Definition Diabetic retinopathy is a diabetes complication that affects eyes. It's caused by damage to the blood vessels of the light-sensitive tissue at the back of the eye (retina). At first, diabetic retinopathy may cause no symptoms or only mild vision problems. Eventually, it can cause blindness. Symptoms You might not have symptoms in the early stages of diabetic retinopathy. As the condition progresses, diabetic retinopathy symptoms may include: - Spots or dark strings floating in your vision (floaters) - Blurred vision - Fluctuating vision - Impaired color vision - Dark or empty areas in your vision - Vision loss Causes Over time, too much sugar in your blood can lead to the blockage of the tiny blood vessels that nourish the retina, cutting off its blood supply. As a result, the eye attempts to grow new blood vessels. But these new blood vessels don't develop properly and can leak easily. Risk factors Anyone who has diabetes can develop diabetic retinopathy. Risk of developing the eye condition can increase as a result of: - Duration of diabetes — the longer you have diabetes, the greater your risk of developing diabetic retinopathy - Poor control of your blood sugar level - High blood pressure - High cholesterol - Pregnancy - Tobacco use - Being black, Hispanic or Native American Prevention If you have diabetes, reduce your risk of getting diabetic retinopathy by doing the following: - Manage your diabetes - Monitor your blood sugar level - Keep your blood pressure and cholesterol under control - If you smoke or use other types of tobacco, ask your doctor to help you quit - Pay attention to vision changes - Ask your doctor about a glycosylated hemoglobin test Treatments Treatment, which depends largely on the type of diabetic retinopathy you have and how severe it is, is geared to slowing or stopping progression of the condition. 1. Early diabetic retinopathy If you have mild or moderate nonproliferative diabetic retinopathy, you may not need treatment right away. However, your eye doctor will closely monitor your eyes to determine when you might need treatment. When diabetic retinopathy is mild or moderate, good blood sugar control can usually slow the progression. 2. Advanced diabetic retinopathy If you have proliferative diabetic retinopathy or macular edema, you'll need prompt surgical treatment. Focal laser treatment This laser treatment, also known as photocoagulation, can stop or slow the leakage of blood and fluid in the eye. During the procedure, leaks from abnormal blood vessels are treated with laser burns. Scatter laser treatment This laser treatment, also known as panretinal photocoagulation, can shrink the abnormal blood vessels. During the procedure, the areas of the retina away from the macula are treated with scattered laser burns. The burns cause the abnormal new blood vessels to shrink and scar. Vitrectomy This procedure uses a tiny incision in your eye to remove blood from the middle of the eye (vitreous) as well as scar tissue that's tugging on the retina. It's done in a surgery center or hospital using local or general anesthesia.

Diabetes per se will not be responsible for varicose veins. But most often those with diabetes especially type 2 diabetes are obese. Because of overweight and those who are standing for long time can cause varicose veins. Problems with varicose veins in diabetes is that the uncontrolled sugars will contribute to venous ulcers and developing of cellulitis. So the patients can have thrombophlebitis. i.e. inflammation of the veins and that can actually worsen the undergoing varicose veins. So diabetes may not directly cause but it will worsen the underlying problem and hence the severity of the varicose veins may increase in those with diabetes and they may develop recurrent cellulitis.

