Lose Weight Before Joint Replacement Surgery!? Why is it important? What happens if you can’t lose weight before surgery? This is a common scenario for those preparing for knee replacement surgery or hip replacement surgery. This is especially true for patients in the age group of 65 or younger. In the younger demographic, surgeons want to maximize the longevity of the knee replacement. A decent weight range for a joint replacement is a BMI of less than 40. If you’ve got a lower BMI, your knee or hip implant will last longer. If you have a BMI above 40 it more than doubles your risk of postoperative infection. About 1-2% of all patients develop an infection after surgery. Infections will always require more surgery. In someone who has a BMI of over 40, the infection rate goes from 1 in 50 to 1 in 25. Getting in a safe weight range minimizes your risk and lowers the odds that you will face problems after surgery. Improving your weight will significantly improve your outcome. At one point, John Allaire was so morbidly obese he wore 8XL shirts and had to be weighed on a commercial scale at a fish market, as standard scales would not hold his weight. But after joining a fitness programme and dropping from 540 pounds to a lean 230 lbs, the 27-year-old was left with severe excess skin. John underwent skin removal surgery in March which saw the bulk of the loose skin on his torso and arms removed – but will require further surgery in the future to remove the skin on his lower body. Knee feels Heavy and tight after a total or partial knee replacement? If you ask 100 people that have had total knee replacement surgery (knee arthroplasty) about 70% will say that they’re aware of their knee. Whether it’s stiffness, pain or swelling, patients say their knee is noticeable on the average day. Majority of patients after knee surgery will perceive some difference from their “natural knee” to their prosthetic knee. However, if you ask patients if they are in significantly less pain or if they’d recommend a total knee replacement to others, they’ll almost certainly say “yes”. Patients should see gains made to the feeling of their prosthetic knee up to 12-18 months after surgery. In this time patients will notice their artificial knee prosthesis less and less. In saying this, most patients will have times of the day or week when they will notice their knee replacement. Heaviness and tightness, although not anything a patient looks forward to, is a part of recovery from joint replacement surgery. When I went into my tummy tuck surgery (May 10th) I weighed about 161 and about the same when I went into knee surgery (Aug 22nd). Since I have been unable to work out and haven't been watching what I eat following knee surgery I have gained weight. So now its time to get back in the gym and get the weight off. During the first week, you may find it easy to lose weight by simply switching to a healthier diet. But as your metabolism adjusts, you won’t burn as many calories as you used to. So losing additional weight will become harder. Making matters worse, as the fat melts away, you’ll start to experience an increase in appetite. After a meal, fat cells release a hormone called leptin into the bloodstream. This surge in leptin levels signals to your brain you’re full and should stop eating. But with less overall fat, people who lose weight show a measurable dip in leptin. Brain scans of obese patients who had lost 10% of their body weight revealed that less leptin leads to increased activity in regions of the brain that control our desire to eat. The result isn’t just an increased appetite but an even stronger urge to eat fatty, high-calorie foods, because your brain is trying to restore the body’s leptin levels to normal. However, fighting that early impulse to gorge on pizza and donuts is worth it in the long run. Besides the decreased risk of heart disease, hypertension, high cholesterol and type 2 diabetes, scientists studying overweight people discovered that losing just one pound of body weight reduces four pounds of pressure on knee joints. Losing excess weight also reduces strain on the blood vessels, increases blood flow to the brain, and boosts overall brain function. Several studies have shown that people who underwent weight-loss surgery saw an improvement in memory, concentration, and problem-solving skills in as soon as three months. Plus, brain scans indicate that people who lost weight and kept it off for nine months reacted differently when shown images of high-calorie foods than before they lost the weight. The brain regions that process reward, motivation, and taste didn’t react as strongly, whereas the areas that promote overall self-control had a boost in activity. So fighting those cravings early on might make them easier to control later. Turns out — like anything else — losing weight can get easier with practice.

