For treatment of type II, dietary modification is the initial approach, but many patients require treatment with statins (HMG-CoA reductase inhibitors) to reduce cardiovascular risk. If the triglyceride level is markedly raised, fibrates (peroxisome proliferator-activated receptor-alpha agonists) may be preferable due to their beneficial effects. Combination treatment of statins and fibrates, while highly effective, causes a markedly increased risk of myopathy and rhabdomyolysis, so is only done under close supervision. Other agents commonly added to statins are ezetimibe, niacin, and bile acid sequestrants. Dietary supplementation with fish oil is also used to reduce elevated triglycerides, with the greatest effect occurring in patients with the greatest severity.

It can occur unintentionally due to an underlying disease or can arise from a conscious effort to improve an actual or perceived overweight or obese state. Unintentional weight loss Unintentional weight loss occurs in many diseases and conditions, including some very serious diseases such as cancer ,. This results in a loss of both fat and lean mass, leading to a significant reduction in total body weight. In addition to weight loss due to a reduction in fat and lean mass, fluid loss can be triggered by illnesses such as diabetes, certain medications, lack of fluid intake or other factors. Causes of unintentional weight loss Starvation , a state of extreme hunger resulting from lack of essential nutrients over a prolonged period. Cancer , a very common and sometimes fatal cause of unexplained idiopathic weight loss. About one-third of unintentional weight loss cases are secondary to malignancy. Cancers to suspect in patients with unexplained weight loss include gastrointestinal, prostate, hepatobillary hepatocellular carcinoma, pancreatic cancer , ovarian, hematologic or lung malignancies. AIDS can cause weight loss and should be suspected in high-risk individuals presenting with weight loss. Gastrointestinal disorders are another common cause of unexplained weight loss - in fact they are the most common non-cancerous cause of idiopathic weight loss. Possible gastrointestinal etiologies of unexplained weight loss are celiac disease , peptic ulcer , inflammatory bowel disease crohns disease and ulcerative colitis , pancreatitis , gastritis, diarrhea and many other GI conditions can cause weight loss. Infection Some infectious diseases can cause weight loss. AIDS , and some other sub-acute or occult infections may cause weight loss. Cardiac disease Cardiovascular disease, especially congestive heart failure, may cause unexplained weight loss. In some people, certain medications may cause weight loss as a side effect. Intentional weight loss refers to the loss of total body mass in an effort to improve fitness and health, and/or to change appearance. Attention to diet in particular can be beneficial in reducing the impact of diabetes and other health risks of an overweight or obese individual. Weight loss occurs when an individual is in a state of negative thermodynamic flux: when the body is exerting more energy i e in work and metabolism than it is consuming i e from food or other nutritional supplements , it will use stored reserves from fat or muscle, gradually leading to weight loss. It is not uncommon for some people who are currently at their ideal body weight to seek additional weight loss in order to improve athletic performance, and/or meet required weight classification for participation in a sport. There are many diet plans and recipes that can be helpful for weight loss. There are many dietary programs that claim to be efficient in helping overweight individuals to lose weight with no effort. Acute esophageal necrosis "The Black Esophagus" dark-pigmented esophageal mucosa, the so-called "black esophagus" is a rare observation during upper endoscopy. Black esophagus is a rare endoscopic finding. While the etiology remains unknown, it occurs in elderly patients and/or those with comorbid conditions, particularly vascular disorders. It usually involves the distal third of the esophagus. The most common presenting symptom is acute esophageal bleeding. The natural history of the disease is that of spontaneous resolution. The differential diagnosis includes acute necrotizing esophagitis, melanosis, pseudo-melanosis, melanoma, acanthosis nigricans, and caustic substance (lye) ingestion. Some associated conditions include ischemia, prolonged hypotension, hyperglycemia, hypersensitivity to antibiotics, herpetic infection, aortic dissection, and anti-cardiolipin antibodies. The pathogenesis of black esophagus is not fully understood, but it is hypothesized that low systemic perfusion plays a dominant role. Although the mechanism of acute esophageal necrosis is unclear, most authors have suggested ischemic injury as the most likely cause. Ischemia can be caused by hypotension or thrombosis. Risk factors including old age, male gender, diabetes mellitus, and hypertension with atherosclerotic vascular disease may have predisposed the patient to esophageal ischemia. However, the patient had neither a hypotensive event nor metabolic acidosis. Anatomically, the esophagus is a well-vascularized organ. The blood supply to the proximal esophagus is provided by the bilateral superior and inferior thyroid arteries and that to the mid esophagus is provided by direct arteries from the aorta. The distal esophagus has been reported to be less vascularized and more vulnerable to ischemic injury. However, all major arteries divide into minute branches, which penetrate into the submucosa to form an extensive interconnecting vascular network in the esophagus. Thus, ischemic injury alone may not be a leading mechanism of black esophagus. Massive acid reflux can cause black esophagus. The prognosis of black esophagus is typically poor and mortality depends on the underlying illness (older age, higher incidence of malignancy, three or more associated chronic conditions). Death secondary to esophageal necrosis occurs in less than 6% of cases. Endoscopy is needed for diagnosis and is considered safe. The characteristic endoscopic appearance of black diffusely necrotic esophageal mucosa, with or without exudates is diagnostic. Classic circumferential involvement of the distal one-third of esophagus with clear demarcation at the gastroesophageal junction is relatively uncommon, rather BE tends to show diffuse involvement in more than half of cases described. Selective distal esophageal involvement can aid in differentiating AEN from corrosive injury of esophagus. Other conditions which can mimic endoscopic appearance of BE are quinine ingestion, acanthosis nigricans, melanosis and pseudomelanosis of esophagus.