Literally thousands of people develop a leg ulcer each year in the United Kingdom and many of those get more leg ulcers as the years go by. Despite the misery they cause, leg ulcers remain a neglected problem. I am going to tell you the 7 essential facts should you know about leg ulcers. Number 1 -- What is an ulcer? The medical definition of an ulcer is a break in the epithelium of a body surface or lining. Many of us will be familiar with a mouth ulcer. This is a break in the cell lining the mouth. It can be caused by an injury such as biting the inside of the mouth by accident. Well if the skin breaks down anywhere on the body it is called an ulcer. If the ulcer is on the leg and has been present for more than 6 weeks it is called a chronic leg ulcer. Number 2- What causes a leg ulcer? Four out of 5 ulcers -- that's 80% - are caused by a problem with the leg vein pump and they are called venous ulcers. The leg vein pump is the mechanism by which blood flows out of the leg back to the heart. So problems with the deep veins such as obstruction from a deep vein thrombosis, superficial vein reflux or perforator vein problems can cause venous leg ulcers. The leg vein pump depends mainly on good ankle movement, good calf muscles and healthy leg veins. A minority of leg ulcers - about 15% - are caused by problems in the arteries that carry blood into the leg. About 95% of venous leg ulcers can be correctly identified by their appearance, by checking for pulses in the feet and by checking sensation. As most leg ulcers are venous -- that's what I am going to concentrate on. Number 3 -- Where on the leg do you get venous ulcers? Well venous leg ulcers appear on the lower part of the leg usually just about the ankle. Often, there are varicose veins or thread veins around the ulcer and the leg is usually swollen. The skin is often thickened and discoloured by varicose eczema which may actually develop many months or years before the ulcer as a warning sign that the vein circulation is not normal. Number 4 -- Who gets a venous ulcer? Well these ulcers affect people who have deep vein problems, usually from a past deep vein thrombosis (DVT), people who have superficial venous disease or those with ankle mobility problems. Leg ulcers are more common as we get older. Number 5 -- How do you treat venous ulcers? The aim of treatment is to get the ulcer healed as quickly as possible. This usually means graduated compression either with bandages or compression stockings. In most cases, antibiotics, creams and ointments are not necessary. Once the ulcer is healed it is very important to identify any problems with the superficial veins or perforator veins and that these are treated to reduce the risk of another ulcer. This means getting a full assessment with a duplex ultrasound scan. Number 6- Can you cure a leg ulcer? Well this is controversial. Some would say that if the problem is only in the superficial veins, the veins under the skin, or in the perforator veins that connect these superficial veins to the deep veins, then the ulcer can be cured. While there is good evidence that such treatment will greatly reduce the risks of getting another leg ulcer, if the skin has been very damaged and the ulcer has healed with a lot of scarring, this area of the leg may remain vulnerable and a trivial knock or injury might cause another skin breakdown even though the vein abnormality has been completely eradicated. Number 7 -- How can we avoid leg ulcers? Well there are 3 things I would suggest. Firstly, if you are unfortunate enough to suffer a deep vein thrombosis (DVT), it should be treated promptly to limit the damage to the deep veins. Unfortunately, damage to the deep veins cannot be reliably reversed by surgery. So, prompt treatment with anticoagulants -- blood thinning medication -- reduces the extent of the clot and the damage to the deep veins. Secondly, if you have had a DVT, you should consider wearing good quality medical grade compression socks -- some studies suggest that doing so may reduce the risk of leg ulcers after a big DVT. Thirdly, if you have superficial vein reflux have it treated early to reduce the chances of progression to varicose eczema which is itself a warning sign that the skin is damaged and liable to ulceration. How to stop leg and foot cramps at night and why do legs cramp at night? Nocturnal leg cramps are one of the most painful problem you can experience – they occur in the middle of the night and the sharp pain can ruin your sleep, leaving you without energy and with sore leg muscles in the morning. Leg cramps are involuntary contractions of the calf muscles that usually occur overnight. They can also strike at the soles or other muscles.   