New Advice Issued for Prostate Cancer Screening

(11 Apr 2017) An influential U.S. government health panel is dropping its opposition to routine prostate cancer screening in favor of letting men decide for themselves after talking with their doctor. The new draft guidelines echo those of several leading medical groups, but they don't make the decision any easier for men: With their doctor's help, they have to decide whether to take an imperfect PSA test that has a small chance of detecting a deadly cancer and a larger chance of triggering unneeded worry and treatment with serious side effects. "This isn't a one-size-fits-all" recommendation, said the panel's chair Dr. Kirsten Bibbins-Domingo, a San Francisco internist who already uses the advice and discusses the potential pros and cons with her patients. Men whose greatest concern is reducing their chances of dying from cancer are sometimes willing to face the consequences and choose testing. "Other men will realize the likely benefit is small and aren't willing to risk the harms," she said. PSA screening to detect the most common male cancer is among the most heated topics in men's health. It involves a simple blood test for elevated levels of a protein that may signal cancer but also can be caused by less serious prostate problems. It can find cancer that frequently doesn't need treatment because it's too small and slow growing to become deadly. Doctors say there's no good way to tell which early cancers might become lethal. The next step is often radiation or surgery to remove the prostate, which may result in impotence and incontinence. The U.S. Preventive Services Task Force says its latest recommendation is based on new evidence indicating that routine PSA blood tests can slightly reduce some men's chances of dying from prostate cancer and that drastic treatment can be avoided with close monitoring when cancer is detected. The shift shelves the panel's 2012 guidance, which prompted criticism from some urologists _ specialists who treat the disease _ and angered some prostate cancer patients certain that PSA screening had saved their lives. The new advice published Tuesday closely aligns the panel with medical groups that also support shared-decision-making. The biggest remaining difference is timing. The task force draft says screening conversations should happen starting at age 55. Other groups say start earlier, depending on family history of prostate cancer and other factors. It recommends against testing men aged 70 and older. The panel leaves open how often men should be screened. It does not recommend earlier testing for blacks and those with a family history but says they should know their risks are higher. For decades, doctors have been treating prostate problems - especially prostate cancer, with drugs that turn OFF your testosterone levels. In fact, it’s called “Medical Castration”. This is because early research in the 40’s showed that when a man no longer has any testosterone, prostate problems “decrease”. 5 Of course, if you think about it - this makes absolutely no sense!... Because if this was true and “high testosterone” is the problem, then guys in their teenage years and 20’s would all have prostate problems, since this is when a man’s testosterone is at its highest. Once activated, the androgen receptor stimulates the expression of specific genes that cause prostate cells to grow (2).” In fact, over 20 years ago when my dad had prostate problems, his doctor wanted to put him on “hormone blockers” which essentially turned off all of this male hormones and thus, “castrated” him. Now, I had just started my medical studies at the time. Yet I was very knowledgeable about how the human body worked, hormones, endocrinology and so forth because of all the professional athletes I had coached… So when I told my dad’s doctor my thoughts,... my “theories” about the cause AND prevention of prostate problems -- he listened carefully. But then he said something that pissed me off and changed my life forever. He said “Sam, this makes sense… what kind of doctor did you say you were?” Well, I had just started medical school and I said “I’m not a doctor…” and before I could say anything else, he quickly dismissed me and had a smirk on his face and said “oh, so you don’t know what you’re talking about… you have no proof” That day changed my life because I did everything I could from that day on to make sure that I get my medical degrees and thus, be able to also back up everything I said and never have someone say that again. But anyway, the point is that I told my dad to please trust me… not take the drugs and I promise he will never have prostate problems. So like any good parent, you know what he did?... He did NOT listen to me. So my dad took the testosterone blocking drugs, became utterly miserable and depressed. It caused problems with his “drive”, emotions, at work, with my mom and so on and so forth. He quickly lost muscle, become fat, started getting “man boobs’ (gynecomastia) and basically was turning into a woman. My dad is almost 90 years old - no prostate problems. Yet, his brother died of prostate cancer a few years ago, while still taking these stupid drugs. What’s the REAL cause