Gliomas are the most frequent and the most aggressive type of brain cancer. In the clinics, in order to choose a treatment for the patient, clinicians divide these tumors in 4 grades, according to the severity of the cancer: grade I, II, III or IV. Now researchers suggest a more accurate classification, with seven subtypes of tumors. Each of them defines a specific prognosis and also relate to different responses to treatment. The new classification results from the broadest analysis of brain tumors done to date, by scientists from 6 countries. Tumors that carried a mutation in one of the IDH genes were found to lead to better prognosis in patients. The research was coordinated by Houtan Noushmehr, from University of Sao Paulo - USP (Brazil); Antonio Iavarone, from Columbia University (United States); and Roel Verhaak, from MD Anderson Cancer Center (United States). Analysis included: DNA methylation, copy number of genes, genetic mutations, RNA expression, codeletion of 1p19q chromosome, telomere length and IDH gene mutations. A tumor is said to be benign when its microscopic and gross characteristics are considered to be relatively innocent, implying that it will remain localized, it cannot spread to other sites, and is amenable to local surgical removal; the patient generally survives. It should be noted, however, that benign tumors can produce more than localized lumps, and sometimes they are responsible for serious disease, as pointed out later. Malignant tumors are collectively referred to as cancers, derived from the Latin word for crab-that is, they adhere to any part that they seize in an obstinate manner, similar to a crab's behavior. Malignant, as applied to a neoplasm, implies that the lesion can invade and destroy adjacent structures and spread to distant sites (metastasize) to cause death. Not all cancers pursue so deadly a course. Some are less aggressive and are treated successfully, but the designation malignant constitutes a red flag.

"We know so much more about brain tumors then we did 10 years ago," said John Jane Sr., MD, PhD, Editor of the American Association of Neurological Surgeons" Journal of Neurosurgery and a neurosurgeon in Virginia, "We still have a lot to learn, especially since brain tumors remain one of the least understood and difficult to treat tumors.But, there have been advances in how we diagnose brain tumors and how we treat brain tumors." What is a brain tumor? A brain tumor is a cluster of abnormal cells growing in the brain. Brain tumors can be benign (non-cancerous) or malignant (cancerous). They can start growing in the brain - a primary tumor, or they can be formed when cancer cells from another part of the body travel to the brain - a metastatic brain tumor. Approximately half of all primary brain tumors are benign. All metastatic brain tumors are malignant. More than 110,000 people are diagnosed each year with a brain tumor. Approximately 80,000 of tumors are metastatic. There are more than a dozen different types of brain tumors and they are classified by level of malignancy, size and degree to which the cancer has spread. First, metastatic brain tumors often are formed when cancers of the lung, breast, colon, prostate and skin spread to the brain. Improvements in treating these diseases have prolonged the life of victims, giving the cancer a greater opportunity to spread. We start with just a quick discussion of the difference between primary brain tumors and metastatic cancer. Mets appear as multiple lesions as well as their propensity to affect the grey/white junction. Now we can talk about primary tumors. In adults, the most common malignant tumor is glioblastoma multiforme. This is a cancer of astrocytes (astrocytoma) and is very aggressive, affecting both hemispheres and the corpus callosum. It can also metastasize. Histologically, it shows pseudo-pallisading cells, and because it's made of astrocytes, it is positive for GFAP. Next up, the most common benign and most common overall adult tumor is meningioma. This is usually a tumor of the arachnoid layer and is more common in women (as it expresses estrogen receptors). Grossly, you can have a mass attached to the dura and is resectable. Histologically, you see psammoma bodies. You can have cancers of the cells that make myelin like Schwann cells (schwannoma). This can affect your cranial nerve VIII and cause deafness, especially if it's seen in neurofibromatosis II. These cells are s100 positive. Another cell that makes myelin is oligodendrocytes. Cancers of this cell usually affect the frontal lobe and histologically you see a lot of capillaries. Last adult tumor is hemangioblastoma, which guessing from the name has a lot to do with blood vessels. You see capillaries and this tumor can release EPO. It is associated with VHL. Now to childhood tumors. The most common is pilocytic astrocytoma. You know it's your astrocytes because, again, it's GFAP positive. Another big one is a cancer of ependymal cells, called ependyoma. Recall epedymal cells make CSF and tumors here can cause hydrocephalus. These like to line blood vessels. Medulloblastoma is a cancer of primitive neuroectoderm cells. This loves to affect the cerebellum and drop mets to the spinal cord. Because they are from primitive cells, histologically they are small blue cells. It is associated with Turcot syndrome, a syndrome with colonic polyps and brain cancer. Next up is craniopharyngioma, which stems from the Rathke's pouch. This is like pituitary adenomas in adults. It will share the signs of bitemporal hemianopia, but will instead have things like calcifications and cholesterol. Lastly, we will talk about pineal gland tumors called pinealoma. The pineal gland produce melatonin and tumors here can produce sex hormones like b-hcg. As it grows it can compress the superior colliculus, which knocks out our vertical gaze. This causes Parinaud syndrome.

