Original release date: July 20, 2015 Show Notes Stomach Acid Basics Stomach acid causes heartburn. Long-term reflux problems leads to a GERD diagnosis. Stomach acid has a pH of 2. Low pH = acid; High pH = base Stomach acid is Hydrochloric acid (HCl) Some Biochemistry The molecules of the acid like to spend their time joining together and breaking apart. So by attaching to something else instead of each other is how it can be dangerous but also how it helps digest food quickly. Your stomach is designed to hold this strong acid safely. There are pumps in the cells of the lining of your stomach that produce the acid. Proton pumps work kind of like a water wheel - they move protons from inside the cell to outside to the stomach cavity. Hydrogen atoms are made up of 1 proton (positive charge) and 1 electron (negative charge). So if you take the electron away from hydrogen, you are left with a proton with a positive charge. Another process that helps create acid in the stomach relies on histamines. They are not quite the same as the histamines that you hear about in relation to allergies. But there is a particular type of histamine that is only in your stomach. Acid-reducing Medications There is a class of medications for reflux called Proton Pump Inhibitors (PPI). These medications turn off the pumps so they pump less protons into the stomach. The medications for the histamine process are called Histamine 2 Receptor Blocker (H2RB). H2RB’s work faster than PPI’s. Just like you can take an antihistamine and it block histamines causing allergies in just a dose or two, H2RB’s can work as fast as one or two doses. That is why they are advertised to treat heartburn after you eat or to help prevent heartburn before you eat. PPI’s take up to 2 weeks to reach maximum effect. The third option for heartburn are your antacids. They are bases that go into your stomach acid and help neutralize it. OTC Medications (by class) Antacids: Tums, Rolaids, Maalox, Mylanta - fastest H2RB’s: Pepcid, Zantac, Tagamet - all have generics, all OTC Tagamet can have drug-drug interactions with other prescription medications, so caution is advised. PPI’s: Prilosec, Prevacid, Nexium - some generic, newly OTC - slowest Cautions The downside to having reflux medications available OTC and people having the opportunity to self-treat is if there are any cellular changes in your esophagus. Your esophagus is not designed to be in contact with that level of acid. As those cells are injured, they eventually change and can become cancer. Fun Tidbit Just like the cells of your skin are epithelial cells and their job is to keep the inside things in and the outside things out. Your digestive tract is also lined with epithelial cells. So technically the food you eat doesn’t go inside your body, it just moves through this tract that is “outside turned in”. so, now, in 2014, a decade after the documentary which this is excerpted from was released, and following an acute gallbladder attack, and emergency gallbladder surgery in July of 2013, I discover that the pain which accompanied the attack was not only the same as the pain which previously resulted as a reaction to naproxen, (which in terms of feeling was the same as the pain which aspirin had previously caused, but could be managed by drinking milk before hand as a buffer, only magnified 10 fold, with milk being of no help), but was now being induced by morphine in dosage amounts as low as 7.5 mg if taken orally, I figure there is something more that has been overlooked. As I have often had to remind people, I am not a doctor. I only have learned so much as to be able to learn as much as my experience with aspirin and naproxen matching exactly with descriptions of ulcer reactions which some people experience as a result of taking non-steroidal anti-inflammatory drugs (nsaids). But now, the exact same reaction occurs in response to what is really very small dosage amounts of orally administered morphine, and I can not find matching accounts as easily as with naproxen and aspirin. the basic aspirin warnings include - Stop using this medication and call your doctor at once if you have any of these serious side effects: severe .... stomach pain; fever lasting longer than 3 days; swelling, or pain lasting longer than 10 days; I never did with aspirin. Stomach pain - yes. But severe? No. Naproxen - Severe - Extreme YES. But fever? No. Whatever accompanied or triggered the Gallbladder attack - Severe pain - Extreme YES. Fever lasting longer than 3 days - didn't get a chance to find out, since after three days I went to a G.P. who wrote an RX for me to go to an ED. So, I figure I'd better go to a gastroenterologist now. He wrote an Rx to try for a week and then report back. The scrawl was not very legible, but I figured that I would be able to read what it was clearly once printed on the bottle label (and from there do my precautionary homework - mechanisms of action, class of medication, warnings, iinteractions, side effects, etc.). But, fortunately my pharmacist came up to me before hand and showed me what would be charged and what it was for, before filling the Rx and charging it. $248.00 For what? A Prescription Brand . What? My insurance won't cover a medication prescribed by a specialist? Isn't there a generic? What is it for anyways? NEXIUM. Uh huh ... Would you like for us to call the doctor to see if he'll prescribe the generic (which my insurance will cover, with the usual $10 co-pay)? Why YES. (I have a really good pharmacist). And just out of curiosity, I asked, do you carry OTC Prilosec by any chance?

