Dr. Abel earned his medical degree with distinction from the University of the West Indies in Kingston, Jamaica. He was a Rhodes Scholar and clinical research fellow with Professor John G. Ledingham at the University of Oxford, England, where he also earned a Ph.D. in physiology. He completed an internship and residency in medicine at McGraw Medical Center, a part of Northwestern University Medical School, where he served as chief resident of internal medicine at the VA Lakeside Medical Center. What is DIABETIC FOOT ULCER? What does DIABETIC FOOT ULCER mean? Diabetic foot ulcer is a major complication of diabetes mellitus, and probably the major component of the diabetic foot. Wound healing is an innate mechanism of action that works reliably most of the time. A key feature of wound healing is stepwise repair of lost extracellular matrix (ECM) that forms the largest component of the dermal skin layer. But in some cases, certain disorders or physiological insult disturbs the wound healing process. Diabetes mellitus is one such metabolic disorder that impedes the normal steps of the wound healing process. Many studies show a prolonged inflammatory phase in diabetic wounds, which causes a delay in the formation of mature granulation tissue and a parallel reduction in wound tensile strength. Treatment of diabetic foot ulcers should include: blood sugar control, removing dead tissue from the wound, dressings, and removing pressure from the wound through techniques such as total contact casting. Surgery in some cases may improve outcomes. Hyperbaric oxygen therapy may also help but is expensive. It occurs in 15% of people with diabetes and precedes 84% of all diabetes-related lower-leg amputations.

Dr. Ted Byers, dentist in Flagstaff and Sedona, Arizona talks with the Dentist News Network about wisdom teeth and the procedure of wisdom teeth extraction. Wisdom teeth are the third molars that people usually get around the age of 17 or 18 and are called wisdom teeth because this is when they are maturing into adulthood, says Dr. Ted Byers of Flagstaff and Sedona, Arizona. Quite often, he says, people don t have room for them to come in and about the time Dr. Byers s patients are done with high school or are entering college, he starts the dialogue about whether or not to keep the wisdom teeth. Dr. Byers says wisdom teeth should be removed when there s not enough room in the mouth for them to totally erupt. He adds that with those wisdom teeth that do come in, about 95% come in crowded or get cavities because patients cannot clean them adequately. Instead of them having create more problems later in life, Dr. Byers recommends having the wisdom teeth extracted. He finds that with 28 teeth left after they re extracted, patients function extremely well. Impacted wisdom teeth is when they come in the wrong direction and the more challenging cases of Dr. Byers are referred to oral surgeons. Dr. Byers does enjoy doing wisdom teeth extraction because it allows the patient the benefit of good oral health in their later years. An oral surgeon would do the extraction under general anesthesia where a general dentist uses nitrous oxide, or conscious sedation, with a local anesthetic. There are less side effects than undergoing general anesthesia and the patient is still numb and doesn t feel anything during the procedure. A dry socket can occur most commonly on the lower wisdom teeth when the clot from a socket becomes absent, exposing raw bone and nerves, creating a lot of pain for the patient. To most of the patients what they ask is if general anesthesia is safe for general anesthesia patients. It depends if the patient is hypertension is under control or uncontrolled. Whether the surgery is planned or an unplanned one, like an emergency surgery. Why are we worried about people having hypertensions is basically because if it is uncontrolled hypertension, on table patient having stroke, the patient can have myocardial infarction or because of high BP, people can have massive blood loss and shock and hypotension. If a person is for a minor surgery, who is a very controlled hypertension, when they come on the operation table, there is sometimes anxiety and stress, the BP shoots up. What we try to do is we try to sedate them, give some anxiolytics , wait and watch if the blood pressure comes down and then take them for surgery. But if it is for a major surgery or planned surgery and the patient comes with an uncontrolled hypertension, we try to defer it till we get a good control.

An epidermoid cyst (Epidermal Inclusion cyst, Infundibular cyst), is a benign growth commonly found in the skin and typically appears on the face, neck or trunk, but can occur anywhere on the body. Another name used is “sebacous cyst” but this is actually an antiquated misnomer, and is not a term used by dermatologists. They are also the most common type of cutaneous cysts. Epidermoid cysts result from the reproduction of epidermal cells within a confined space of the dermis. The pasty contents are mostly composed of macerated keratin (wet skin cells), which creates this “cheesy” consistency, and there can be a pungent odor. An epidermoid cyst may have no symptoms and are typically harmless. Usually people seek removal but they don’t like the appearance of these bumps, or the cyst has ruptured or been inflamed or “infected” in the past. Rupture is associated with sudden redness, pan, swelling, and local heat, and can lead to abscess formation. Also, a history of inflammation, often increases scar tissue in the area, makes the cyst more firmly adherent to surrounding skin, and makes it more difficult to remove. Surgical excision is curative, but the complete cyst removal including the entire cyst sac and contents need to be removed to ensure that the cyst won’t reoccur. The bunion, however, is, in most cases, treatable with conservative measures that prevent these excessive ground reactive forces against the First Metatarsal. And the most common treatment for that is a functional orthotic. In some cases, the deformity becomes very bad, where surgical intervention is needed. But what does that actually mean? Your diaphragm is a muscle that separates your chest from your abdomen and helps you to breathe. Normally, the diaphragm has an opening for the esophagus to pass through where it connects with the stomach. A hiatal hernia occurs when part of the stomach pushes upward through this small opening. Medical Malpractice Your hiatal hernia may be causing considerable discomfort, with symptoms like heartburn, difficulty swallowing, chest pain and belching. The reasons why hiatal hernias form are not known, but they are quite common. A hernia is dangerous only if it becomes strangulated. That means that the portion of the stomach that is pushed up into the chest may become pinched - preventing blood from reaching it. Medical Malpractice If this happens, you may require emergency surgery to restore blood flow and to repair the hernia. Luckily, the vast majority of hernias are not considered to be emergencies.