The cramps may last from a few seconds to a few minutes, and the intense pain is often difficult to bear. Nocturnal leg cramps affect everyone regardless of age or gender, although they’re more common in middle aged people. Although science is still unsure of the main cause, there are numerous factors that contribute towards it. Dehydration Dehydration is one of the biggest causes of nocturnal leg cramps. Proper hydration means proper body and mind function, including the contraction of muscles. If you’re not keeping properly hydrated during the day, you will experience cramps overnight. Without sufficient amounts of water, your muscles will be robbed of essential nutrients they need to work normally. Nutritional deficiencies Potassium, magnesium, calcium or sodium deficiency can definitely cause the dreaded leg cramps. All these nutrients play different vital roles in your muscles and help them contract – if the muscles are robbed of them, they will fail to work properly. Potassium works with sodium and chloride and generates electrical impulses in the nerves and muscles, while calcium generates proper muscle contractions and nerve impulses. Magnesium stabilizes the levels of adenosine triphosphate, so any of these mineral deficiencies will result in an increased risk of muscle cramps and other problems. Standing for too long Standing for prolonged periods or wearing improper footwear can overburden your muscles and increase the risk of nocturnal leg cramps. According to one study, the same factor raises the risk of varicose veins, so make sure to cut the time spent on your feet. Sitting with crossed legs or sitting for too long can also cause the condition. Pregnancy Cramps are pretty common during pregnancy, especially from the second trimester onward. They can range from mild to pretty painful and occur as a result of the increased pressure of the uterus on some nerves and reduced blood circulation in the legs. Hypothyroidism Low levels of thyroid hormones in the body are definitely a cause for concern and may lead to a number of health problems including nocturnal leg cramps. Hypothyroidism reduces the levels of calcium in the body, resulting in muscle pain, weakness and numbness as well. Low thyroid hormone levels can also cause fatigue and low metabolism, which can result in inflammation and muscle cramps. Diabetes Diabetes patients often experience nerve damage (diabetic neuropathy) in the legs which results in sharp pain and numbness. The high blood sugar levels can dehydrate your body and lead to muscle cramps – if you experience this symptom and you’re a diabetic, you should consult with your doctor immediately. A few tips that will help you prevent nocturnal leg cramps. In order to prevent the cramps from occurring, you need to stay hydrated at all times. Consuming sport drinks with electrolytes can also prevent the problem. Drinking too much coffee, or sodas can dehydrate your body and increase the risk of cramps. Leech therapy is the use of leeches in the treatment of disease conditions. Medicinal leeches are bloodsucking worms that they live in both aquatic and terrestrial environments. Primary Cause Of Heart Attacks & Strokes Poor blood flow and circulation is one of the primary causes of a heart attack or stroke. This is because it can lead to: -Hypertension and high blood pressure -Hardening and narrowing of your arteries -Higher cholesterol and plaque build up Warning Signs Some of what “warning signs” that you may have circulation problems include Frequent pain and cramping in your legs Tingling or numbness in your feet Tired, aching or weak legs and feet Slow-healing wounds Thickened toenails Varicose and “spider” veins Why Circulation Is So Important Now, improving circulation to your legs is very important because you have to pump the blood uphill to your heart… going against gravity! When you walk, your foot and leg muscles are contracting and relaxing and basically acting like a “second heart”, to continually pump blood back up through your legs and to your heart. Unfortunately, if you are sedentary for prolonged periods of time … like sitting for too long, on a long flight or bedridden… your “second heart” is inactive and blood can gather in your veins and around your ankles and this can lead to pain in the lower legs… and worst case scenario it can lead to a life-threatening blood clot. The good news is that by performing regular leg exercises, you can promote healthy blood flow from you legs to your heart and thus, help prevent blood clots and leg pain. Exercises Walking I think walking is one of the best exercises you can do and almost anyone can do it, any where, any time of the day and it doesn’t cost you anything. Ideally you would walk outside in fresh air if possible. First thing morning and/or late evening are good times but, whenever you can is a better than nothing. To start, all you need is about 15 minutes, 3x weekly. Ideally you would work up to daily walks of about 30 minutes. And if you have pain or trouble walking, even 3 minutes is a good start. Try your best to walk just a little bit every single day. Rebounder A rebounder is basically a mini trampoline. I like this because it is very low impact and super easy on your joints. It also helps clean your lymph nodes, which helps prevent many cancers and illnesses. I put mine in front of the TV while watching a favorite show and do it for about 10-20 minutes daily. You don’t need much at all. By the way, you’re not trying to jump high on it and try to touch the ceiling. You only need to go up a few inches. Just enough for your toes to be off the roubound. It’s just a light up and down motion. Again, almost anyone can do it, any time and anyplace. You can even travel with it. Body Weight Squats This last exercise is great for building and strengthening your entire lower body … From your ankles to your leg muscles and all the way to your lower back and stomach. You can simply use your own bodyweight and do sets of about 15-20 repetitions. Of course, if you’re first starting out, simply doing even 2-3 comfortable repetitions is a great start. Go down as far your comfortable and without