That is why antidepressants aim to increase the levels of these neurotransmitters in the synaptic cleft. They do this by slowing the reuptake of the neurotransmitters so that they stay in the cleft longer and interact with post synaptic receptors more often. The first drugs in this group were non-specific and increased all of the monoamines, which lead to lots of side effects and safety issues related to toxicity. Newer antidepressants are more selective and mostly only effect 1 or 2 monoamines. General principles: Unfortunately, antidepressants take at least a month to start working. Good patient education about the delayed onset of effect and close monitoring of the patient during this initial period is extremely important. Patients can become hopeless if they expect the drug to start working right away. This may be one reason why antidepressants are associated with suicide, especially in patients 25 years old and younger. Another proposed mechanism is that a depressed person may have the energy to carry out their suicide once the medications start to work. There is now a black box warning for suicide on antidepressants. Some psychiatrists argue that they don’t actually see this association with suicide in clinical practice, and that the thing that really increases the risk for suicide is not treating a depressed person with the proper medications. However, it is still standard practice to have a close follow up with patients you are starting on antidepressants. Usually this will involve a follow up visit about 2 weeks after the medication is started. At this visit the drug will not have started working yet so you can’t evaluate efficacy, but you can monitor for side effects like suicidality. Another serious side effect you have to be on the lookout for soon after initiating treatment is mania. If a bipolar individual is incorrectly diagnosed as having depression, an antidepressant may induce a manic episode. Another very serious side effect that has to be considered for antidepressants is Serotonin Syndrome. This usually occurs when you combined multiple antidepressants at the same time or combine an antidepressant with another medication that increases serotonin such as dextromethorphan or an opioid. It presents with tremor, diaphoresis, tachycardia, flushing and hypertension. If not corrected it can progress to delirium, AMS and death. Treatment includes medication cessation and the use of Cyproheptadine (a serotonin antagonist). In order to prevent this from happening you should have about a month “Wash Out” period when you are switching between antidepressants. So you taper the 1st medication down and then stop it, give the patient at least a month with no antidepressant and then start adding the new medication slowly. Most side effects begin immediately after starting the medication, but diminish over the course of a month. This is another reason why patient compliance is poor with these meds. It makes them sick and the drug doesn’t work during the first few weeks. However, if they can stick with it the medications will likely start working and the side effects will diminish over time. A principle that applies to all of the antidepressants is “start low and go slow.” This means that you start with a lower dose and slowly increase it in order to decrease side effects and increase patient compliance. The dose you start the patient on may not even be at a therapeutic level, but every month or so you can increase the dose a bit. Lexapro, has ruined my life. One 10mg pill severely damaged my brain. 8 hours after taking the pill, I entered into a world of a neurological nightmare. I am afraid this happens more often than we know. I could have very easily been placed in a psyche-ward or accidentally killed myself the first few days after taking that pill. Then, no one would be able to tell my story. What happened........ After suffering a chronic sleep disorder for decades, I couldn't get more than 4 hours of sleep a night so I went to the doctor for some medical help. September 23rd 2006: The doctor prescribed Lexapro (10mg). So I took this pill having been reassured by frequent television adverstisements that it was safe. My doctor also acted as if these pills were harmless. I took the pill and went to sleep about noon. 8 hours later I woke to the most awful, strange, powerful inner sensations. I was also filled with a feeling of dread or horror. I had never experienced anything like this before and I couldn't believe it. It felt as if my brain was dying...... I jumped out of bed and ran around my house.... I couldn't stop moving or the sensations would get worse. Sheer horror! The symptoms: Many are similar to Serotonin Syndrome. Benzodiazepine withdrawal syndrome—often abbreviated to benzo withdrawal—is the cluster of symptoms that emerge when a person who has taken benzodiazepines, either medically or recreationally, and has developed a physical dependence undergoes dosage reduction or discontinuation. Development of physical dependence and/or addiction and the resulting withdrawal symptoms, some of which may last for years, may result from either drug-seeking behaviors or from taking the medication as prescribed. Benzodiazepine withdrawal is characterized by sleep disturbance, irritability, increased tension and anxiety, panic attacks, hand tremor, sweating, difficulty with