of prostate problems?... The ancient Greek word for sperm means living been because you have this little cell with a tail that could actually swim like a mino carrying half of the genetic material to connect up with the ovum to fuse and form one body. The code for female and male is in the sperm. So this video is based on a book called The Great Prostate Hoax by Richard Albin PHD – this is the guy in 1970 who actually discovered PSA and he is writing a book about it. It is quite fascinating. So if you do research on PSA, they have certain blood values. You have Low risk: less than 10, Immediate Risk: 10-20 and High Risk: greater than 20. He found that these are arbitrary numbers, they are not based on fact, they are based on opinion because he found that you can have prostate cancer with low numbers and not have cancer with high numbers. There are many reasons for high PSA. Lets say for example the day before you went in a long jog or you are under stress, or you have inflammation, or you have an enlarged prostate gland – all of these could cause high PSA and as you get older, most men developed an enlarged prostate as the normal aging process. In fact he said this, some men died because of prostate cancer and many man that have prostate cancer die from old age with prostate cancer. The thing about prostate cancer is that its very rare for it to spread outside the gland itself. Immunotherapy prostate cancer drug gives 'amazing results'. Men given just months or weeks to live after being diagnosed with prostate cancer are surviving for more than a year thanks to a breakthrough in immunotherapy treatment, a trial has shown.Almost 40% of patients who spent 12 months on the drug pembrolizumab - known as a "checkpoint inhibitor" - as part of a new study were still alive and one in 10 had not seen the cancer grow. The 258 men with advanced prostate cancer were treated with the drug as part of a trial led by a team at the Institute of Cancer Research (ICR) in London and the Royal Marsden NHS Foundation Trust. The results - which are to be presented at the American Society of Clinical Oncology annual meeting in Chicago - have been lauded as "amazing". 'Holy grail' blood test detects cancer years before symptoms show. AI to be 'new weapon' in cancer fight. Professor Johann de Bono, director of the drug development unit, said: "I have these men who are basically dying, with weeks to months to live, who we gave this drug to and had complete responses. Researchers believe that a staggering 25,000 men would have been diagnosed with advanced prostate cancer in 2008, if not for the PSA. Dr. Samadi, a consistent supporter of routine PSA screening, commented on how these findings could impact the U.S. government task force's anti-PSA stance. "The clinical significance of this historical review of the PSA test is tremendous," he said. "Unfortunately, late stage diagnosis is extremely difficult to treat. Acknowledgement that the PSA test could be saving 17,000 lives each year isn't something you turn your back on." Now with this latest PSA test update report, this seems to vindicate my prior opinion. Personally, I think this whole non-testing stick your head in the San philosophy is just a misguided way by the government to try to save money. Naturally, this real reason is cloaked in their cover story that some people when diagnosed are getting unnecessary treatments. Now, while I don't disagree that that has been going on, I personally believe that it's probably a very small percent of the men that are diagnosed. Plus, I really don't see the majority of doctors being so unscrupulous as to suggest unnecessary treatment when they know that some types of prostate cancer may take so long to develop that the majority of patients will die from other causes. As I said in the video a large number of men today when first diagnosed, just select to keep an eye on their PSA levels by being tested every three months. Then, should the PSA level start to rise it may be time for the biopsy. From the biopsy samples, a Gleason score can be arrived at, that can then let the doctor and the patient make a very educated decision on whether to continue watchful waiting or go into a more active treatment option. But to suggest to men not to get an annual PSA test (keep in mind this is basically just a simple blood test) in my opinion is totally ludicrous! Please keep in mind that I'm not a doctor or medical professional and these are just my personal opinions. 