In a previous video, we covered the importance of clot prevention in heart attack and stroke prevention. Plavix was the first option to coumadin. It is on the WHO list of Essential Medications. This means it's been found safe, effective and necessary. It costs about $25/month is the US. The major side effects are the same as all the other anti-clotting or anti-platelet drugs: bleeding or hemorrhage. There were other issues mentioned but not covered in depth: relatively higher benefit for smokers; dipyridamole; CHAD2VAS2. Here's the thing, the big issue - less than 1/3 of people needing this medication are actually taking it. Why? Patients blame docs, docs blame Pharma, ... On Monday, the US Supreme Court tightened limitations on where injury lawsuits may be filed, handing a victory to corporations by undercutting the ability of plaintiffs to bring up claims in friendly courts over a case involving litigation against the Bristol-Myers Squibb Co for its blood-thinning medication Plavix. In an 8 to 1 ruling, the justices threw out a lower court decision to allow hundreds of out-of-state-patients who took Plavix to sue the California-based company. However, the justices ruled that state courts cannot hear claims against companies that are not based in the state when the alleged injuries did not occur there. AstraZeneca received a much-needed boost when its blood-thinning drug, Brilinta, a potential blockbuster, was approved by the US health regulator for wider use. The Food and Drug Administration approved Brilinta tablets for longer-term use in patients with a history of heart attack. The new 60 mg dose can be taken twice a day along with aspirin beyond a year after a heart attack. Until now, the blood thinner could only be prescribed for one year after a heart attack, as a 90 mg dose. The debate on Cinnamon continues. There is little evidence that it impacts cholesterol or HgA1c. But there are several studies indicating that it may improve fasting glucose levels. For prediabetics, it's the post-meal glucose values that are the problem, not fasting glucose values. In addition, the most recent Cochrane meta-analysis did not support any impact at all from cinnamon on any cholesterol or glucose values. So, why take cinnamon? It's cheap ($3/month). You might enjoy those occasional cinnamon burps. And maybe it will turn out to help. But if you do take it, you may want to make sure to look for ceylon, burmanii, or cassia. These are all really the same thing. Korintje cinnamon is different. Although it's not known, there has been far less research on this. Logically, that's surprising when you consider that vascular imaging can clearly show us when plaques are "hot" (they have a liquid necrotic core), and the plaque cap is getting thin. So if you have one of those issues this is a video you need to listen to because what you have and when you had it --originally-- makes a dramatic difference on what you can qualify for because many people cannot qualify for life insurance unless they work with someone --like myself-- for a broker who looks at all different companies who are out there and can give you the best value quoted, the best value price. Now, let's get to the specifics. When we look at issues like heart attacks, seizures –I should say let’s just start with each one and go through it. Heart attacks: Typically if you've been heart attacked...if you've had a heart attack within the last year you're going to go qualify for a guaranteed-issue product that’s going to make you wait to two full years. If you had a heart attack between one year and two years, you’re gonna probably most likely qualify for a great and partial program. And in some cases you’ll qualify for a first day 100% coverage. Again, not a lot of people offer this plan but I do have access to that kind of company. So if that fits you, I may be able to help you in ways that other agents can’t. That if you've been at least two years out with most companies they will give you 100% first full day coverage natural and unnatural and accidental death. For strokes, it's a little different situation. You have full-blown Strokes and then you have mini strokes and what’s called transient ischemic attacks. For full-blown strokes it’s the same guidelines as a heart attack. When you're out less than one year out --two year wait on a gratted-or a guaranteed issue plan, one or two years partial coverage or full day coverage, and then two years and beyond-- most companies full day 100% coverage . If you have some kind of mini stroke or transcametic attack those considered less of an issue --even though they're serious-- to a lot of companies. And there are even companies that will give you 100% first day full coverage even if you’ve just had a mini stroke or a transcametic attack yesterday. So, again, hard to find but if that fits you talk to me because I can help you out in many ways that other agents cannot. Seizures: a lot of companies make you wait 2 years. Many companies cover you 100% from the very first day from natural death even if your seizing as you're talking to them. So we have heart attacks, strokes, seizures, bypasses –bypasses follow