pain. Over time, as you get stronger and more flexible, you can do more repetitions and go deeper. Ideally, you would do maybe 1 set of 15 or more repetition, 2-3x daily. Maybe in the morning, noon and night time. More often and throughout the day helps keep the blood flow regular and continuous. And if you don’t have good coordination, simply hold onto a wall or something sturdy as you squat down. Summary Poor blood flow is bad for your health, if affects your entire body. Step 1 - Iyengar yoga to reduce varicose veins Sit sideways on the chair, raise both legs onto the handle of the chair. Move your bottom into the chair and slowly start moving until your shoulders touch the blocks. You chins should lock onto your chest. Stretch your toes out and close your eyes. Step 2 - Iyengar yoga to reduce varicose veins Variation -- Get your toes together and allow your knees and thighs to roll out. Hold this position for at least 7 to 8 minutes. Bring your feet down, start sliding down from the chair till your bottom touches the blocks. Rest your calf muscles on the seat of the chair. Slowly remove the blocks, roll over to right side and come up. Step 3 - Iyengar yoga to reduce varicose veins Press the bolster to the side of the wall. Hold the rope, raise both legs up, allow your buttocks to press onto the bolster. Let go of your hands, observe your breath. Hold for at least 5 minutes. To come out of the asana, keep feet flat on the wall, press the bolster, wiggle back a bit, cross legs on the bolster, chin down towards your chest, hug your knees, roll over to right side and come up. Symptoms of Diabetes Diabetes is often called the silent killer, But if you have these symptoms, see your Doctor If you need to urinate frequently—particularly if you often have to get up at night to use the bathroom The kidneys kick into high gear to get rid of all that extra glucose in the blood, hence the urge to relieve yourself. The excessive thirst means your body is trying to replenish those lost fluids. Weight Loss Overly high blood sugar levels can also cause rapid weight loss, say 10 to 20 pounds over two or three months—but this is not a healthy weight loss Because the insulin hormone isn't getting glucose into the cells, where it can be used as energy, the body thinks it's starving and starts breaking down protein from the muscles as an alternate source of fuel The kidneys are also working overtime to eliminate the excess sugar, and this leads to a loss of calories (and can harm the kidneys). Hunger Excessive hunger, can come from sharp peaks and lows in blood sugar levels. When blood sugar levels plummet, the body thinks it hasn't been fed and craves more of the glucose that cells need to function. Skin problems Itchy skin, perhaps the result of dry skin or poor circulation, can often be a warning sign of diabetes. Darkening of the skin around the neck or armpit area Slow Healing Infections, cuts, and bruises that don't heal quickly are another classic sign of diabetes. This usually happens because the blood vessels are being damaged by the excessive amounts of glucose traveling the veins and arteries. Yeast infections Diabetes is considered an immunosuppressed state. That means heightened susceptibility to a variety of infections, although the most common are yeast (candida) and other fungal infections, she says. Fungi and bacteria both thrive in sugar-rich environments. Women, in particular, need to watch out for vaginal candida infections Fatigue and irritability When people have high blood sugar levels, depending on how long it's been, they can get used to chronically not feeling well. Sometimes that's what brings them into the office. Getting up to go to the bathroom several times during the night will make anyone tired, as will the extra effort your body is expending to compensate for its glucose deficiency. And being tired will make you irritable. "We see people whose blood sugar has been really high, and when we bring the blood sugar down, it's not uncommon that I hear, ‘I didn't realize how bad I felt,'" she says. Blurry Vision Having distorted vision and seeing floaters or occasional flashes of light are a direct result of high blood sugar levels. Blurry vision is a refraction problem The good news is that this symptom is reversible once blood sugar levels are returned to normal or near normal. But let your blood sugar go unchecked for long periods and the glucose will cause permanent damage, possibly even blindness. And that's not reversible Tingling or Numbness Tingling and numbness in the hands and feet, along with burning pain or swelling, are signs that nerves are being damaged by diabetes. If (the symptoms are) recent, it's more likely to be reversible. Still, as with vision, if blood sugar levels are allowed to run rampant for too long, neuropathy (nerve damage) will be permanent. That's why we try to control blood sugar as quickly and as well as possible. Blood Tests Several tests are used to check for diabetes, but a single test result is never enough on its own to diagnose diabetes (the test has to be repeated). One is the fasting plasma glucose test, which checks your blood sugar after a night (or eight hours) of not eating. Blood glucose above 126 milligrams per deciliter (mg/dL) on two occasions means you have diabetes. The normal cut off is 99 mg/dL while a blood sugar level of 100 to 125 mg/dL is considered prediabetes, a serious condition on its own.

