousands of tons of myriad forms of sulfa. This and nonexistent testing requirements led to the elixir sulfanilamide disaster in the fall of 1937, during which at least 100 people were poisoned with diethylene glycol. This led to the passage of the Federal Food, Drug, and Cosmetic Act in 1938 in the United States. As the first and only effective antibiotic available in the years before penicillin, sulfa drugs continued to thrive through the early years of World War II.[11] They are credited with saving the lives of tens of thousands of patients, including Franklin Delano Roosevelt, Jr. (son of US President Franklin Delano Roosevelt) and Winston Churchill. Sulfa had a central role in preventing wound infections during the war. American soldiers were issued a first-aid kit containing sulfa pills and powder, and were told to sprinkle it on any open wound. The sulfanilamide compound is more active in the protonated form. The drug has very low solubility and sometimes can crystallize in the kidneys, due to its first pKa of around 10. This is a very painful experience, so patients are told to take the medication with copious amounts of water. Newer analogous compounds prevent this complication because they have a lower pKa, around 5–6, making them more likely to remain in a soluble form. Many thousands of molecules containing the sulfanilamide structure have been created since its discovery (by one account, over 5,400 permutations by 1945), yielding improved formulations with greater effectiveness and less toxicity. Sulfa drugs are still widely used for conditions such as acne and urinary tract infections, and are receiving renewed interest for the treatment of infections caused by bacteria resistant to other antibiotics. https://en.wikipedia.org/wiki/Sulfonamide_(medicine)

SEVERE side effects my friend began having after taking a second dose sample medicine of 60 mg Dexilant Delayed-Release Capsules(a new medicine used to treat heartburn & made in China): Severe rash, hives & itching; difficulty breathing; tightness in the chest; swelling of the mouth, throat, neck, face, lips, & tongue; wheezing, stridor & shortness of breath; body pain & body tenderness; chest pain, numbness; vomiting & dizziness; fast heartbeats; pressure in throat & sternum; warm, red, & swollen all over body , very dry skin; & unusual tiredness & weakness. This is the first time my friend has ever had an allergic reaction to medicine. She was in excellent health prior to taking this medicine and had no chronic illnesses. The medicine sample was given to treat mild symptoms ie, cough & sore throat thought to be from a potential bout of seasonal allergy. Although this medicine is for heartburn it can be used to treat seasonal allergies. As of 11/05/10 my friend is still in the hospital Update: My friend has to keep a Rescue Inhaler with her at all times and she is back to work living a normal life again. :)

Bushbuckridge/ Mpumalanga/ South Africa 02/ 10/ 2017 By Yvonne Silaule Bushbuckridge- Ngobeni is angered that it is a month after he buried his son and Mapulaneng Hospital has not called to discuss a way forward with regards to the death they caused his son. Patrick Ngobeni (73), father of the disclosed on Monday that his son Hebert Ngobeni (54) was involved in a car accident 10 years ago where his legs were trapped; he recovered but retained a knee pain that kept him going in and out of hospital. He said that his son was hospitalised in September where he met his untimely death “One of his legs was draining its pulse for a while since the accident and that caused him pain so we took him to hospital last month where he was admitted for treatment,” said Ngobeni. “On admission day in hospital my son told the nurses that he was allergic to sulpha and they respected his wishes and gave him medication that he responds well to.” He said. Ngobeni said that it was discovered at home, a minute too late that the doctor who discharged him wrote the pills that contain the sulphur and that he had already taken some of them after his meal. “He was sent home with a stash of pills that brought his life to quick death and we wouldn’t have known which pills are when we gave him medication dose at home.” He said. Ngobeni said that his son started scratching in response to the false medication he tool before he was rushed back to the hospital where he was given allergex to calm the itch and sent back home. “Even though he received medication for the itch, two days later his skin had water like he sustained great burns. He was taken back to hospitals where his entire body was bandaged but only to slide off the skin that was water swollen.” said the father. Ngobeni said that despite his son’s critical state, the hospital did not place him in intensive unit or high care but just a room with only a drip in it. “What hurts the most is that he stayed the entire week without proper care even though he was in great pain consisting of those first degree burns. It was like he was not being nursed in a medical ward” “He was poorly managed inside an ordinary ward with only a drip and no machines to monitor him. They should have transferred him to another hospital with an intensive care unit because they told us that their intensive care only had two beds,” said Ngobeni . He disclosed that the hospital refused him the right to conduct a post-mortem as he wanted to get closure. “They told me that post-mortem is only conducted over deaths caused by poison or unclear deaths.” He said. Ngobeni said that he is giving the department of Health Mpumalanga along with the hospital a minimum of 14 days to act towards the mess that happened at their account or he will sue. “I am surprised that they have not called for any discussion as it was their promise. I need them rectifying what they have done coming within14 days or I sue. I have already started seeking legal advice but it will all depend on the attitude in the department’s action,” said Ngobeni “ It sickens me because they have murdered my son and now the y are acting as if nothing has happened; they are murderers, they are criminals and that is how I can describe the”. He said. According to Chriss, Acting Director of Mpumalanga Department of Health, they are aware of the matter and that they are still busy investigating.

