Endometrial ablation should never be performed on women who wish to have children. Endometrial ablation is most often employed in women who suffer from excessive menstrual bleeding, who have failed medical therapy and do not wish to undergo a hysterectomy. Heavy menstrual bleeding is most commonly due to dysfunctional uterine bleeding or adenomyosis. The procedure is almost always performed as an outpatient treatment, either at the hospital, ambulatory surgery center, or physician office. The Endometrial Ablation procedure is primarily performed while patients are under local and/or light sedative anesthesia, or if necessary, general or spinal anesthesia. Patients normally leave the treatment facility within one hour following the procedure and generally spend one day resting at home, before returning to the activities of daily living. After the procedure, the endometrium heals by scarring over, thus reducing or eliminating future uterine bleeding. The patient's hormonal functions will remain unaffected because the ovaries are left intact. Placenta accreta may occur if the patient becomes pregnant after endometrial ablation, so birth control is necessary. Depending on the treatment that is chosen, endometrial ablation is sometimes conducted after treatment with hormones, such as norethisterone or Lupron to reduce the thickness of the endometrium. The U.S. Food and Drug Administration (FDA) approves and audits clinical studies to test and evaluate the effectiveness of all endometrial ablation treatments. Two patient effectiveness outcomes are measured at one year following treatment: 1.) Success Rate = the % of women who have their bleeding reduced to a normal period level or less, and 2.) Amenorrhea Rate = the % of women that have their bleeding completely eliminated. According to the results of the Randomized Controlled Trials performed for the FDA approval of the different treatment options, effectiveness Success Rates range from a high of 93% to a low of 67%, and the Amenorrhea Rates range from a high of 72% to a low of 22%. A number of treatment options are available. They all work by destroying (ablating) the endometrium, the lining of the uterine cavity. 1. The Minerva - Endometrial Ablation System (Minerva Surgical), FDA approved in July 2015, is the first new FDA-approved surgical treatment for heavy menstrual bleeding in over 15 years. Minerva works by generating heat from plasma energy that is created and contained inside a leak-proof ablation array that takes the shape of the uterine cavity. The hot membrane surface of the array ablates (destroys) the endometrium. The Minerva procedure is the fastest FDA approved treatment, average procedure time is 3.1 minutes from device insertion to removal, and is usually performed under local and/or conscious sedation anesthesia. Most patients leave the treatment center within one hour of treatment. In the Minerva Randomized Controlled Trial for FDA approval, the Success Rate was 93% (bleeding reduced to a normal or less level) and Amenorrhea Rate was 72% (bleeding completely eliminated). After years of living like that, Cathy was at a breaking point. But endometrial ablation is not for someone who wants to have children. The procedure permanently destroys the uterine lining, making them infertile. "This is not a sterilization procedure, but the uterus can no longer carry a pregnancy," says Dr. Halpren. However, the ablation technique may 'deliver' a welcome bonus. "Patients that have good results from endometrial ablation may have some decrease in subjective symptoms like migraines, PMS. But we don't do it for that reason," says Dr. Halpren. It is a very common symptom that women experience. However, it is a very subjective symptom. It is difficult to objectively measure the amount of blood lost during the period but anywhere between 30-60 ml is considered normal and if it exceeds 80 ml then that is considered as a heavy period. A detailed history is taken to understand heaviness. How many pads you use? Whether you combine it with tampons? Is it the extra large that you use? Do you get up in middle of the night to change your pads? Do you experience flooding that is soaking through your clothes? How often you do you need to change the bed linen or clothes etc. Association of clots with flooding is also an indication of heavy period. Fatigue, shortness of breath and tiredness also indicate heavy blood loss. Pain is a common symptom but if the pain is getting worse month on month or if it is starting prior to the period itself it may need to be checked out. Many times it is difficult to pinpoint the cause for heavy period, some conditions like Poly Cystic Ovarian Syndrome, fibroids, endometrial polyps which are small grape like growths within the endometrium, coagulation problems , and hypothyroidism may contribute to heavy menstrual bleeding, very rarely it may indicate endometrial cancer or cancer of womb. Detailed history is taken and baseline blood tests such as thyroid and hemoglobin. Scan is avoided something abnormal is seen during the examination. Mostly treatment is not necessary. Watch and observe is done. If treatment is required one starts with simple medications to reduce the flow, this helps up to 50-70%. If simple medications don’t work Medication with hormones are tried. If this does not help then intra uterine system is tried. The last resort is endometrial ablation or hysterectomy. Heavy periods associated with pain and getting worse month on month may need to be checked out.

