Endometrial ablation should never be performed on women who wish to have children. Endometrial ablation is most often employed in women who suffer from excessive menstrual bleeding, who have failed medical therapy and do not wish to undergo a hysterectomy. Heavy menstrual bleeding is most commonly due to dysfunctional uterine bleeding or adenomyosis. The procedure is almost always performed as an outpatient treatment, either at the hospital, ambulatory surgery center, or physician office. The Endometrial Ablation procedure is primarily performed while patients are under local and/or light sedative anesthesia, or if necessary, general or spinal anesthesia. Patients normally leave the treatment facility within one hour following the procedure and generally spend one day resting at home, before returning to the activities of daily living. After the procedure, the endometrium heals by scarring over, thus reducing or eliminating future uterine bleeding. The patient's hormonal functions will remain unaffected because the ovaries are left intact. Placenta accreta may occur if the patient becomes pregnant after endometrial ablation, so birth control is necessary. Depending on the treatment that is chosen, endometrial ablation is sometimes conducted after treatment with hormones, such as norethisterone or Lupron to reduce the thickness of the endometrium. The U.S. Food and Drug Administration (FDA) approves and audits clinical studies to test and evaluate the effectiveness of all endometrial ablation treatments. Two patient effectiveness outcomes are measured at one year following treatment: 1.) Success Rate = the % of women who have their bleeding reduced to a normal period level or less, and 2.) Amenorrhea Rate = the % of women that have their bleeding completely eliminated. According to the results of the Randomized Controlled Trials performed for the FDA approval of the different treatment options, effectiveness Success Rates range from a high of 93% to a low of 67%, and the Amenorrhea Rates range from a high of 72% to a low of 22%. A number of treatment options are available. They all work by destroying (ablating) the endometrium, the lining of the uterine cavity. 1. The Minerva - Endometrial Ablation System (Minerva Surgical), FDA approved in July 2015, is the first new FDA-approved surgical treatment for heavy menstrual bleeding in over 15 years. Minerva works by generating heat from plasma energy that is created and contained inside a leak-proof ablation array that takes the shape of the uterine cavity. The hot membrane surface of the array ablates (destroys) the endometrium. The Minerva procedure is the fastest FDA approved treatment, average procedure time is 3.1 minutes from device insertion to removal, and is usually performed under local and/or conscious sedation anesthesia. Most patients leave the treatment center within one hour of treatment. In the Minerva Randomized Controlled Trial for FDA approval, the Success Rate was 93% (bleeding reduced to a normal or less level) and Amenorrhea Rate was 72% (bleeding completely eliminated). In addition, 95% of the Minerva patients, "Would recommend Minerva to a family member or friend") and 92% of the patients were Satisfied.....

The NovaSure procedure is a quick, safe, simple, one-time endometrial ablation treatment. This is a minimally invasive procedure that controls heavy bleeding by using radio-frequency energy to remove the hormonally active lining of the uterus. Here's how it works: "We use a radio frequency probe which takes about 90 seconds. The patients sometimes do it under local anesthesia, meaning they get an injection, so they're numb. The instrument is placed into the uterus and the electrical current is used that removes the endometrio," says Dr. Halpren. For women who are ready to part with their periods, this is an option. But endometrial ablation is not for someone who wants to have children. The procedure permanently destroys the uterine lining, making them infertile. "This is not a sterilization procedure, but the uterus can no longer carry a pregnancy," says Dr. Halpren. However, the ablation technique may 'deliver' a welcome bonus. "Patients that have good results from endometrial ablation may have some decrease in subjective symptoms like migraines, PMS. But we don't do it for that reason," says Dr. Halpren. It sounds like you've had some sort of procedure that permanently sterilized you and also an endometrial ablation in the past, both of which should make it so you can't have any more children. But, as we know, there are exceptions to all rules and I'm a labor and delivery nurse and I've seen women have babies when there was, you know, logically speaking, no chance that they could have. So there's always a small percentage of people who get pregnant after permanent sterilization procedures. You mentioned that you're normally like clockwork and you've missed your last period. It would be best for you to either take a pregnancy test or talk with your doctor. If the pregnancy test comes back negative and you still continue to not have a period, then, again, this is a reason to talk with your doctor because they can ask you more specific questions about your cycles, take your health history into account, and also determine if there's maybe medications that you've been on that might affect it. There's a ton of different factors that can go into irregular cycles. Stress, travel, medications--- there's tons of different things. So they can help you examine different things in your life that might factor into everything and then decide if further investigation or treatment is warranted. Comprehensive OB-GYN uses NovaSure to Help Busy Women Stay Active and "On the Go" All Month Women are starting to discover an in-office procedure that can help cure their heavy bleeding with little downtime, endometrial ablation. Endometrial ablation is a procedure that destroys a thin layer of lining of the uterus. This stops the endometrium from building up each month and then sloughing off, resulting in menstrual bleeding. It's a great alternative to a hysterectomy and endometrial ablation treats heavy menstrual bleeding, which in turn reduces menstrual cramps. The procedure can be done on pre-menopausal women of any age, but they have to be done having children and cannot desire future fertility. However, endometrial ablation is not considered contraception. While it's very unlikely to become pregnant after having the surgery, it's possible. Unfortunately for many women, their periods interfere with their lives in a major way. Cramps can be so bad that some women spend the first day of their period in bed. Also, their bleeding can be so heavy that it is hard to do the activities they enjoy. Frequently, many women with heavy periods have to skip doing planned activities both for work and pleasure. Heavy periods can make it difficult to feel like a good mom and to be there for their children who need their mom to be active and on the go. Dr. David Adler and Dr. Greg Latchaw at Comprehensive OB-GYN of the Palm Beaches (www.comprehensiveobgyn.net) in Loxahatchee perform endometrial ablation right in their offices, under local anesthesia. Endometrial ablation can be done several ways but the Doctors at Comprehensive OB-GYN use a device called NovaSure (www.novasure.com). The slender wand is inserted into the uterus and then expands into a triangular mesh device that touches all the walls of the uterus. Current runs through the mesh, creating a thermal destruction of the uterus' lining. It literally cauterizes the inside of the uterus. Then the physician looks inside the uterus using a device called a hysteroscope to make sure the entire lining has been destroyed. Dr. Adler and Dr. Latchaw's goal is that patients have no more menses after the procedure, and they are usually able to achieve that. Patients can expect to be at the office for an hour for the procedure, with the procedure itself taking about 15 minutes. They may experience some cramping within the first 24 hours after the procedure, which is treated with pain medication. Usually by the 24-hour mark, it's back to life as usual, except without the debilitating period every month. Some women may experience pain for longer than 24 hours, however, the practice has also had patients who have the procedure done on a Friday and are off to the gym on Saturday. Heavy menstrual bleeding can lead to anemia and fatigue. Comprehensive OB-GYN has found that many women's lives are severely affected by heavy bleeding. It's probably in the top three complaints Dr. Adler hears from his patients. "Many women can go much of their life without having problem periods and then suddenly start experiencing heavy bleeding in their 40s," says Dr. Adler. The problem also can be treated by birth control or hysterectomy. A hysterectomy requires two to four weeks of down time and if the ovaries are also removed, some women may need hormone replacement therapy. Endometrial ablation leaves the ovaries intact, and women still ovulate every month and don't have hormonal changes. The procedure is covered by most health insurance.

