This process usually takes place efficiently and without causing any harm. When new medications are developed, they are extensively tested to make sure they are safe before being approved for general use. Acetaminophen, when used as directed, is extremely safe even for people with liver disease. However, taking too much acetaminophen at once, or taking a high dose of acetaminophen continuously over several days can cause damage to the liver. How it Damages the Liver: A dose of Tylenol is absorbed by the gastrointestinal system and released into the bloodstream, where it alleviates pain and symptoms of fever. The liver filters the blood, which is responsible for metabolizing Tylenol into compounds that can be excreted by the body. About 90% is metabolized into harmless substances and excreted in the urine by the kidneys. Another 2% is removed without being metabolized. Between 5-10% is metabolized into a toxic compound called N-acetyl-p-benzoquinoeimine (NAPQI). Normally, the liver can safely remove NAPQI by combining it with gluthathione and converting it into a water-soluble product that is excreted in bile. However, after an overdose, the liver does not have enough gluthathione to combine with the NAPQI. Excess amounts of the toxic chemical cause rapid cell death, necrosis, tissue damage, and potentially deadly acute liver failure. (1,2,3) 2) Alcohol abuse: The liver can only handle a certain amount of alcohol at any given time, so if you drink more than the liver can deal with by drinking too quickly, or drinking too much, your liver cells struggle to process it. When alcohol reaches the liver, it produces a toxic enzyme called acetaldehyde, which can damage liver cells and cause permanent scarring. 3) Smoking: Though cigarette smoke does not directly come into contact with the liver, it does indirectly affect the liver. The chemicals in cigarette smoke eventually make their way to the liver. These chemicals cause oxidative stress on the liver, which leads to damage to the liver cells and fibrosis. Oxidative stress is involved in the aging of all the organs of the body. Oxidation produces free radicals that damage the cells of the body. 4) Lack of Sleep: If you want to keep your body — and particularly your liver ­– in good condition, it is important that you get enough sleep. During sleep, the body repairs and renews its cells, so lack of sleep will interfere with that. metabolism is the protective biochemical process by which our bodies alter xenobiotics either enzymatically or nonenzymatically. generally, drug metabolism begins with hydrophobic drug and converts it to a more hydrophilic metabolite to facilitate its elimination. an understanding of the drug metabolism process and the potential outcomes is critical for developing safe and useful pharmaceuticals. drug metabolism can result in one of two products, an inactive metabolite and an active metabolite. inactive metabolites of the drugs basically have no pharmacological activity of the original drug. an example of that would be the hydrolysis of procaine into para aminobenzoic acid and diethylethanolamine which results in the loss of the anesthetic activity of procaine. on the other hand, an active metabolite can mean that a metabolite can retain the same activity of the parent drug, that's apparent when codeine is demthylated to a more active drug which is morphine. however, in some cases we notice a result known as bioactivation where the parent drug is inactive and the metabolite would have a pharmacological activity, in this case the inactive parent drug is called a prodrug. an example of a prodrug is enalapril which has no activity as an antihypertensive, but upon hydrolysis it becomes enalaprilat which is a potent antihypertensive drug. bioactivation of a drug can also lead to toxic metabolites, the widely used acetaminophen has a metabolite that is called n-acetyl-p-benzoquinone imine which is hepatoxic, I explained the mechanism of the toxicity and the antidote in my previous video which I linked in the description below. The liver has the highest concentration of drug-metabolizing enzymes, because of it’s location between the gastrointestinal tract and the systemic circulation. Based on the reactions involved in the metabolism process we can classify the metabolic pathways into phase I metabolism and phase II. Phase I metabolism is characterized as a functionalization reaction, where they add or reveal a functional group by oxidation, reduction, or hydrolysis, hence, leading to increase in overall polarity of the drug which facilitates its excretion in the urine Oxidation is the most common phase I reaction, Cytochrome p450 is a superfamily of oxidases that are responsible for the majority of oxidation reactions, it’s found in very high concentrations in the liver. Oxidation can also happen through alcohol dehydrogenase which is an enzyme that oxidizes alcohols into aldehydes from primary alcohols and to ketones from secondary. Aldehydes can be oxidized from to carboxylic acid by the enzyme aldehyde dehydrogenase. We can see that in the example of the conversion of acetaldehyde to acetic acid in the metabolism of ethanol. Another phase I reaction is the reduction reaction, there are several reductase enzymes common reduction reactions include the reduction of disulfide bonds, in which the disulfides would be reduced to free sulfhydryls. Another reduction reaction is done by the aldo-keto reductases which reduce carbonyl containing compounds back to alcohol in a process opposite to the oxidation done by alcohol dehydrogenase. The last type of phase I metabolism reaction is hydrolysis, hydrolysis is basically the addition of water across a bond resulting in a more water-soluble metabolite. A great example of hydrolysis is ester hydrolysis which is performed by the enzyme esterase found throughout the body, esterase is responsible for the hydrolysis of an ester into a more soluble alcohol or carboxylic acid. Phase II Phase II reactions are commonly called conjugation reaction owing to the fact they add a functional group on the drug for the purpose of increasing its polarity. The conjugation process requires an enzyme generally termed as transferase, that transfers the large polar molecule called a co-factor onto the drug, Examples of phase II reactions: glucuronidation is the most common phase II reaction, glucuronosyltransferase is the enzyme that uses UDP-GA as the cofactor to transfer glucuronic acid to several functional groups like hydroxyl groups, carboxylic acid ,and hydroxylamines. The glucuronic acid adds a significant amount of hydrophilicity to the molecules facilitating it’s excretion process. Another popular reaction is glutathione conjugation which results from the addition of glutathione molecule to an electrophilic substrate. Being a nucleophile glutathione generally acts to detoxify electrophiles. Glutathione-s-transferase is the enzyme responsible for the reaction of glutathione with electrophiles like epoxides and halides. Pay attention, because we used to be able to call the pharmacy and say, "Hey, I'm gonna need my patient to come by and pick up a pain pill prescription." We can't do that anymore. They have changed the laws so we can no longer call in pain medications. You have to physically come from our office with a unique piece of paper that allows for that. So what do you do if it's over the weekend at night? What if it's not bad enough to need pain pills but you need some help, and Advil's just not cutting it? Well, here's some easy over-the-counter pain control instructions that you can do at home, that you can get from any pharmacy or grocery store out there. So the first thing is, if you have kidney problems or if you have liver problems, please take note that these instructions do not apply to you because Ibuprofen, which is also known as Advil or Motrin, that is a kidney-metabolized drug. Tylenol is a liver metabolized-drug. So if you have problems with either of those organs, don't do this. Also, if you're taking Tylenol, you cannot take alcohol or drink alcohol at the same time. It's really, really dangerous. The leading cause of acute liver failure is somebody taking Tylenol after a hangover or while they're drinking. So be careful with that, okay? Read the bottle and take it as directed on the bottle. So do whatever it says on the bottle and stagger them. So you're taking your Ibuprofen. Then two to three hours later, you take that dose of Tylenol. Two to three hours later, you're due for your ibuprofen. Two to three hours later, you're due for your Tylenol. They have a synergistic effect. That means when they're both in your system at the same time, they're much more powerful than just the two of them combined. It's actually about as pain-killing as effective as codeine, which people don't really perceive because you're not having that painkiller feeling. You're not getting any drowsiness or buzz or anything like that. But it's super, super effective. If you're hurting, make sure to stagger them, make sure to read the label so you're taking them safely, and you can get a lot of relief from this.