concentration, confusion and cognitive difficulty, memory problems, dry retching and nausea, weight loss, palpitations, headache, muscular pain and stiffness, a host of perceptual changes, hallucinations, seizures, psychosis, and suicide (see "Signs and Symptoms" section below for full list). Further, these symptoms are notable for the manner in which they wax and wane and vary in severity from day to day or week by week instead of steadily decreasing in a straightforward monotonic manner. It is a potentially serious condition, and is complex and often protracted in time course. Long-term use, defined as daily use for at least three months, is not desirable because of the associated increased risk of dependence, dose escalation, loss of efficacy, increased risk of accidents and falls, particularly for the elderly, as well as cognitive, neurological, and intellectual impairments. Use of short-acting hypnotics, while being effective at initiating sleep, worsen the second half of sleep due to withdrawal effects. Nevertheless, long-term users of benzodiazepines should not be forced to withdraw against their will. Benzodiazepine withdrawal can be severe and can provoke life-threatening withdrawal symptoms, such as seizures, particularly with abrupt or overly rapid dosage reduction from high doses or long time users. A severe withdrawal response can nevertheless occur despite gradual dose reduction, or from relatively low doses in short time users, even after a single large dose in animal models. A minority of individuals will experience a protracted withdrawal syndrome whose symptoms may persist at a sub-acute level for months, or years after cessation of benzodiazepines. The likelihood of developing a protracted withdrawal syndrome can be minimized by a slow, gradual reduction in dosage. Chronic exposure to benzodiazepines causes neural adaptations that counteract the drug's effects, leading to tolerance and dependence. Despite taking a constant therapeutic dose, long-term use of benzodiazepines may lead to the emergence of withdrawal-like symptoms, particularly between doses. When the drug is discontinued or the dosage reduced, withdrawal symptoms may appear and remain until the body reverses the physiological adaptations. These rebound symptoms may be identical to the symptoms for which the drug was initially taken, or may be part of discontinuation symptoms. In severe cases, the withdrawal reaction may exacerbate or resemble serious psychiatric and medical conditions, such as mania, schizophrenia, and, especially at high doses, seizure disorders. Failure to recognize discontinuation symptoms can lead to false evidence for the need to take benzodiazepines, which in turn leads to withdrawal failure and reinstatement of benzodiazepines, often to higher doses. This is when my myoclonic jerks became to much to handle. They were worse a few hours earlier resulting in convulsing on the ground like a severe seizure. I wasnt quite prepared for chorea like uncontrolled movements, extreme stuttering, no congnative thought process and nearly the complete inability to control all motor functions. It started 5 days ago after taking just one doze of fluoxetine (prozac 20mg) with the sudden onset of jerking movements then followed by intense fear resulting in contempalting suicide and losing touch with all reality. luckily I was got to in time I had hallucinations and dementia/ This was most likely caused by the Prozac itself and the rest of the time was the effects of the antipsychotic Zyprexa or olanzapine. The next days were better and I thought it would stop but when the seizure-like movements started this morning I was taken to the ER but I dont have video of that After benadryl, valum, and hour passed things werent as bad. Now I have parkinson like movement and the doctor said they should stop up in a couple of days when the Zyprexa (anti-psychotic) is out of my system. needless to say I will not be back on Symbyax. There was misleading, vague, and very little information about akathisia and I wanted people to know this could happen with any antipsychotic but mostly happens with older drugs or a-typical drugs. The doctors dont know much and I have read a lot of people with more mild but still akathisia symptoms who need information. You don't have to be on these drugs long, ithey dont have to be abused, and it could happen in a very short amount of time. People need to know.

This is a compilation of the various involuntary muscle movements in my neck, face, eyes, arms, legs, hands, feet I've been having for several months. These movements get worse when relaxed, for example at bedtime, but also appear periodically throughout the day. They also vary depending how I am sitting or laying down. MRI results were negative, neurologists and movement disorder specialist seems to think this is stress/anxiety related and was worsened by surgery I had in November 2012, where I had a temporary pin surgically inserted to repair a broken finger. Tried Clonidine and Gabapentin with no success. Took Ambien for 4 months to be able to fall and stay asleep but made me groggy and unable to think clearly. Currently only taking 15mg of Remeron at bedtime to help with sleep and anxiety. Thought it was worth posting in case anyone has experienced something similar or has any suggestions on possible diagnoses or treatments.