Exercise After Weight Loss Surgery - Lower Body Exercises | Blossom Bariatrics | Las Vegas Welcome to Blossom Bodies. Today we have two really amazing guest trainers with us, Kristen and Tiff from Riot Conditioning. They're here to show you some really great tips. Okay, today we're going to talk to you about some lower body movements to start with. We're going to work these major muscle groups. It's really important to start with the major muscle groups because those are key to strengthening your joints. So many of you might have issues with your knees or your hips or your ankles. We start with these movements and do them slow and controlled to make sure that you implement them correctly and that you do the right form to strengthen your quads, your hamstrings, and your glutes. We'll start with a lunge. So, today we're just going to use this bench, but you could use a chair. So you're going to hold on to the chair or this bench and just do the lunge with the knee coming all the way down to the ground, stepping up. We're going to have you do ten on one side and then you'll switch and do ten on the other side. Great, the next exercise will be just a squat. Now you can take a chair right behind you. We'll use this bench again and you're going to make sure your feet are shoulder width apart and you're going to go ahead and sit down in that chair and come up. Now really focus on the muscle that your working right now, which would be my quadriceps and my glute muscles. Make sure you squeeze those and your out of your knees. You're not using your knees. She's also keeping her chest up and not bending over, but sitting back. Again, we'll go for ten reps and you can do that three times. Great, and our last exercise is.. Calf raises. .Calf raises. We can do both feet at the same time, or single legged calf raise. So we're going to squeeze at the top, slowly release it, and we're going to do that for 10 reps, three times. Okay, so those are three lower body exercises you can implement to really start to strengthen those major muscle groups. It's really important to really secure those joints. He discusses exercise, diet and nutrition to help with losing weight. For more information watch this video. Transcription of video: How can I lose weight before orthopedic surgery? That's a common question and a common desire before having surgery is to trim down a little bit. My name is Dr. Dan Albright. I'm an orthopedic surgeon in Raleigh, North Carolina and I see a lot of patients normal weight, underweight and overweight. Losing weight before surgery can really help you do better at surgery. The first way is get in the pool. Indoor pool, it's got to be an indoor pool because if you wait for outdoor it's going to rain or it's not going to be sunny, or warm enough or something. So indoor pool. I have a lot of patients with arthritis, crippling pain in the hip, knee back and getting in a pool takes weight off your joints and you can move. You can actually get a good muscle workout and I promise, you get in the pool, you will sleep better, think better, you'll feel better, you'll trim down, your muscle tone will get better. You'll be a safer patient for surgery. So it's really important, get in the pool. If not that, then a stationary bike, but not everybody can do a stationary bike. The pool is the place. You've got to walk in the pool. You've got to dance in the pool, run in the pool, swim in the pool anything in the pool. Water aerobics, pool therapy. I do pool therapy all the time. That's part one. Part two is what you put in your mouth. That's a big part of nutrition and health and I am anti-gluten. We were raised throughout our whole lives as children and all. Healthy whole grains, healthy whole grains. And the government and everyone was saying that. Well, grains are not healthy. And there's a lot of science coming out in the last several years grains are not healthy. So losing weight, I invite you to please go one month gluten-free. You've heard of gluten. Gluten is a reactive protein in wheat and bread and pasta. Go gluten-free for one month and you'll be amazed. One, you feel better, you'll think better, you'll lose 20 pounds without even trying and you'll be safer for surgery. So that means no bread, no pasta, no wheat. Read the label if it's processed food almost always it has wheat in it. You've got to eat whole food. You have to eat good food, good calories, healthy calories. That's the main thing is gluten-free. Now I'll just tell you, gluten goes to your brain. It occupies the morphine receptors in your brain and then you're addicted to bread and pizza. At least I was when I was doing it. And also it gives you a sugar rush, which is a problem and it's inflammatory. Gluten actually inflames tissues, it makes you swollen and puffy and bloated in at least one out of three Americans. So there's several reasons to stop gluten. So that's my main message here for weight loss. The other part is just sugar and carbohydrates. Pure sugar is not good. It shoots up your insulin. Insulin is called the fat hormone. So if you want to get fat, get the fat hormone going. Which is a lot of insulin. The