They do not work against infections that are caused by viruses - for example, the common cold or flu. Antibiotics are normally only prescribed for more serious bacterial infections, as many infections get better on their own. Correct use of antibiotics is absolutely essential to help reduce antibiotic resistance. Germs become resistant to antibiotics over time, which then makes them less effective. The World Health Organization (WHO) says "the world urgently needs to change the way it prescribes and uses antibiotics" as antibiotic resistance is a major global threat. What are antibiotics? Antibiotics are a group of medicines that are used to treat infections. Antibiotics are sometimes called antibacterials or antimicrobials. Antibiotics can be taken by mouth as liquids, tablets, or capsules, or they can be given by injection. Usually, people who need to have an antibiotic by injection are in hospital because they have a severe infection. Antibiotics are also available as creams, ointments, or lotions to apply to the skin to treat certain skin infections. It is important to remember that antibiotics only work against infections that are caused by bacteria and certain parasites. A parasite is a type of germ that needs to live on or in another living being (host). Antibiotics do not work against infections that are caused by viruses (for example, the common cold or flu), or fungi (for example, thrush in the mouth or vagina), or fungal infections of the skin. Each type of antibiotic only works against certain types of bacteria or parasites. This is why different antibiotics are used to treat different types of infection. The main types of antibiotics include: Penicillins - for example, phenoxymethylpenicillin, flucloxacillin and amoxicillin. Cephalosporins - for example, cefaclor, cefadroxil and cefalexin. Tetracyclines - for example, tetracycline, doxycycline and lymecycline. Aminoglycosides - for example, gentamicin and tobramycin. Macrolides - for example, erythromycin, azithromycin and clarithromycin. Clindamycin. Sulfonamides and trimethoprim - for example, co-trimoxazole. Metronidazole and tinidazole. Quinolones - for example, ciprofloxacin, levofloxacin and norfloxacin. Nitrofurantoin - used for urinary infections. As well as the above main types of antibiotics, there are a number of other antibiotics that specialist doctors or hospital doctors may prescribe for more uncommon infections such as tuberculosis (TB). For other severe cases, your doctor may advice you to undergo a test using an upper gastrointestinal endoscopy (esophagogastroduodenoscopy, or EGD) to detect Barrett's esophagus (a change in the cells of the esophagus) which can make a person become more prone to esophagus cancer. Practically, a helpful way to cure GERD is simply avoiding the things that stimulate your symptoms. This includes abstaining from spicy and fatty foods, chocolates, drinks with caffeine, alcohol, and vices like smoking, and taking nonsteroidal anti-inflammatory drugs (NSAIDs) or other medications that trigger GERD symptoms. Stage 2: Continuous Cure It is important that you religiously continue the medications that best suit you as per advice of your health expert because discontinuing the therapy often brings back the symptoms. Your doctor will guide you in the proper timing, frequency of terms or intervals of taking medicines. To balance the effectiveness and safety of various treatments, you have to cooperate well with your doctor. Otherwise your condition will just worsen and even get fatal. Stage 3: Critical Cure At a critical stage of acid reflux cure, complications have been developed and no signs of significant improvements are observed. You may begin taking more frequent medication or switched to a stronger prescribed medicine. The role of a physician is very much needed to check, reevaluate and assess your health conditions, including your diet and lifestyle. Surgical operations may be considered when: o All the possible treatments, remedies, relief and medical cure are not able to remove your symptoms, and if symptoms are directly caused by reflux of stomach juices. o You personally reject oral medication or unable to take medications over an extended period of time and are willing to accept the risks of surgery. o You are suffering from different complications such as asthma, hoarseness, or cough along with incurable reflux symptoms. Essentially, to cure acid reflux today is to know the causes, symptoms and how they come together in your body that affects your system.
