Nurses warn of mental health services strain

Staff cuts and bed shortages are leaving mental health services "under unprecedented strain", says the nurses' union. The Royal College of Nursing says there are now 3,300 fewer posts in mental health nursing, and 1,500 fewer beds, than in 2010. At the same time, demand has increased by 30%, the RCN said. A mental health charity said this was damaging the care patients received, leaving them needing long-term support. According to the RCN's figures, mental health nursing posts declined by 8% in the past four years in England. In Scotland, Wales and Northern Ireland, posts were cut by 1%. However, the total number of nursing posts has increased since 2010 in all countries of the UK. Mental health nurses are specifically trained to work in mental healthcare settings and treat and care for people with a wide range of illnesses and complex needs - from dementia to schizophrenia. Mental health is widely recognised as a specialist area of nursing. 'Money is tight' Dr Peter Carter, chief executive and general secretary of the Royal College of Nursing, said cuts to mental health services could have a real and lasting impact. "We are running the serious risk of turning back the clock and undoing all the good work that has gone before. "Money is tight in the NHS but if we are serious about treating mental illness in the same way as physical illness, then people must be offered what they need before reaching crisis point. "If staffing levels and services are cut back further, then services will continue to crumble, which would be a tragedy for us all, to say nothing of all the thousands of private tragedies that could result." Mark Winstanley, chief executive of mental health charity Rethink Mental Illness, said: "Our nursing workforce is increasingly ill-equipped to give people with mental illness the specialist, recovery-driven care they need. "Nurses are being forced to take a risk-averse approach to care which prioritises keeping people safe, rather than helping them get better. "Not only is this detrimental to the quality of care that people receive, it makes little financial sense." He added that reducing nursing staff levels and skillsets would end up with more people needing long-term support, including expensive hospital treatment. The RCN wants the government and healthcare providers to ensure that they have enough staff with the right level of skill to deliver the care needed. With almost one-third of mental health nurses aged over 50, the RCN says a long-term strategy is needed to recruit and train more of them to deliver care in the community and inpatient units.

We apologize for any inconvenience this may cause. Welcome to the Society for Pediatric Sedation's online provider course on pre-sedation assessment. Our goal for this lecture is to review the different levels of sedation, to talk through how to perform a pre-sedation risk assessment, the equipment needed to conduct procedural sedation in a safe manner, and the skill set needed by sedation team members. We will finish by reviewing a systematic approach to providing safe and effective pediatric procedural sedation. First, we will review some general considerations regarding the scope of sedation, including defining different levels of sedation, which are important for planning your medication regimen, monitoring, et cetera. We will discuss patient factors that are important in pre-sedation planning, including health history, key points on the physical examination of the patient, NPO status and guidelines, and the American Society of Anesthesiologists physical status scoring system. In addition, we will discuss important provider and procedure factors, including team member skills, necessary monitors and other equipment, and a brief word on documentation. We will look at relevant environmental concerns, including available rescue resources. And finally, we will go through a brief overview of matching the sedation medication regimen to these factors, and will continue to expand on that base in subsequent lectures. The Boy Scout motto is particularly applicable to this phase of providing sedation. Be prepared. The pre-sedation phase is the assessment, preparation, and planning phase. This phase allows for sedation risk assessment, preparation for the sedation, and planning for the sedation, including taking the time to review the goals for the sedation and for the procedure. Safety is our top priority. Clearly, we want to get the test or procedure completed, but we also want to keep the child as comfortable as is safely possible. So we use the pre-sedation assessment phase to plan for accomplishing these goals. The best way for us to accomplish our goals is by carefully considering the various factors that contribute to a successful sedation. Foundational to this process are those factors associated with the program itself, namely, the institutional setting or environment, the sedation team itself, and the organizational structure of the program, including its policies and procedures. These form the pool from which we plan and draw resources. Next, we must consider the factors specific to the situation. The patient (having reviewed underlying history and risk factors), the procedure and its associated need (such as immobility and/or pain control), and the pharmacology-- what drugs will be optimal for this case. When these factors come together, quality sedation care can occur. Let's start by discussing some initial general considerations. For each sedation encounter, we should consider the age of the child as well as their underlying developmental level and personality in our planning. For example, the same procedure-- let's say a PICC line placement-- may require three very different sedation regimens for three different eight-year-old girls.