Understanding the stage of cancer helps doctors to: Develop a prognosis and design a treatment plan for individual patients. Learn how serious your cancer is and your chances of survival Describe where cancer is, if or where it has spread Predict the chance of cancer recurrence Doctors collect data about cancer to determine its stage. This information comes from: Physical examinations Imaging tests such as x-rays, CT scans, and MRI Laboratory tests from blood, urine and other fluids and tissues removed from the body Pathology Surgical reports The TNM system describes cancers that form solid tumors, such as breast, colon, or lung cancers. However, doctors use other staging systems to classify other types of cancer, such as brain tumors, blood cancers and childhood cancers. In the TNM system, each cancer has a letter or number to describe its spread. The letter "T" plus a number (0-4) describes the size and location of the tumor. It includes how much the tumor has grown into nearby tissues. A larger tumor that grows deeper into the surrounding tissue receives a higher number. For some types of cancer have lowercase letters, such as “a,” “b,” or "m" (for multiple), added to the “T.” The extra letters provide more detail about the tumor. TX means that you can't measure the tumor. T0 means there is no evidence of a primary tumor. Tis (T I S) means that the cancer cells are only growing in the most superficial layer of tissue. Numbers after the T (such as T1-T4) describe the tumor size or the amount of spread into nearby structures. Node (N). The letter "N" plus a number (0-3) describes whether you can find cancer in the lymph nodes. NX means that you can't evaluate the nearby lymph nodes. N0 means nearby lymph nodes do not contain cancer. Numbers after the N (such as N1-N3) describe the size, location, or the number of lymph nodes affected by cancer. The higher the N number, the greater the cancer spread to nearby lymph nodes. Metastasis (M). The letter "M" indicates whether the cancer has metastasized, or spread, to other parts of the body. M0 means that the cancer has not spread. M1 Indicates that cancer has spread. Most cancer types have their version of this classification system. So letters and numbers don’t always mean the same thing for every kind of cancer. Doctors combine the T,N,M results to determine the stages of cancer 1-4 for each person. Most cancers have four stages. Some cancers also have a stage 0 (zero). Stage 0: This stage describes cancer in situ, which means “in place.” Stage 0 cancers live in the place they started and have not spread to nearby tissues. Stage I: This is a small tumor that has not grown deep into nearby tissues. It also has not spread to the lymph nodes or other parts of the body. It is often called early-stage cancer. Stage II and III: These stages indicate larger cancers or tumors that survive deeper into nearby tissue. They may also spread to lymph nodes but not to other parts of the body. Stage IV. This stage means that the cancer has reached other organs or parts of the body. Some call it advanced or metastatic cancer. The 10 Most Common Brain Tumors 1. Gliomas Approximately 30-percent of all brain tumors are glioma tumors, which originate in the brain or spine. Gliomas also make up 80-percent of all malignant brain tumors. This tumor is named glioma because it develops in the glial cells—specifically the astrocytoma, ependymoma, oligodendroglioma cells, or a combination of these. As a glioma brain tumor grows, it may cause headaches, nausea and vomiting, seizures, and vision problems if it places pressure on the optic nerve. The treatment and removal depends on the tumors placement. For instance, depending on how close it is to the patient’s delicate brain stem, the tumor may only be partially removed to avoid brain damage. 2. Craniopharyngiomas Craniopharyngiomas most often afflict children, teens, and young adults. These tumors form near the base of the brain, typically above the pituitary gland, causing a domino effect of health issues as they amass—including eyesight issues or total loss of vision (if they push on the optic nerve), weight gain, delayed physical development, and stunted hormone levels. 3. Lymphomas Lymphoma tumors can develop in the brain in response to cancer of the lymphatic system due to Hodgkin’s disease, which occurs when white blood cells (or lymphocytes) become cancerous. These types of cerebral lymphomas can also develop in individuals with low immune systems—as a result of AIDS or following a major surgery (i.e., organ transplant). 4. Haemangioblastomas Haemangioblastomas is typically genetic, meaning it’s prone to individuals with a family history of von Hippel Lindau syndrome (vHL), a disorder characterized by the development of fluid-filled cysts (or tumors) in different areas of the body.