The edges are little raised and a little reddened and if the usual leg will be a little different from this because it is usually quite a lot larger and it is quite a lot more swollen. Because of the swelling of it, it makes the skin tense. And that tends to draw the edges apart more and to retard healing. So, one of the things that we can do to improve the healing is to put on a type of bandage that will make the, the leg, the skin on the leg to pull together more, so that the, the tissue can grow, can spread over, the skin surfaces can spread over the wound and heal it in that way. Now, another thing that we need to do with this ulcer is to clear up the pus and the dead material down in the active part of the ulcer. And, there is a rigid bandage, which we call the Unna boot, which I would like to show to you now. The, and for that I would like to have a product, which you can buy on the market may not go by this brand name, but it will have or may have the name of Unna boot or Unna's boot as this one does. Now, the Unna boot is the original and of course we have used the Unna boot for I suppose 100 years or more and it goes on very nicely, even when you use the old fashion Unna's boot itself. And, I've here the ingredients for the old Unna boot that we used to make up 50 years ago and it is zinc oxide, which you simply measure out into a cup and you get the formula properly for it. And at which you can get online and then you use glycerin. And glycerin can also be obtained from a pharmacy as this one was. And the glycerin is mixed with it along with some gelatin, which you can get from a grocery store just the plain Knox gelatin or any kind of clear gelatin, which you can use. I suppose Agar could also be used in the same way, but the Knox gelatin is by all means the best. And so you can make, you can make this yourself, it needs to be heated because of the gelatin, so the gelatin is softened in some water and heated to dissolve it and then the zinc oxide and the glycerine put in. And then the part of foot and leg and the ulcer is wrapped with gauze such as this and then with a little paint brush the old Unna paste is painted on the first layer of the gauze and then a second layer of gauze is wrapped on that and it's allowed to harden and to gel and that makes the rigid bandage. Now, while I have put on I suppose a hundred Unna boots with the old fashion paste and the paint brush. I would like to show you how to use this, this new variety. This is more expensive, but it's also it's so much more convenient. So, I've asked an assistant and that so Arianna Hartsfield. So, Arianna if you will come here and join me, and I would like you to sit right here and Arianna is one of our students in our church school at Uchee Pines and we are pretending that she has a large diabetic ulcer on her left leg. So, if you remove your left sock, we will proceed to put on an Unna boot. Unna boots are extremely successful and they will both clear up the ulcer. It will clear that to dead an infective material up. And also will make the, the swelling go down and heal the ulcer, you can see it from one bandage change to the next. So, we will put this right there and then we will put this on top of that just like so that will help it to stay on. And, then we just wrap this on, it isn't difficult to put it on, it goes on sort of like a little cast. And because it is made of rather thin gauze, it is of course going to mold a bit to her foot and this has to go down all the way to the base of the toes as you see me doing here.