Standard & Isolation Precautions Nursing | Infection Control Contact, Droplet, Airborne PPE NCLEX

Nursing standard precautions and isolation precautions (nursing) review for the NCLEX exam. This review will cover standard precautions for infection control and transmission-based precautions (also called isolation precautions) such as contact, droplet, and airborne precautions. In addition, I will give you 3 isolation precaution mnemonics on how to remember droplet, contact, and airborne precautions. Standard precautions are precautions we follow with every patient at all times because we do not know if the person has an infectious disease or not. This is to protect ourselves and other patients. Standard precautions include: hand hygiene, wearing appropriate PPE (personal protective equipment), injection safety, respiratory/cough etiquette, patient room placement, handling of patient laundry and equipment. Isolation precautions are precautions we follow when we know or suspect a patient has an infectious disease. Depending on the type of infectious disease the patient has we will place the patient in the appropriate precautions. For example, if a patient has Pertussis (whooping cough) we will place the patient in droplet precautions. As the nurse, we would continue to follow standard precautions along with always wearing a surgical mask during patient contact. For a patient in airborne precautions, we would wear an N95 mask along with following standard precautions and place the patient in a special AIIR room (which is a negative pressure room). For a patient in contact precautions, we would always wear gloves and a gown during patient contact and follow standard precautions. The anxiety seems to start at the beginning of orientation (because you are new) then decrease during orientation and peaks when orientation is over (when you're on your own as a nurse). Learn about: administering eye drops nursing demonstration to a patient along with how to perform punctal occlusion in patients who take glaucoma medications. As a nurse, you will be required to instill eye drops correctly into a patient's eye or eyes. When giving eye drops to a patient it is important to instill the eye drop medication into the conjunctival sac (not the eyeball/cornea). In addition, the nurse will want to make sure the patient is positioned correctly with the head tilted back and the patient is looking up at the ceiling. In patients with glaucoma, it is may be necessary to perform punctal occlusion with the index finger after administering the eye drops. This is performed to decrease the amount of medication absorbed through the nasolacrimnal duct. This will prevent the medication be absorbed by the systemic circulation. Such settings must be a licensed or approved home or facility that can take in 7-12 children, as seen in group homes, or 12 or more children, as seen in institutions. Fourteen is the average age of children who enter into congregate care. In the United States out of the 400,000 children in the foster care system around 55,000 live in congregate care settings. In the United States in 2013 there was 55,916 children in congregate care. In the last ten years, there has been a 37% decrease in the amount of children in congregate care, which is greater than the overall 21% drop in number of children in the foster care system. Among the options of placements for youth in the foster care system, congregate care settings are supposed to be used as a temporary placement, until youth are considered stabilized and ready for a family-like setting. Reports show that on average, youth spend anywhere around 8 months in congregate care ). According to Title IV-E of the Social Security Act, children are required to have a case plan that aims to place them in the most appropriate and least restrictive settings that supports their wellbeing and needs. A 2015 report by the Department of Health and Human Services revealed that states are not consistently lessening the use of congregate care and that more information is required. Some children are placed in congregate care because they are thought to be in need of behavioral or mental health support services, or because they have a clinical disability. In 2013 out of all children in congregate in the United States, 36% had a mental health disorder, 45% had behavioral issues, 10% had a disability, and 28% did not have any clinical labels. Many children are placed in these settings because there is a lack of space available in family settings. According to officials, depending on the state there are anywhere between 5 and 32 percent of children who are placed in such settings that are not in need of such high surveillance.