4 Early Warning Signs of Prostate Cancer: Understanding Prostate Cancer Symptoms If you’re a man, the federal government estimates that you have about a one-in-seven chance of developing prostate cancer at some point during your lifetime. Based on 2012 data, nearly three million American men are currently living with prostate cancer. Although the condition is highly treatable, many men are suffering needlessly because they didn’t know the signs of cancer and catch it before it reached an advanced stage of cancer. Or they didn’t know the causes of prostate cancer and what to do to avoid it. Knowing how to identify the early-onset signs and symptoms of prostate cancer is critical. It could mean the difference between having to undergo invasive radiation treatment and surgery (and suffering through a laundry list of side effects in the process), or making simple dietary and lifestyle changes now to nip this cancer in the bud and stop it from growing and spreading. Where is Your Prostate? Only men have a prostate, which is a walnut-sized gland located directly under the bladder in front of the rectumm. How to Know if You Have Prostate Cancer: Is PSA Testing a Valid Risk Indicator? Getting your prostate-specific antigen (PSA) levels tested on a routine basis is what the medical system says is the best way to catch prostate cancer early and eradicate it with minimal intervention. High PSA levels are suggestive of prostate cancer onset, the public has long been told. Men who fall into this category are often encouraged to get biopsied and undergo invasive treatment like surgery and radiation. The problem is that a biopsy or the prostate “removal” operation can cause a dormant cancer to spread through the rest of the body. The PSA test is known as the “gold standard” for detecting prostate cancer. But is it really? Does a high PSA equal prostate cancer? This is an important question, because a high PSA leads most men straight to biopsies, then to “the knife,” and then straight to pain, incontinence, and erectile issues such as impotence. Of course, let’s not forget that these procedures will guarantee billions of dollars for your doctor and the medical industrial complex. According to recent articles in the New York Times and Washington Post, PSA tests are essentially worthless. You see, the PSA test simply reveals how much of the prostate antigen a man has in his blood, which is a marker of inflammation and can indicate cancer, but not necessarily. You see, infections, benign swelling of the prostate, and over-the-counter drugs (like Ibuprofen) are all factors that can elevate a man’s PSA level. Dr. Thomas Stamey of Stanford University was one of the original boosters of the PSA test. At a 2004 conference, he stated, “PSA no longer has a relationship to prostate cancer. The PSA test is not relevant any more. You might as well biopsy a man because he has blue eyes.” In fact, the PSA test has been such a dismal failure in detecting prostate cancer, its inventor (Richard J. Ablin) has been speaking out against his own discovery for more than a decade! Most recently, in a March 2010 edition of The New York Times, Ablin wrote, “The [PSA] test is hardly more effective than a coin toss. As I’ve been trying to make clear for many years now, PSA testing can’t detect prostate cancer…The test’s popularity has led to a hugely expensive public health disaster.” On a side note, a large body of evidence demonstrates that PSA is not a “prostate-specific” antigen at all. As a matter of fact, PSA has been shown to be expressed in many forms of female tissues. 4 Early Warning Signs of Prostate Cancer: Understanding Prostate Cancer Symptoms If you’re a man, the federal government estimates that you have about a one-in-seven chance of developing prostate cancer at some point during your lifetime. Based on 2012 data, nearly three million American men are currently living with prostate cancer. Although the condition is highly treatable, many men are suffering needlessly because they didn’t know the signs of cancer and catch it before it reached an advanced stage of cancer. Or they didn’t know the causes of prostate cancer and what to do to avoid it. Knowing how to identify the early-onset signs and symptoms of prostate cancer is critical. It could mean the difference between having to undergo invasive radiation treatment and surgery (and suffering through a laundry list of side effects in the process), or making simple dietary and lifestyle changes now to nip this cancer in the bud and stop it from growing and spreading. Where is Your Prostate? Only men have a prostate, which is a walnut-sized gland located directly under the bladder in front of the rectumm. It surrounds the urethra, the tube that carries urinee and semenn out of the body. The prostate gland can affect urinee control, known as continence. How to Know if You Have Prostate Cancer: Is PSA Testing a Valid Risk Indicator? Getting your prostate-specific antigen (PSA) levels tested on a routine basis is what the medical system says is the best way to catch prostate cancer early and eradicate it with minimal intervention. High PSA levels are suggestive of prostate cancer onset, the public has long been told. Men who fall into this category are often encouraged to get biopsied and undergo invasive treatment like surgery and radiation. The problem is that a biopsy or the prostate “removal” operation can cause a dormant cancer to spread through the rest of the body. The PSA test is known as the “gold standard” for detecting prostate cancer. But is it really? Does a high PSA equal prostate cancer? This is an important question, because a high PSA leads most men straight to biopsies, then to “the knife,” and then straight to pain, incontinence, and erectile issues such as impotence. Of course, let’s not forget that these procedures will guarantee billions of dollars for your doctor and the medical industrial complex. According to recent articles in the