the same guidelines as heart attacks- aneurysm, same thing. Some companies actually won't talk to you even if you have an aneurysm that hasn't been operated. That’s not the case of all companies. Even if you've had an aneurysm and it's been okay for two years there are options. Stints: same set up as heart attacks --anything within the first two years. First year is harder to get, between one to two years you get greater full coverage. And then two years and beyond you can get full coverage. And then lastly you have pacemakers. Most companies do make you wait for two years for coverage. I don't . I have a company that will cover you as long as you as long as it's been a year since your pacemaker has been installed. Along with-oh, and there's also congestive heart failure, yet another situation where most companies will only offer you a two year guarantee issue product before you're even covered, whereas I have access and limited cases of coverage where I can get you 100% approved from the first day . Very nice. There's also a couple other factors that you have to be aware of. Sometimes your circulatory medicine can make an impact on whether you can get qualified or not. For example, you may be taking Plavix or a blood thinner like Coumadin or Carvedilol or Furosemide. Lots of carriers out there will rate you up at a higher price and then won't even cover you if you take those medications. I on the other hand work with those particular medications daily. Two studies are mentioned: one saying that it's safe to stop statins, and another says it's not. Although these studies, like all studies, have problems. But I think they both reflect biologic medical realities. The study showing safety was on stable patients. The study showing danger in stopping statins was on patients post strokes. Plaque instability is systemic; there is no such thing as one vulnerable plaque. The VULNERABLE PLAQUE THEORY is rapidly dying. Therefore the patients in the post-stroke study selected for patients with an arterial tree full on unstable plaque. The study showing safety selected for patients with stable plaques. Plaque instability appears to be a cyclical phenomenon, with cycles taking many months. So I would love to see a 2 year follow-up on the study showing safety in stopping plaques. Therefore, "blood thinners", or antiplatelet medications are critical concerns if you have plaque and inflammation. Aspirin was covered in another video. Viewers have asked that we discuss the newer medications. Warfarin (or coumadin) was the next generation after aspirin. It's been around for years and helped cover situations, like stents, that required more than aspirin. Warfarin has always had a lot of problems. That's understandable. It's been the major component of rat poisons. It requires constant monitoring. There are often bleeding episodes associated with things as simple as a change in diet. Plavix is the next generation. (Clopidogrel). It's a major step forward. But it really should be accompanied by a genetic test to get the right dose. There is yet another generation of these medications. They are called NOACs (Novel Oral AntiCoagulants). Many have debated that these are as safe - or safer than- aspirin. Ellaquis and Xarelto are examples. Studies show that less than 1/3 of patients needing these meds are actually taking them. Is that the doctors' fault? or the patients? Clearly both. Most doctors prescribe statins to lower LDL (or "bad" cholesterol). However, that's probably not how statins prevent heart attacks and strokes. It's more likely due to an impact on cardiovascular inflammation. What is cardiovascular inflammation? Inflammation is similar whether it's causing arthritis, skin disease or inflammatory bowel disease: inflammation is the body's attempt to correct a problem. The immune system sends antibodies and immune cells. Immune cells release enzymes to "digest" tissue that should be there. In the artery wall, that tissue is plaque. Inflamed plaque is unstable. It can leak into the bloodstream and cause life-threatening clot formation. Ford Brewer is a physician that started as an Emergency Doctor. News Update: Eli Lilly Switching Study Shows Promising Results During Phase II Trial

Drug maker Eli Lilly (NYSE:LLY) reported that results from a Phase II trial showed that patients with ACS (acute coronary syndrome) showed a significant reduction in platelet aggregative after taking 10mg of Effient instead of taking 75mg Plavix. Platelet aggregation is a critical step in the formation of blood clots, which pose a significant risk to patients following an ACS event, including heart attack and heart-related chest pain. Dominick J. Angiolillo, M.D., an assistant professor of medicine at the University of Florida College of Medicine said, "These findings are important because they provide new insights into potential differences in the levels of platelet inhibition that can be achieved with dual oral antiplatelet therapy in patients with ACS. The data showed that Effient plus aspirin may provide additional reduction in platelet aggregation in ACS patients over those taking standard-dose clopidogrel plus aspirin.