Recognizing Child Development Problems / Educational Video. Public domain video courtesy of CDC. It's time to change how we view a child's growth. Do you know all the ways you should measure your childs growth? We naturally think of height and weight, but from birth to 5 years, your child should reach milestones in how he plays, learns, speaks and acts. A delay in any of these areas could be a sign of a developmental problem, even autism. The good news is, the earlier its recognized the more you can do to help your child reach her full potential. Early recognition of developmental disabilities such as autism is key for parents and providers. CDC realized the impact on families and invested in a campaign to help parents measure their children's progress by monitoring how they play, learn, speak and act. What is child development? A child's growth is more than just physical. Children grow, develop, and learn throughout their lives, starting at birth. A child's development can be followed by how they play, learn, speak, and behave. What is a developmental delay? Will my child just grow out of it? Skills such as taking a first step, smiling for the first time, and waving "bye bye" are called developmental milestones. Children reach milestones in playing, learning, speaking, behaving, and moving (crawling, walking, etc.). A developmental delay is when your child does not reach these milestones at the same time as other children the same age. If your child is not developing properly, there are things you can do that may help. Most of the time, a developmental problem is not something your child will "grow out of" on his or her own. But with help, your child could reach his or her full potential! What is developmental screening? Doctors and nurses use developmental screening to tell if children are learning basic skills when they should, or if they might have problems. Your child's doctor may ask you questions or talk and play with your child during an exam to see how he or she learns, speaks, behaves, and moves. Since there is no lab or blood test to tell if your child may have a delay, the developmental screening will help tell if your child needs to see a specialist. Why is developmental screening important? When a developmental delay is not recognized early, children must wait to get the help they need. This can make it hard for them to learn when they start school. In the United States, 17 percent of children have a developmental or behavioral disability such as autism, intellectual disability (also known as mental retardation), or Attention-Deficit/Hyperactivity Disorder (ADHD). In addition, many children have delays in language or other areas. In addition, many children have delays in language or other areas. But, less than half of children with problems are identified before starting school. During this time, the child could have received help for these problems and may even have entered school more ready to learn. Talk to your child's doctor or nurse if you have concerns about how your child is developing. If you or your doctor think there could be a problem, you can take your child to see a developmental pediatrician or other specialist, and you can contact your local early intervention agency (for children under 3) or public school (for children 3 and older) for help. To find out who to speak to in your area, you can contact the National Dissemination Center for Children with Disabilities by logging on to http://www.nichcy.org/ or calling 1-800-695-0285. In addition, the Centers for Disease Control and Prevention (CDC) has links to information for families at www.cdc.gov/ncbddd/resources/familyresources.htm. If there is a problem, it is very important to get your child help as soon as possible.

Dementia represents an increasing challenge for health care in Australia and an expected deficit in the dementia care workforce necessitates the need for training and more skilled management. There are now nearly 300,000 people in Australia living with dementia. By the year 2050 there will be almost one million. Current trends indicate about 1600 new cases of dementia each week. One in four people over the age of 85 now have dementia. Dementia is a broad term used to describe a group of conditions or symptoms that cause a progressive decline in a person's functioning, including memory loss and cognitive impairment. It is usually accompanied by changes in normal emotional reactions, as well as psychological and behavioural changes, such as depression, wandering, anxiety, hallucinations, aggression and disinhibition. This progressive decline presents particular problems for family and carers as well as primary health care professionals and the staff of aged care or specialist residential units. Managing dementia in any setting can be challenging and frustrating and usually requires well designed decision pathways and management strategies. In non-specialist settings delirium can often be confused with dementia and pharmacological treatment can present problems. This program takes a practical skills based approach to the challenge of dementia behaviour in various settings. It examines best practice strategies and principles for a range of behavioural and psychological dementia symptoms. It examines the issues for people in rural and remote Australia. Odd Fellow - Rebekah Children's Services