Cirrhosis of the liver nursing NCLEX review over the symptoms, nursing care/management, pathophysiology, complications, and treatments. Cirrhosis is a liver disease where liver cells become extremely damaged due to long-term/severe damage. This damage can be from a viral infection (Hepatitis B or C), heavy alcohol consumption, obesity, hyperlipidemia, bile duct problem, autoimmune etc. These damaged liver cells are replaced with fibrous tissue, and this leads to scarring of the liver. The role of the liver is to filter the blood and perform the following functions to it: metabolizing, detoxifying, storing, producing etc. When liver cells are damaged these processes do not happen. Complications of cirrhosis include: portal hypertension, splenomegaly, ascites, jaundice, hepatic encephalopathy, esophageal varices etc. All of these conditions are discussed in depth in the video. During the early stages of cirrhosis the patient may be asymptomatic, however, in the late stages the patient will have signs and symptoms that include: asterixis, fetor hepaticus, jaundice, reduced platelets and white blood cells, spider angiomas, ascites, gynecomastia, increase ammonia level etc. Nursing interventions for cirrhosis include monitoring for bleeding, abnormal glucose levels, implementing a proper diet: if patient has confusion (hepatic encephalopathy) they need a low protein diet, however, if patient isn't confused they need a high lean protein diet, low sodium, fluid restriction, assessing for rupture of varices, administering lactulose etc. Treatment for cirrhosis includes a liver transplant and medications such as diuretics, blood products, vitamin K, beta blocker or nitrates. In addition to a paracentesis. which helps remove fluid from the abdomen, if ascites is present. NATURAL SAFE AND EFFECTIVE WEIGHT LOSS. Poor management of type 1 diabetes mellitus, also known as insulin-dependent diabetes mellitus (IDDM), leads to an excessive amount of glucose and an insufficient amount of insulin in the bloodstream. This triggers the release of triglycerides from adipose (fat) tissue and catabolism (breakdown) of amino acids in muscle tissue. This results in a loss of both fat and lean mass, leading to a significant reduction in total body weight. Untreated type 1 diabetes mellitus can produce WEIGHT LOSS. In addition to WEIGHT LOSS due to a reduction in fat and lean mass, fluid loss can trigger by illnesses such as diabetes, certain medications, lack of fluid intake or other factors. Fluid loss in addition to reduction in fat and lean mass exacerbates the risk for cachexia. 1 Infections such as HIV may alter METABOLISM, leading to WEIGHT LOSS. 2 Hormonal disruptions, such as an overactive thyroid (hyperthyroidism), may also exhibit as WEIGHT LOSS. 3 One cutoff value of where unintentional WEIGHT LOSS is of significant concern is where there is a WEIGHT LOSS of more than 5% in the past month, or at least 10% during the last 6 months. 4 CAUSES OF UNINTENTIONAL WEIGHT LOSS • STARVATION, a state of extreme hunger; resulting from lack of essential nutrients over a prolonged period. o CANCER, a very common and sometimes fatal cause of unexplained (idiopathic) WEIGHT LOSS. About one-third of unintentional WEIGHT LOSS cases are secondary to malignancy. Cancers to suspect in patients with unexplained WEIGHT LOSS include GASTROINTESTINAL, PROSTATE, HEPATOBILLARY (HEPATOCELLULAR CARCINOMA, PANCREATIC CANCER), OVARIAN, HEMATOLOGIC OR LUNG MALIGNANCIES. • AIDS can cause WEIGHT LOSS and CAN SUPPOSE, in high-risk individuals presenting with WEIGHT LOSS. • GASTROINTESTINAL DISORDERS are another common cause of unexplained WEIGHT LOSS -- in fact, they are the most common non-cancerous cause of idiopathic WEIGHT LOSS. Possible gastrointestinal etiologies of unexplained o WEIGHT LOSS ARE CELIAC DISEASE, PEPTIC ULCER, INFLAMMATORY BOWEL DISEASE (CROHNS DISEASE AND ULCERATIVE COLITIS), PANCREATITIS, GASTRITIS, DIARRHEA AND MANY OTHER GI CONDITIONS CAN CAUSE WEIGHT LOSS. • INFECTION. Some infectious diseases can cause WEIGHT LOSS. These include fungal illness, endocarditis, many parasitic diseases, AIDS, and some other sub-acute or occult infections may cause WEIGHT LOSS. • RENAL DISEASE. Patients who have uremia often have poor or absent appetite, emesis and nausea. This can cause WEIGHT LOSS. • CARDIAC DISEASE. Cardiovascular disease, especially congestive heart failure, may cause unexplained WEIGHT LOSS. • PULMONARY DISEASE. • CONNECTIVE TISSUE DISEASE • NEUROLOGIC DISEASE, INCLUDING DEMENTIA • IN SOME PEOPLE, CERTAIN MEDICATIONS may cause