After reading the stories they would rate them from 1 (no pain at all) to 5 (worst possible pain). They also rated how much the protagonists in the stories felt hurt, wounded and pained. The students who took acetaminophen rated the pain to be less severe compared to those who only received the placebo without the drug. The second experiment involved 114 college students. Once again, one group took a placebo and the other group took the drug. The students received four two-second blasts of white noise ranging from 75 to 105 decibels. After receiving the blasts of noise they would rate them on a scale of 1 (not unpleasant at all) to 10 (extremely unpleasant). They also were asked to imagine how much pain it would cause someone else put in the same situation. Once again the results showed that the placebo drug free group thought they were unpleasant. Whereas the acetaminophen group had less pain and also reduced their empathy for anyone else experiencing the unpleasant noise. An overdose of tylenol can cause death. It only seems plausible that these drugs could cause permanent damage making you permanently less empathetic. There is also a study linking autism to acetaminophen which I will talk about in another video. So basically those taking drugs with acetaminophen don’t care about others. Before reaching for a bottle of tylenol or any of the other acetaminophen containing drugs, remember it can make you less caring towards others.

Acetaminophen Doesn't Work for Osteoarthritis Pain

A widely used useless treatment for osteoarthritis. Are you taking it? Acetaminophen may have little effect on osteoarthritic pain Melissa Healey writing for the Los Angeles times reported that investigators analyzed data from 74 trials of pain medications. The data, published in the Lancet, indicated that “in older patients with osteoarthritis, acetaminophen provides no more pain relief and improvement in day-to-day function than does a placebo.” The researchers also found that several non-steroidal anti-inflammatory drugs (NSAIDs) had high probabilities of improving arthritis pain of the hips and knees. Comment: No surprise here.