Escherichia coli, a member of the Enterobacteriaceae family of bacteria, is the leading cause of community- and hospital-acquired urinary tract infections (UTIs). Although E. coli commonly colonizes the intestines, certain food-borne strains can cause serious gastrointestinal infections. These maps show inpatient E. coli resistance to trimethoprim-sulfa (also known as TMP/SMZ or co-trimoxazole) , a combination antibiotic that is frequently administered as a first-line therapy for uncomplicated UTIs when resistance rates are below 20%. Inpatient TMS/SMZ resistance is similar in magnitude and geographic variation but increases slightly faster than outpatient rates. In 2000, varied resistance levels to the drug appear across the United States, with most of the resistance in the western half of the country. Higher resistance levels then spread eastward. In 2006, all regions have passed the 20% threshold with West South Central and Pacific regions reporting the highest levels of resistance, at just over 30%. East North Central, Mountain and the South Atlantic are nearly as high, with resistance levels between 25% and 30%. By 2009, resistance levels higher than 30% are widespread, with only New England remaining close to the 20% resistance threshold. The rise in both TMP/SMZ and ciprofloxacin resistance has motivated calls for a revision in UTI treatment and a switch to nitrofurantoin, an older antibiotic that now shows higher effectiveness. Increases in TMP/SMZ resistance signal a problem with broader implications for both nosocomial (health care--associated) and community infections, namely the rise of extended-spectrum beta-lactamase (ESBL) strains of Enterobacteriaceae. ESBLs are bacterial enzymes that confer resistance to many antibiotic classes, including sulfonamides. The epidemiology of ESBL-producing strains, particularly E. coli, has increasingly shifted from the hospital setting to the community. ESBL infections are still susceptible to the newer and more expensive antibiotics of the carbapenem class. However, newer types of beta-lactamase enzymes are diminishing the effectiveness of these last-resort treatments and posing an emerging threat to public health. The national average resistance level for the sample was 24.1%, rising from 17.7% in 2000 to 29% in 2009. The sample consists of 872,563 inpatient isolates tested for trimethoprim-sufamethoxazole resistance. Data are not available from the following states: AR (2008--2009), CO (2006--2009), GA (2007--2009), IA (2008--2009), IN (2007--2009), KY (2007--2009), MS (2006--2009), NV (2009), RI (2000--2004), SD (2005--2009), UT (2007--2009), CT, NH, MT, and WY.