WEIGHT LOSS as a side effect. • STRESS can cause WEIGHT LOSS. However, recent research (Jastebott, Potenza et al. 2010) shows a correlation between OBESITY and high levels of stress. INTENTIONAL WEIGHT LOSS INTENTIONAL WEIGHT LOSS REFERS TO THE LOSS OF TOTAL BODY MASS IN AN EFFORT TO IMPROVE FITNESS AND HEALTH, AND/OR TO CHANGE APPEARANCE. THERAPEUTIC WEIGHT LOSS, IN INDIVIDUALS WHO ARE OVERWEIGHT OR OBESE, CAN DECREASE THE LIKELIHOOD OF DEVELOPING DISEASES SUCH AS TYPE 2 DIABETES, HEART DISEASE, HIGH BLOOD PRESSURE. OSTEOARTHRITIS AND; CERTAIN OTHER TYPES OF CANCER. While being OVERWEIGHT had been thought 8 to be linked to stroke there is no strong evidence to support that link. Attention to diet in particular can be beneficial in reducing the impact of diabetes and other health risks of an OVERWEIGHT or OBESE individual. Hypoparathyroidism NCLEX nursing review of hypoparathyroidism pathophysiology, symptoms, causes, and nursing interventions. Hypoparathyroid is the low production of parathyroid hormone by the parathyroid gland. This parathyroid disorder leads to hypocalcemia and hyperphosphatemia. Signs and symptoms of hypoparathyroidism include paresthesia, positive Trousseau and Chvostek's sign, tetany, and lab values that reflect hypocalcemia and hyperphosphatemia. Causes of this parathyroidism disorder include thyroidectomy, low magnesium level (hypomagnesemia), autoimmune, or the body is resistant to PTH (parathyroid hormone). Nursing interventions include monitoring airway, lab levels, vital signs, initiating seizure precautions, ensuring high calcium and low phosphate diet, and administering medications as prescribed by the doctor. The goals of medication treatment in hypoparathyroidism are to increase the calcium level and decrease phosphate levels. There is preliminary evidence in vitro that its extracts induce apoptosis and modify gene expression in osteoclasts involved in rheumatoid arthritis and osteoporosis. It may prove to have potential activity against some cancers.[4] One recent animal study found treatment with E. officinalis reduced severity of acute pancreatitis (induced by L-arginine in rats). It also promoted the spontaneous repair and regeneration process of the pancreas occurring after an acute attack. Experimental preparations of leaves, bark or fruit have shown potential efficacy against laboratory models of disease, such as for inflammation, cancer, age-related renal disease, and diabetes. A human pilot study demonstrated a reduction of blood cholesterol levels in both normal and hypercholesterolemic men with treatment. Another recent study with alloxan-induced diabetic rats given an aqueous amla fruit extract has shown significant decrease of the blood glucose, as well as triglyceridemic levels and an improvement of the liver function caused by a normalization of the liver-specific enzyme alanine transaminase activity.[10] Amla Medicinal use: In traditional Indian medicine, dried and fresh fruits of the plant are used. All parts of the plant are used in various Ayurvedic/Unani medicine (Jawarish amla) herbal preparations, including the fruit, seed, leaves, root, bark and flowers. According to Ayurveda, aamla fruit is sour (amla) and astringent (kashaya) in taste (rasa), with sweet (madhura), bitter (tikta) and pungent (katu) secondary tastes (anurasas). Its qualities (gunas) are light (laghu) and dry (ruksha), the postdigestive effect (vipaka) is sweet (madhura), and its energy (virya) is cooling (shita). According to Ayurveda, aamla balances all three doshas. While aamla is unusual in that it contains five out of the six tastes recognized by Ayurved, it is most important to recognize the effects of the "virya", or potency, and "vipaka", or post-digestive effect. Considered in this light, aamla is particularly helpful in reducing pitta due to its cooling energy and balances both Pitta and vata by virtue of its sweet taste. The kapha is balanced primarily due to its drying action. It may be used as a rasayana (rejuvenative) to promote longevity, and traditionally to enhance digestion (dipanapachana), treat constipation (anuloma), reduce fever (jvaraghna), purify the blood (raktaprasadana), reduce cough (kasahara), alleviate asthma (svasahara), strengthen the heart (hrdaya), benefit the eyes (chakshushya), stimulate hair growth (romasanjana), enliven the body (jivaniya), and enhance intellect (medhya). In Ayurvedic polyherbal formulations, Indian gooseberry is a common constituent, and most notably is the primary ingredient in an ancient herbal rasayana called Chyawanprash.