Abnormal uterine bleeding generally occurs secondary to either hormonal irregularities or pathology, which includes benign and malignant tumors. The source of bleeding is most commonly from the lining of the uterus, also called the endometrial cavity or endometrium. In the past, hysterectomy procedures have been performed in most cases of refractory bleeding that persists after conservative measures such as hormonal therapy and minor procedures such as cleaning the lining with dilatation and curettage (D&C). More recent advances in the application of heat energy to the endometrial cavity have allowed for a third option now called endometrial thermal ablation. With this procedure, an inflatable balloon can be advanced into the uterus and used to apply thermal energy to the lining. When treated in this way, women may achieve significant reduction in the amount of bleeding during menses, and also have a remarkable decrease in the amount of pain during periods.

ABNORMAL VAGNIAL BLEEDING Symptoms, Causes and Treatments OVERVIEW Abnormal vaginal bleeding is any vaginal bleeding unrelated to normal menstruation. This type of bleeding may include spotting of small amounts of blood between periods — often seen on toilet tissue after wiping — or extremely heavy periods in which you soak a pad or tampon every one to two hours for two or more hours. CAUSES Abnormal vaginal bleeding can relate to an issue with your reproductive system (a gynecologic condition) or to other medical problems or certain medications. If you have reached menopause — defined as 12 consecutive months, give or take, without a menstrual period — subsequent vaginal bleeding may be a cause for concern and should be evaluated. Possible causes of abnormal vaginal bleeding include: • Cancers and precancerous conditions • Cervical cancer • Endometrial cancer (uterine cancer) • Endometrial hyperplasia • Ovarian cancer • Uterine sarcoma • Vaginal cancer TREATMENT This will depend on the cause of the abnormal uterine bleeding -- if a chronic illness or a blood disorder is at the root of your symptoms, treating that can help. Your treatment also can depend on whether you plan to have children. It might not be safe to get pregnant after some treatments, while others can make it impossible. If you’re close to menopause, your doctor may want to take a wait-and-see approach because your symptoms may get better on their own. Drugs are usually the first thing your doctor will try: Hormones. Birth control pills and other hormone treatments may be able to give you regular menstrual cycles and lighter periods. Gonadotropin-releasing hormone agonists (GnRHa). These stop your body from making certain hormones. They can shrink fibroids for a while, but they’re usually used along with other treatments. NSAIDS. If you take anti-inflammatories like ibuprofen or naproxen a few days before your period starts, they may help lighten the bleeding. Tranexamic acid. This is a pill that helps your blood clot and can control heavy uterine bleeding. Sometimes surgery can be needed to stop the bleeding: Endometrial ablation. This uses heat, cold, electricity, or a laser to destroy the lining of the uterus. It may end your periods entirely. You probably won’t be able to get pregnant after having it done, but it can be very dangerous if you do. You’ll need to use birth control until menopause. Myomectomy or uterine artery embolization. If you have fibroids, your doctor may take them out or cut off the vessels that supply them with blood. Hysterectomy. This is when your uterus is taken out. You may need a hysterectomy if your fibroids are very large or you have endometrial or uterine cancer. Otherwise, it’s a last resort when other treatments haven’t worked.