The story began in 1897 with five orphaned kids. Over 115 years later, the Odd Fellow and Rebekah Children's Home, now known as Rebekah Children's Services, located in Gilroy, California, have grown to serve and help over 3,000 children annually. Founded as the Odd-Fellow Rebekah Children's Home of Northern California in 1897, the organization operated as an orphanage until the 1950's; providing a home for orphaned children. At one time Odd Fellow and Rebekah homes existed in every State. But because of the adoption of the system of foster homes, many of the orphanages owned by the Odd Fellows and Rebekahs in other States were closed while others were converted into retirement homes and skilled nursing facilities. Today, the Rebekah Children's Services is the last operating Odd Fellow-Rebekah Children's Home in the nation which primarily serves the needs of orphaned and abused children. In the 1950's services transitioned to meet the needs of the community and we began accepting children referred by the courts for mental health treatment. Today we offer an array of programs to meet the needs of children and families in our communities. Adoption & Foster Care The steps of adoption and fostering can be difficult and intimidating. Our Family Linkage Program provides help along the way to support families through the entire process. Wraparound Focusing on the needs of the child and family, our Wraparound services provide a family-centered, strength based, needs driven planning process designed to stabilize the family unit. Outpatient Mental Health Outpatient Mental Health Services serve children and youth, ages 0-21 and address mental health symptoms and associated functional impairments. Residential Treatment Group Home Our RCL-14 Residential Treatment Services provide a time limited, interdisciplinary, psycho-educational, and therapeutic 24-hour-a-day structured program on our Gilroy campus. Hospital Diversion Offering an alternative to inpatient hospitalization, the Hospital Diversion Program at Rebekah Children's Services is an unlocked, community-based, therapeutic environment. Day Treatment Our Intensive Day Treatment Services provide structured, daily activities for youth or children with mental disabilities, emotional disorders, and behavioral disorders. Therapeutic Behavioral Services The aim of TBS is to allow children to live safely without intensive care while maintaining their current home placement or transition to a less restrictive environment. Non-Public School Our on grounds non-public school serves students with a designated non-public school Individualized Education Plan (IEP). The small class size and high staffing ratio provide individualization of the learning experience for each child. Prevention & Education Prevention and Education Services are prevention oriented and are provided in the community to help families and individuals of all ages cope with the stresses of daily living. Services emphasize personal growth, development, and situational change. Vocational Services RCS will support youth and young adults in transition with Job Seeking Skills classes, vocational training and community involvement with On-the-Job Training and Internships. The Odd Fellows is a worldwide fraternity which evolved from the tradition of craft guilds in England. Lodges of Odd Fellows have been in existence at least as early as the 18th Century. Odd Fellows came to the North America in Baltimore, Maryland, on April 26, 1819 when Thomas Wildey and four members of the Order from England instituted Washington Lodge No. 1. Workshops are held on the 4th Friday of each month from 9:00 am -11:00 am on the campus of Williamsburg Place. Presentation Objectives As a result of attending this session, participants will be able to: Understand the tripartite model of ageism, including: Cognitive dimension (stereotyping) Affective dimension (attitudes and prejudices) Behavioral dimension (discrimination) Define ageism in its various forms, including: Explicit and implicit bias Internalized and externalized ageism Positive and negative bias Recognize our ethical obligation to prevent the harms of ageism in healthcare practice, including: Overtreatment of older adults Therapeutic nihilism toward older adults Infantilization of older adults Identify ways to practice healthcare with the intention of providing age neutral-services. ABOUT THE SPEAKER Jenny Inker teaches and conducts research in the Virginia Commonwealth University School of Allied Health Professions Department of Gerontology. Her research interests include elderhood and disrupting ageism in healthcare and long-term care. A gerontologist, licensed assisted living facility administrator, and experienced leader in senior living and affordable housing environments, Jenny holds an MS in strategic management and housing from the University of Wales (Cardiff), an MBA from George Washington University, and a masters in gerontology from Virginia Commonwealth University. She expects to receive her PhD in gerontology from the VCU School of Allied Health in 2018. Tina Charlton Children's Community Nursing Team & Diabetes Service Peace Prospect West Hertfordshire Children's Community Nursing Team About the Service The Children’s Community Nurses are a team of nurses who are trained to care for sick children. We do most of our work in the child’s home. We also work in community settings such as clinics, schools and nurseries. The service is based in Apsley and the Nurses work across all of West Hertfordshire. Clinics are held in St Albans, Hemel Hempstead and Watford. We work closely with the Paediatric Team at Watford General Hospital as well as Specialist Hospitals such Great Ormond Street. The service is provided for all children age 0 to 18 or 0 to 19 (if still at Learning Disability School). The service operates seven days a week and extends to 24 hour care when this is required to support a child at the end of life. Our aim is to reduce the impact of childhood illness and reduce the need for hospital admissions. When hospital admission is needed, we work to reduce the length of stay whenever possible. The Nurses will support children with long term conditions such as childhood cancer. They will also see children who require short term care for example following surgery. The Nurses are skilled in managing a wide range of complex conditions, which allows the child to spend as much time as possible at home with their family. People with mental illness are drastically overrepresented in U.S. prisons and jails. One study found that more than half of all prisoners have a mental health problem. Once behind bars, people with mental illness rarely receive adequate care. Even worse, many are targeted for sexual abuse — a devastating crime that can be especially traumatic for someone who has a mental illness. Delivering rape crisis services in prisons and jails poses many obstacles. When the survivor has a mental illness, the challenges for advocates are even greater. During this 90-minute webinar, we’ll identify and address some of these challenges, and help you develop the skills and resources to provide timely crisis services to these incarcerated survivors. This webinar will also: - Provide advocates with a basic overview of mental illness and sexual abuse in detention - Increase advocates’ knowledge of incarcerated survivors with mental health issues and the role of mental health providers - Assist advocates in developing their advocacy skills to support incarcerated survivors with mental illness The webinar will be interactive and participants will have an opportunity to ask questions at the end. The webinar is geared toward rape crisis advocates and other community-based service providers, but anyone who works with current or former detainees is encouraged to attend. This series of webinars is being supported by the Department of Justice’s Office on Violence Against Women.