However there are many pharmacological options available at present to manage diabetes and control blood sugar levels. But it is just a treatment not a permanent cure. Though, calorie control with strict caution and prevention in lifestyle can boost the treatment. Vohra Wound Physicians: Healing Wounds, Saving Lives In this demonstration, a wound care physician shows how to apply multi-layer compression wraps for the treatment of venous ulcers. Chris Leonard, DO discusses venous ulcers, their causes and treatment options. This demonstration is performed by a trained wound care physician for educational purposes only and should not be tried at home. Video Text: This video shows a brief demonstration of a three-layer compression wrap which is the mainstay of treatment for the underlying venous insufficiency. Venous stasis ulcers are difficult to treat often because the underlying cause is not addressed. Various dressings can be used but it's very unlikely that they will heal unless the edema caused by the venous insufficiency itself is addressed. The deep venous system becomes incompetent and shunts blood to the superficial venous system. That causes visible varicosities. Because of the hydrostatic pressure, particularly when the lower extremities are in the dependent position, the interstitial fluid increases. That increases the distance from the capillary bed to the skin surface meaning that there's a greater diffusion distance for nutrients and important cells that are involved in the healing process to get to the skin. So, if a wound should form on the skin it's less likely that it will heal. This is one of the many reasons to treat the underlying edema associated with venous stasis. Secondly, after blood pools because of the hydrostatic pressure, hemosiderin deposits occur and that's where you get your discoloration and ultimately your brawny edema. The idea of a multi-compression wrap is that the initial layer is for protection of the skin and to some degree comfort. The second layer is generally a high stretch fiber that provides a constant but yet gradient of pressure which is high at the proximal end and continuously gets lower towards the knee. This creates a conduit and augmentation to venous return proximally. The third layer is generally a short stretch fiber which dynamically augments the pump action of the calf muscles for the blood back proximally towards the heart. It does not create a tourniquet effect and at rest it does not create undue pressures. But, during ambulation or during physiologic muscles movement it augments that pumping action to produce increased blood flow via the venous system back towards the heart. It's been shown that single layer compression wraps are much less effective than single layer graded compression wraps. And, multi-layer compression wraps are more efficacious than single layer. So, they have to multi-layer and also graded. What I mean by graded is that you want to have the highest pressure down here at the foot and ankle and the decreasing pressure as you come up so you don't create a tourniquet effect.

The first is that if you have to start taking medication to treat your diabetes, you’ve failed. The second is that if you have to increase the amount or number of medications you’re taking, your diabetes is somehow getting “worse”. Today we’re going to talk about why both of these beliefs just aren’t true and cover what you need to know about diabetes and medications… One consistent aspect of type 2 diabetes is that it will change over time because the genes that are responsible for diabetes cause changes inside your body that progress as time passes—over time, your pancreas will become less able to make insulin. This will take place at different rates in different people, but the important thing to know is that these changes don’t mean that you’ve failed to manage your diabetes… this is just what happens. What IS your responsibility though is to keep track of what’s happening with your numbers (your A1c, blood pressure, LDL cholesterol, kidney tests, and eye exam results) so that you know when they’re changing and can work with your doctor to change your diabetes treatment plan. When your lifestyle changes aren’t working to keep your numbers at your target, then it’s usually time to make changes to your medication regimen to get to your goal. Probably the most important thing you can learn about diabetes is that the only measure of success is what your A1c is—not how many medications you’re taking. someone who is on two medications and has an A1c of 7 is at a much lower risk of complications than someone who is on no medications but has an A1c of 8.5. There are many different types of medications that work effectively to treat your blood glucose and each of them has a different effect on your body so combining medications can often be a more effective way to lowering your A1c. Here are the different categories of medication currently on the market: Insulin, metformin, sulfonylureas, TZDs, GLP-1 agonists, DPP-4 inhibitors, SGLT-2 inhibitors, but here’s the main thing to know: consistently taking the medication that you and your doctor have decided on is the most important thing you can do to be healthy. If you’re concerned about something that you’re taking, talk to your doctor and work with him or her to figure out if a different medication would be a better choice. It may take some time to find the right type and amount of medication that will work to lower your numbers to your target, but stay vigilant—having your numbers at goal will help prevent future complications and enable you live a long, healthy life.