A normal woman will have menstrual cycle lasting for 2 to 3 days and she will have about 80mL of blood loss. If the blood loss exceeds 80mL or more than 100mL it is called heavy menstrual bleeding. In India it is the commonest cause of anaemia in women. It can affect any age group. If it affects younger age group we call it puberty menorrhagia. There are various causes for this. One important cause is haemophilia and polycystic ovarian disease. We need to treat these patient with haemostatic drugs and hormonal drugs. The commonest age group for menorrhagia is after the age of 40. The cause cannot be found out and the treatment can be medical treatment, hysteroscopy followed by D&C and uterus removal depending upon the response of the patient. Fibroid is a frequent cause for heavy menstrual bleeding in this age group also. Endometrial polyps are one of the important cause for heavy menstrual bleeding around the age group of 40 to 50. So removal of the polyp or fibroid can cure the heavy bleeding and at the same time the patient can opt for uterus removal at this age. Another important cause for heavy bleeding around the perimenopausal age group is the cervical cancer and the endometrial cancer. So we have to rule out in all the patients about 40 years the possibility of endometrial cancer. For this we do hysteroscopy and endometrial biopsy and make sure the patient don’t have malignancy. At this age group 40 to45 when there is no identifiable pathology and the patient is not responding to the medicine there is another treatment called endometrial ablation wherein only the inner layer of the uterus is cauterised and removed. There are various methods by which endometrial ablation can be done. It can be done with hysteroscopy, balloon ablation or various other methods. So heavy bleeding is an important cause for anaemia in India and we have to group the patient in adolescent, middle age patient and perimenopausal patient and individualise the treatment and the treatment could be medication or hysteroscopic treatment or removal of the uterus or hysterectomy.

It sounds like you've had some sort of procedure that permanently sterilized you and also an endometrial ablation in the past, both of which should make it so you can't have any more children. But, as we know, there are exceptions to all rules and I'm a labor and delivery nurse and I've seen women have babies when there was, you know, logically speaking, no chance that they could have. So there's always a small percentage of people who get pregnant after permanent sterilization procedures. You mentioned that you're normally like clockwork and you've missed your last period. It would be best for you to either take a pregnancy test or talk with your doctor. If the pregnancy test comes back negative and you still continue to not have a period, then, again, this is a reason to talk with your doctor because they can ask you more specific questions about your cycles, take your health history into account, and also determine if there's maybe medications that you've been on that might affect it. There's a ton of different factors that can go into irregular cycles. Stress, travel, medications--- there's tons of different things. So they can help you examine different things in your life that might factor into everything and then decide if further investigation or treatment is warranted. Endometrial ablation is a procedure that destroys a thin layer of lining of the uterus. This stops the endometrium from building up each month and then sloughing off, resulting in menstrual bleeding. It's a great alternative to a hysterectomy and endometrial ablation treats heavy menstrual bleeding, which in turn reduces menstrual cramps. The procedure can be done on pre-menopausal women of any age, but they have to be done having children and cannot desire future fertility. However, endometrial ablation is not considered contraception. While it's very unlikely to become pregnant after having the surgery, it's possible. Unfortunately for many women, their periods interfere with their lives in a major way. Cramps can be so bad that some women spend the first day of their period in bed. The slender wand is inserted into the uterus and then expands into a triangular mesh device that touches all the walls of the uterus. Heavy menstrual bleeding can lead to anemia and fatigue. Comprehensive OB-GYN has found that many women's lives are severely affected by heavy bleeding. It's probably in the top three complaints Dr. Adler hears from his patients. "Many women can go much of their life without having problem periods and then suddenly start experiencing heavy bleeding in their 40s," says Dr. Adler. A hysterectomy requires two to four weeks of down time and if the ovaries are also removed, some women may need hormone replacement therapy.