New York Times and Washington Post, PSA tests are essentially worthless. You see, the PSA test simply reveals how much of the prostate antigen a man has in his blood, which is a marker of inflammation and can indicate cancer, but not necessarily. You see, infections, benign swelling of the prostate, and over-the-counter drugs (like Ibuprofen) are all factors that can elevate a man’s PSA level. Dr. Thomas Stamey of Stanford University was one of the original boosters of the PSA test. At a 2004 conference, he stated, “PSA no longer has a relationship to prostate cancer. The PSA test is not relevant any more. You might as well biopsy a man because he has blue eyes.” In fact, the PSA test has been such a dismal failure in detecting prostate cancer, its inventor (Richard J. Ablin) has been speaking out against his own discovery for more than a decade! Most recently, in a March 2010 edition of The New York Times, Ablin wrote, “The [PSA] test is hardly more effective than a coin toss. As I’ve been trying to make clear for many years now, PSA testing can’t detect prostate cancer…The test’s popularity has led to a hugely expensive public health disaster.” On a side note, a large body of evidence demonstrates that PSA is not a “prostate-specific” antigen at all. As a matter of fact, PSA has been shown to be expressed in many forms of female tissues. The PSA test only has a small percent chance of detecting cancer, but often times the procedure causes several unwanted side effects. 4 Early Warning Signs of Prostate Cancer: Understanding Prostate Cancer Symptoms If you’re a man, the federal government estimates that you have about a one-in-seven chance of developing prostate cancer at some point during your lifetime. Based on 2012 data, nearly three million American men are currently living with prostate cancer. Although the condition is highly treatable, many men are suffering needlessly because they didn’t know the signs of cancer and catch it before it reached an advanced stage of cancer. Or they didn’t know the causes of prostate cancer and what to do to avoid it. Knowing how to identify the early-onset signs and symptoms of prostate cancer is critical. It could mean the difference between having to undergo invasive radiation treatment and surgery (and suffering through a laundry list of side effects in the process), or making simple dietary and lifestyle changes now to nip this cancer in the bud and stop it from growing and spreading. Where is Your Prostate? Only men have a prostate, which is a walnut-sized gland located directly under the bladder in front of the rectumm. It surrounds the urethra, the tube that carries urinee and semenn out of the body. The prostate gland can affect urinee control, known as continence. How to Know if You Have Prostate Cancer: Is PSA Testing a Valid Risk Indicator? Getting your prostate-specific antigen (PSA) levels tested on a routine basis is what the medical system says is the best way to catch prostate cancer early and eradicate it with minimal intervention. High PSA levels are suggestive of prostate cancer onset, the public has long been told. Men who fall into this category are often encouraged to get biopsied and undergo invasive treatment like surgery and radiation. The problem is that a biopsy or the prostate “removal” operation can cause a dormant cancer to spread through the rest of the body. The PSA test is known as the “gold standard” for detecting prostate cancer. But is it really? Does a high PSA equal prostate cancer? This is an important question, because a high PSA leads most men straight to biopsies, then to “the knife,” and then straight to pain, incontinence, and erectile issues such as impotence. Of course, let’s not forget that these procedures will guarantee billions of dollars for your doctor and the medical industrial complex. According to recent articles in the New York Times and Washington Post, PSA tests are essentially worthless. You see, the PSA test simply reveals how much of the prostate antigen a man has in his blood, which is a marker of inflammation and can indicate cancer, but not necessarily. You see, infections, benign swelling of the prostate, and over-the-counter drugs (like Ibuprofen) are all factors that can elevate a man’s PSA level. Dr. Thomas Stamey of Stanford University was one of the original boosters of the PSA test. At a 2004 conference, he stated, “PSA no longer has a relationship to prostate cancer. The PSA test is not relevant any more. You might as well biopsy a man because he has blue eyes.” In fact, the PSA test has been such a dismal failure in detecting prostate cancer, its inventor (Richard J. Ablin) has been speaking out against his own discovery for more than a decade! Most recently, in a March 2010 edition of The New York Times, Ablin wrote, “The [PSA] test is hardly more effective than a coin toss. As I’ve been trying to make clear for many years now, PSA testing can’t detect prostate cancer…The test’s popularity has led to a hugely expensive public health disaster.” On a side note, a large body of evidence demonstrates that PSA is not a “prostate-specific” antigen at all. As a matter of fact, PSA has been shown to be expressed in many forms of female tissues.