only way to get your insulin down is to go less sugar, less carbohydrates. Again a lot of science exists for this and a lot of good books on this topic. So I have an interest in doing safe surgery and I want my patients to trim down some. So losing weight before surgery is good. This is how: get in the pool, tone up, go gluten-free for a month and think about less sugar. And that will do it. I've got many patients who've done it and they've been very thankful for that. I'm happy to talk to you more about that, about how to make you a safer person to undergo a surgery procedure. After the surgery patient is given Walker for 15 days to walk with support at the end of 15th day the staples are removed, the dressing is removed and the patient is given a walking stick to walk with, it may be used for a few months of time. The most common rules after the joint replacement surgeries are exercise and activities within limits of pain at least for a period of 3 to 6 months from the date of surgery, exercises are very important to build muscles around the knee joint so that the knee joint implanted is unloaded by the muscle strength. Just in case patient fails to exercise the implant lasts for 20 years may wear out in 15 years. So the most important thing to be done after knee joint surgery exercises a fix protocol of exercise which has to be followed, the physiotherapist is well trained and he basically teaches you the exercises you have to do registered, weightbearing, teraband everything will be thought to you and that is the only thing you are supposed to do, walking within the limits of pain is absolutely no issue, work as long as your knees don't hurt but don't exert yourself because pain is the only way your body can speak to you. Dont's after knee joint surgery immediately post operative squatting or sitting on floor is not allowed, even if a high flection knee, the knee joint squatting or sitting on floor at least for first 3 months. Once the tissues are healed the sitting and other things can be done. Running, jogging, weight bearing exercises like aerobics, kick boxing are not allowed after the surgery be cause the implants wear out soon and cement matrix under the implants develops fractured and will loosen the implants more sooner than they are expected to do.Otherwise there are no major restrictions patients are allowed to cycle, walk, swim, all exercises can be done without having the thumping action at the knee joint.

I read the article when Dr. Linda Bacon, author of Health at Every Size: The Surprising Truth About Your Weight and ally of the Association of Size Diversity and Health shared the link on her Twitter. My warning to anyone considering weight loss surgery as a remedy to their eating disorder is that you will still have to do the hard work of overcoming your eating disorder even after surgery. Your eating habits will change but your need to self soothe and cope with emotions doesn't change. As I explain in the video you can still suffer from a binge eating disorder even if you can only consume a quarter cup of food at a time. When that sprang a leak I received another gastric band via open surgery in 1993. After the port flipped over inside me I decided to just live with it rather than undergo yet another surgery. My weight reached its highest (400+ pounds) in 2006 as the old gastric band dug painfully into my stomach causing adhesions and complications that scared off most of the surgeons I approached. Finally had a revision from the band to an RNY gastric bypass in 2006 (by the very competent Dr. Daniel Davis). Riding the honeymoon-high of rapid weight loss from post-op starvation and energy drinks to fuel my compulsive exercising, I lost over 140 pounds in under 2 years. Then I hit a plateau and my energy bottomed out. Half the weight came back even though I consumed a restricted amount of food. My body began to break down from fatigue and lack of nourishment. My hair fell out and my complexion looked grey (the WLS world calls it 'grey ghost syndrome'). I spent 3 years in a wheelchair due to severe arthritis and multiple knee surgeries. I hold two Master’s degrees: an M.T.S. in Theological Studies from Drew University, '02, and an M.A. in Counseling and Human Services from Montclair State, '06. I also have my B.A. in Philosophy and Women's Studies. I was honored with the Tikkun Olam Award for my interfaith work in 2012. I received the ‘Mirror Mirror’ Award in 2009 for my work in body positivity and eating disorder recovery. In a second study, the researchers "looked at the effect of weight loss on 10 morbidly obese people with knee osteoarthritis and pain" who had lost an average of "51 pounds a year after gastric bypass surgery." Although there was some pain and physical function score improvement, scores on symptoms, function and activity did not improve."

This was Six weeks post-op (around 06/13/13). I was terrified of having my staples removed after surgery. In fact, I freaked out when we arrived, but I swear I was in 0 pain. NONE. I could not get anyone to explain how this felt on any of the message boards, so I decided to record my own experience!
