Erectile dysfunction and L- arginine What is Erectile Dysfunction? Erectile dysfunction, formerly called impotence, is the inability to obtain an adequate erection for satisfactory sexual activity. It's more common in men over 65, but it can occur at any age. What's more, taking L-arginine in combination with ribonucleic acid and eicosapentaenoic acid (an omega-3 fatty acid) may reduce recovery time, protect against infection, and promote wound healing after surgery. According to the NIH, there's not yet enough evidence to rate the effectiveness of L-arginine in treatment of migraines, dementia, high blood pressure, male infertility, and the common cold. It's also unknown whether L-arginine can boost the immune system and/or improve athletic performance. Video abstract of original research paper “The impact of hypomagnesemia on erectile dysfunction in elderly, non-diabetic, stage 3 and 4 chronic kidney disease patients: a prospective cross-sectional study” published in the open access journal Clinical Interventions in Aging by authors Omer Toprak, Yasin Sarı, Akif Koç et al. Background: Erectile dysfunction (ED) is common in older men with chronic kidney disease. Magnesium is essential for metabolism of nitric oxide which helps in penile erection. There is little information available about the influence of serum magnesium on ED. The aim of the study was to assess the influence of hypomagnesemia on ED in elderly chronic kidney disease patients. Subjects and methods: A total of 372 patients aged 65–85 years, with an estimated glomerular filtration rate of 60–15 mL/min/1.73 m2, were divided into two groups according to serum magnesium levels: hypomagnesemia, n=180; and normomagnesemia, n=192. ED was assessed through the International Index of Erectile Function-5. Hypomagnesemia is defined as serum magnesium ,1.8 mg/dL. Results: The prevalence of ED was higher among hypomagnesemic subjects compared to that among normomagnesemics (93.3% vs 70.8%, P,0.001). Severe ED (62.8% vs 43.8%, P=0.037), mild-to-moderate ED (12.2% vs 5.2%, P=0.016), abdominal obesity (37.2% vs 22.9%, P=0.003), metabolic syndrome (38.4% vs 19.2%, P=0.026), proteinuria (0.83±0.68 vs 0.69±0.48 mg/dL, P=0.023), and C-reactive protein (6.1±4.9 vs 4.1±3.6 mg/L, P,0.001) were high; high-density lipoprotein cholesterol (48.8±14.0 vs 52.6±13.5 mg/dL, P=0.009), and albumin (4.02±0.53 vs 4.18±0.38 g/dL, P=0.001) were low in the hypomagnesemia group. Serum magnesium #1.85 mg/dL was the best cutoff point for prediction of ED. Hypomagnesemia (relative risk [RR] 2.27), age $70 (RR 1.74), proteinuria (RR 1.80), smoking (RR 21.12), C-reactive protein (RR 1.34), abdominal obesity (RR 3.92), and hypertension (RR 2.14) were predictors of ED. Conclusion: Our data support that ED is related to hypomagnesemia in elderly patients with moderately to severely reduced kidney function. You know, erections begin in the brain. That’s right. Sexy stimulations come in through the senses—the eyes, the fingers, the nose, the mouth—but all those sexy stimulations fall flat if there aren’t adequate levels of testosterone in the brain. That’s right. Testosterone is the male hormone that’s made in the testicles and it circulates all throughout the bloodstream, and it has a variety of very important functions from muscle mass, bone density, the immune system, even longevity. But perhaps its most important function is erections. So, with normal levels of testosterone, all those sexy stimulations cause nerve signals in the brain to coalesce, and then go down the spine to the penis. There, those signals trigger blood vessels to open up, and blood rushes in to the chambers of the penis, filling them up, elongating them, making them wider, and then harder and harder—kind of like pumping up a bicycle tire. And testosterone plays a role below the belt as well, because with testosterone, those blood vessels are conditioned, and they can be more elastic and open up more, allowing for better erections. And when testosterone level is low, those blood vessels become a bit hardened, a bit fibrotic, and they don’t open up as well, and it interferes with the erection. Testosterone is also important in conditioning the skin and nerves of the penis. Very important nerves, I might add. And when the testosterone is low, the sensation just isn’t as intense, and to maintain that erection, you need a reflex of that stimulation—those nerves and the skin of the penis, giving you a reflex for more stimulation to the blood vessels filling the penis back up. So when a guy’s testosterone level is low, he doesn’t have that “on” switch in the brain, and the blood vessels and the nerves of the penis just aren’t working at full speed. So, fortunately, testosterone can be replaced straightforwardly and safely, but it must be done under the supervision of a doctor, and it really should only be used in cases where a guy’s testosterone is actually low, verified by blood tests. Testosterone is not a medication that’s going to make you a superman. If your levels are normal, adding more testosteroone will not do you any good, and in fact, it just shuts off your body’s own natural production. Furthermore, if you’re trying to raise a family, don’t go on testosterone ‘cause it does shut off sperm.