Upon examination of the throat, the uvula is swollen, pale, and somewhat translucent (uvular hydrops). If greatly enlarged, the uvula might rest on the tongue and move in and out with respiration. There might be an associated rash or a history of exposure to phsical stimuli, allergens, or a recurrent seasonal indicence. What to do: Because of the known association of uvular with hypopharyngeal edema, watch for signs of airway compromise. If a patient complains of resiratory difficulty or breathes with stridor, commence treatment with intravenous lines and intubation and cricothyrotomy equipment at the bedside, and a crosstable lateral soft tissue neck x ray to rule out epiglottic swelling. If there is no acute respiraory difficulty, ask about precipitating events. Consider foods, drugs, physical agents, inhalants, insect bites and hereditary angioedema. When fever, sore throat and pharyngeal injection are present, culture the throat with a rapid strep screen and give an antibiotic that covers Haemophilus influenzae (e.g., Biaxin, Augmentin, Bactrim). It is reasonable to obtain a complete blood count with a manual differential to demonstrate eosinophilia to support the possibility of an allergic reaction or a high leukocyte count with increased granulocytes and bands to support a bacterial infection. Initially the patient should receive parenteral H1 and H2 antihistamines like hydroxazine 50-100mg im or diphenhydramine 25-50mg iv along with cimetidine 300mg iv or po or ranitidine 50mg iv or 150mg po. More severe cases should receive repeated doses of epinephrine 0.3ml of 1:1000 sq every 20 minutes x3. Nebulized isomeric or racemic epinephrine or albuterol are also effective. Parenteral corticosteroids like SoluMedrol 125mg iv are also typically used, although efficacy remains umproven. If there is a history of recurrent episodes of edema and there is a family history of the same, consider ordering a C4 complement level or C'1 esterase inhibitor levels as a screening test for hereditary angioedema. In this condition, the edema often involves the uvula and soft palate together. Uvular decompression may be useful in patients that are resistant to medical therapy or whose symptoms progress rapidly. This procedure consists of grasping the uvula with forceps and either making several lacerations with a sterile needle or snippint the distal centimeter as a patial uvulectomy. All patient s should be observed for an adequate period of time to insure that there is either improvement or no further worsening of the swelling before being discharged home. Upon discharge, they should receive 4-5 days of H1 and H2 blockers and steroids if required.

I myself am a hay fever allergy kind of guy I’ve struggled with allergies for lots of years and unfortunately I passed that to a trait on my son. He has even worse allergies than I do and we have battled hay fever and allergies with antihistamines and unfortunately he has had multiple sinus infections so he's been on rounds of antibiotics which you know as a chiropractor I hate to have to roll down that antibiotic road. He's tried nasal steroids, he has a little of, I'm excited to say that after discovering a company that’s based out of Colorado Springs here named Allergy 123, we formed an alliance and Allergy 123 now comes to this office on Tuesday mornings. We have our allergy clinic to illuminate a little bit about what happens with our allergy clinic basically Dr. Ogrodnick is our medical doc that is involved and what we basically do with our allergy clinic is we do traditional allergy testing skin prick testing. I've been through it, my son has been through the idea, of course, is to see if you're a good candidate for immunotherapy or allergy shots. Allergy shots work, basically, they're all natural… there's a serum that's produced based on your individual allergens and that serum is diluted and then gradually escalate as your allergy shots progress. As you inject this allergen, your immune system adapts to that allergen. So if you're allergic to ragweed for example and that’s what you react to in the skin grid test then there will be ragweed in the serum and initially a very low dose and your body will hardly notice it and then little by little your body adapts. So that’s a tried and true theory; it’s been around forever. The problem has been who has time to go to an allergy clinic once a week for multiple years that has been a standard treatment protocol. At our office, with Allergy 123 here at Springs Chiropractic, a patient can come in have their test. If they’re a good candidate the serum is provided for them and then you can do your own allergy shots at home. My son is doing his own allergy shots, I’m doing my own allergy shots and so far so good were noticing improvements in our symptoms after two months, so I know the system works but the break though basically is the ability to in the program. There are 10 year old, eight year old children giving their own shots so it is a very tiny tiny little needle. It’s an easy process you basically use a small fold of skin you inject the serum and you’re good to go you do that self-administered shot twice a week. In about 18 months your body is fully comfortable with those allergens and your symptoms are dramatically reduced. So compliance is the key Allergy 123 boasts a 90% success rate and as I said, all natural. If you can get away from the daily Zyrtec or Allegra or other antihistamines that you might be on Claritin. Basically it over taxes your immune system and then you have trouble fighting off other viruses and things you want to have that immune system for.
